2 WellStar West Georgia Medical Center 2016 Community Resource Directory Members of the WellStar West Georgia Medical Center team created this Community Resource Directory in response to a need identified through the health system s 2013 Community Health Needs Assessment. With more than 75 categories and 650 resources identified, this directory is intended to be a comprehensive and inclusive list of social-, child-, behavior- and health-related services offered throughout Troup County and beyond. It is possible that some services or organizations were inadvertently overlooked during the development of this directory. 3 Chambers County Department of Family & Children Services 410 9th Avenue South West LaFayette, AL (334) d=chambers Chambers County Department of Family & Children Services offers many programs to assist families including TANF, SNAP and child support resources. Circles of Troup County Office Parks and Rec Center 1220 Lafayette Parkway (706) Circles of Troup County is a nationally recognized program that is a high-impact approach to addressing poverty. They combine best practices in several disciplines including community organizing, case management, SMART goal setting, financial literacy, peer-to-peer counseling and learning, and child/youth development. Participants in Circles must apply and go through an interview process, and once accepted attend a 12-week class that includes a wide variety of concepts including goal setting, financial literacy, "hidden rules" of economic class and more. When they have completed the graduates are matched with two to four middle class volunteers who commit to working with them to reach their goals. Participants are the "Circle Leaders" of this small group and volunteers are "allies." The group meets weekly Thursdays, where other volunteers provide free meals an free childcare. The weekly classes include education meetings among the 'matched circles' and Circles planning sessions. Meetings on the first, second and fourth Thursdays of every month are open to visitors who are considering participating or volunteering with Circles or who want to know more about what they do. Circles of Troup County Thursday Meetings Troup Baptist Association 1301 Washington Street (706) The meeting for Circles of Troup County meets every Thursday at 5:30 for a free meal with the meeting from 6:15 to 7:45 PM. Meetings on the first, second and fourth Thursdays of every month are open to visitors who are considering participating or volunteering with Circles or who want to know more about what they do. Emmaus Women's Shelter 321 Greenville Street (706) Emmaus Women's Shelter provides services and assistance such as emergency shelter, food, housing, counseling and job training. Georgia Food Stamp Program Supplemental Nutrition Assistance Program (SNAP) Troup County DFCS Office 1220 Hogansville Road (877) SNAP is a federally-funded program that provides monthly benefits to low-income households to help pay for the cost of food. A household may be one person living alone, a family or several unrelated individuals living together who routinely purchase and prepare meals together. Page 1

4 Heard County Women, Infant and Children Nutrition Center 1191 Franklin Parkway Franklin, GA (706) Women, Infant and Children Nutrition Center program helps to provide low income pregnant, postpartum and breastfeeding women, infants and children up to age five with nutritious food to supplement diets. They also provide information on healthy eating, breastfeeding promotion and support and referrals to healthcare. Highland Baptist Church Food Bank 409 Askew Avenue Hogansville, GA (706) Church food pantry. Please contact Pastor Terry Rainwater for more information. Hogansville Empty Stocking Fund Administered by the Hogansville Pilot Club Hogansville, GA /info/?tab=page_info This project, sponsored by the Hogansville Pilot Club buys toys and clothes for needy children in the Hogansville area at Christmas. LaGrange Personal Aid Association 416 Pierce Street (706) LaGrange Personal Aid Association provides a variety of programs including LaGrange Personal Aid which provides emergency assistance whether from disaster or illness by helping with rental, mortgage and utility payments if the main income earner is hospitalized or out of work and seeing a doctor. The Interfaith Food Closet provides temporary and emergency food assistance to those who have proof of need. The LaGrange Empty Stocking Fund provides Christmas gifts for needy children from birth to 10 years of age. LaGrange Personal Aid Association Empty Stocking Fund 416 Pierce Street (706) The LaGrange Empty Stocking Fund provides Christmas gifts for needy children from birth to 10 years of age. LaGrange Personal Aid Association Interfaith Food Closet 416 Pierce Street (706) The Interfaith Food Closet is administered in coordination with local churches. This program provides temporary and emergency food assistance to those who have proof of need until government aid is required or medical/work circumstances change. Page 2

5 Meriwether County Women, Infant and Children Nutrition Center 51 Gay Connector Greenville, GA (706) Women, Infant and Children Nutrition Center program helps to provide low income pregnant, postpartum and breastfeeding women, infants and children up to age five with nutritious food to supplement diets. They also provide information on healthy eating, breastfeeding promotion and support and referrals to healthcare. Randolph County Department of Family & Children Services 865 Hillcrest Avenue Wedowee, AL (256) d=randolph Randolph County Department of Family & Children Services provides many programs to assist families including TANF, SNAP and child support resources. Saint Peter's Catholic Church St. Vincent De Paul 200 LaFayette Parkway (706) Through the St. Vincent de Paul Society, this ministry provides food and clothing to those less fortunate in the community. Salvation Army LaGrange Corps 202 Church Street (706) The Salvation Army LaGrange on a day to day basis provides to families in crisis situations, assistance in the forms of clothing food, furniture, medical prescriptions, rent and utilities. During the summer the organization provides a weeklong camping experience in the north Georgia Mountains to underprivileged youth. At Christmas the Organization provides new clothing, new toys and wholesome food boxes to low-income families who otherwise would not afford these items for Christmas. All request for assistance are evaluated through a formal interview process. Troup County Department of Family & Children Services 1220 Hogansville Road (770) to report child abuse CPS intake center -(855) Troup County Department of Family & Children Services investigates child abuse, finds foster homes for abused or neglected children, assists with childcare costs, job training and other support services for low income parents and children.
