2 Quick Demographic Facts There are approximately 23,000 incarcerated men and women in Colorado. The average annual cost to taxpayers, per inmate, is estimated at over $32,000. Approximately 840 incarcerated men and women are released monthly from Colorado prisons. 70% of those released return to the Denver metro area. Three year Colorado recidivism rates average 49.6%, exceeding the national average of 39%. Second Chance Center serves, on average, 800 client visits per month. Many of them are outside of current grant parameters. Since its inception, Second Chance Center recidivism rates average < 11.0%. Source: 2013 report from Colorado Criminal Justice Reform Coalition.

3 Second Chance Care Management Staff Hassan A. Latif, CAC II, is the Founder and Executive Director. He is a nationally recognized Cognitive Behavioral Change Facilitator (NCTI). After serving 18 consecutive years in Colorado penitentiaries, Hassan embarked on a career of helping others avoid prison. Hassan is the author of Never Going Back: 7 Steps to Staying Out of Prison, which is read in prisons nationwide. Sean Ahshee Taylor is the Deputy Director. He is a former teen gang member, convicted at age 17 and sentenced to life in prison without parole. Upon his incarceration, Sean committed his life to good works and bettering himself and others. After serving 22 years, his sentence was commuted by Governor Ritter in Sean is also a Certified Personal Trainer. Adam Abdullah is a Care Manager. He brings a wealth of wisdom and experience, much of which was garnered over the course of 33 years served in the U.S. Federal prisons. While incarcerated, he educated himself, earning two Associates degrees. Upon his release, he continued his education, earning a Bachelor s Degree in Human Services in 2013, at the age of 68. Sandy Bishop Maynard is the Executive Assistant and has been on staff since October 2014, bringing an organizational expertise to SCC just in time to meet the demand of a rapidly growing agency. With a personally giving nature, and many years experience supporting marginalized populations in need, she plays an integral part on Second Chance Center's Care Management Team. Rachel M. Burns is the Coordinator of the BCPC (Breakthrough Career Pathways Collaborative) Program at the Second Chance Center she started as an intern here, She is a representative of the exception to the rule a product of the juvenile justice system who was able to reverse the impact of such indoctrination, and find success as a well-balanced, healthy adult. The experience, and knowledge allows her to coach and mentor clients whom have been down the same path. She is currently in school finishing up her studies in the human services field at Metro State University. She recently gained her associates degree (May 2015) at Community College of Denver. Dana Jenkins Care Manager and Intake Specialist. After serving 20 consecutive years in U.S. Federal Prisons, she has dedicated herself to help people stay out of the system by helping them with, clothing, food, support, and sometimes just an ear to listen. While incarcerated she earned her Cosmetology License and helped others obtain their license. She was also on the Hospice Team. Wanda Harrison is a Senior Care Manager, she moved to Colorado from Salt Lake City Utah, where she attended Metro State University of Denver and earned a Bachelor s of Science Degree in Human Service. Wanda has several years of experience as a human service professional and has worked with high risk youth, adults dealing with substance abuse and mental health conditions. Wanda s experience has cultivated into working with formerly incarcerated men and women. She is passionate and dedicated to the success of men and women returning to the community. Kim Kendrick Care Manager and Outreach Specialist. She has worked in Customer Service for more than 20 years. She has been involved with helping others through several different organizations since moving to Colorado in She has served on several Mission teams to traveling the world helping those less fortunate. She is dedicated to serving those that are in need. She is very caring in nature and is always looking to give a helping hand. Robert Battle Maintenance Specialist, he has served 16 consecutive years in US Federal Prisons. While Incarcerated he received several certificates of completion for Social Behavior classes, Stress Management and also Communication skills. He enjoys talking with others and sharing his story with those that it might help. He is an avid listener and likes to problem solve with people to keep them out of the system. He is very caring in nature and is always looking out for those around him.

4 What We Do SCC guides clients in the discovery of influences and behaviors that have contributed to criminal conduct, including addictions, problematic cognitive mental processes, trauma, abuse and family dysfunction, and socio-economic stress, lacking opportunity, bias SCC provides pre-release transition planning and post-release counseling to help clients claim individual responsibility and develop pro-social skills. SCC works with clients to develop a parole plan and measure their career development. SCC provides job search, job training and job placement, with a 72% job placement rate, 30% credential achievement rate and 11% recidivism rate.

5 What We Offer The goal of Second Chance Center is to provide critical transition services and support to men and women after periods of incarceration. Traditional approaches to re-entry transition have widely varying outcomes. Second Chance Center offers clients multiple services to assist their transition, such as: Behavioral change curriculum based upon Hassan A. Latif s Never Going Back: 7 Steps to Staying Out of Prison. Employment Preparation Computer Classes, Resume Assistance Transportation Assistance Addictions Counseling and Support Groups Employment Referrals and Placement Limited housing assistance Fitness Program A Weight Lifted Job supplies, including clothing, tools, uniforms, etc.

6 Tenets of Our Approach SCC Counseling Approach The level of an individual s personal investment of focus, effort and commitment will determine the success of his or her re-entry This effort, focus and commitment will also determine how one s freedom will be maintained Individual and group cognitive counseling and mentoring is essential to success

7 Giving Back to the Community The community service arm of SCC is FORCE (Former Offenders Reintegrated for Community Enrichment). It is comprised of former offenders who commit to being substance abuse and criminal conduct-free, by choice, not sanction. They participate in restorative justice activities, promote legislative changes, and speak with employers and landlords about the benefits of hiring or housing transitioning persons.

8 SCC Actions to Address Collateral Consequences Collateral consequences are those barriers (to employment, housing, etc.) that continue to negatively impact someone who has already completed their sentence. SCC staff and FORCE have testified numerous times to address this issue. In 2014, such testimony led to House Bill 1355, which set aside funds from the Department of Corrections budget for community-based organizations to help individuals address these barriers. SCC was awarded one of four grants.
