2 Elyse DeGroot- Who Even Are You?. Elyse DeGroot, a Massachusetts native, has lived in Falmouth and worked on Cape Cod since Elyse has been the Director of Community Relations and Marketing for Duffy Health Center since 2007, working previously as the Director of Development for Elder Services of Cape Cod and the Islands and as the Director of Senior Services for the town of Barnstable.

3 Elyse has been an active volunteer for her church, the United Way, Falmouth Public Schools, Falmouth Band Parents and has served on the Falmouth Service Center board of directors since A graduate of Union College, Elyse received an MSW from Boston University and an MBA from Simmons College Graduate School of Management

4 Duffy Health Center MISSION: The mission of the Duffy Health Center is to prevent and reduce homelessness on Cape Cod through community collaborations and the provision of integrated medical, behavioral health, case management and housing services to persons who are homeless or at risk of homelessness. In FY 2012 Duffy served 3,200 individuals, an increase of 20% since moving into our new building.

16 Reviewed gifts from Comprehensive Campaign Some were one time gifts, others were multi year gifts Assigned them to the appropriate society or circle category Published annual report with explanation

17 Duffy Health Center is pleased to announce the creation of a giving society in honor of Arthur Bickford, MD and Judy Best, Nurse Practitioner, Duffy s founders. The Bickford-Best Society is being established to clearly recognize their generosity of spirit and concern for the wellbeing of all citizens of Cape Cod and to ensure that Cape Cod s homeless and at-risk population continues to receive the care they deserve. Bickford-Best Society membership includes recognition in the annual Report of Donors, increased contact with Duffy Health Center leadership, as well as invitations to special events to learn more about new and exciting programs and services in which Duffy is involved.

18 An annual gift of $2,000 is required for basic membership in the Bickford- Best Society. Within the Bickford-Best Society there are two additional Circles donors can join depending upon their giving level Founders Circle- gifts of $1000-$1,999 Directors Circle-gifts of $500- $999 For gifts received by (date) we will be creating a special Charter Member recognition designation
