3 Welcome Meadowside is a friendly and welcoming care home situated close to North Finchley High Street, North London. Our expert team provide a 24-hour specialist residential and dementia care service. At Meadowside, we ensure care and kindness are at the heart of everything we do and we respect the views and wishes of everyone. By encouraging residents to be involved in the decisions made about their care, we are able to ensure their needs are met by creating a care and support plan that is unique to them. We take great care to ensure that every person at Meadowside feels at home, so they can live life to the full. As soon as you walk into Meadowside you feel at home. 4 Purpose built for care Meadowside is a popular and spacious home providing care and support for 68 people across three floors. The home offers a comfortable, lively and caring environment where people are encouraged to add homely touches to their individual rooms. Each bedroom has an en-suite, phone and TV points and a 24-hour call system. The recently refurbished indoor communal areas provide plenty of space for socialising and the beautiful and peaceful outdoor space is ideal for enjoying some fresh air. The layout of the home has been designed to support and enable a person living with dementia to live well. We include features throughout the home to make it easier for people to find their way around and engage in purposeful activities which stimulate the senses. The home also has a BBQ area, a hairdressing salon for those who enjoy a spot of pampering, a tea shop and onsite parking. Meadowside has a very homely feel to it. It has proved to be the perfect place for my mother who previously found it very hard to settle elsewhere. MR (Family member) 4

5 Our approach to care Meadowside provides 24-hour residential and dementia care and support for people with a wide range of healthcare needs including those with long-term conditions and physical disabilities. We understand that every person is unique and will have their own needs and ways of doing things. We aim to ensure that these needs are met in a truly person-centred way, so each person has their own personal care and support plan specially tailored around them. As part of the care and support provided, we have regular visits from local GPs and other healthcare professionals, such as chiropodists, opticians, dieticians and physiotherapists. We are proud of the exceptional care delivered at Meadowside, and indeed all the Trust s homes. What makes us special, is the time and attention we pay to really get to know people; what s important to them and their families in daily life. This unique approach is what we believe sets us apart. 7 Living at Meadowside So that we can fully understand a person s care needs and lifestyle choices, we match every resident with a named key worker with whom a close relationship of trust and support can be built. We want all residents to be involved in making decisions about the running of the home and we hold regular meetings so they can provide comments and feedback. We welcome family and friends to get involved too! At The Fremantle Trust we believe in active living, which is to say that there is the opportunity to participate as fully as possible in whatever daily living activities individuals are able and would like to take part in. Our team is on hand to help with the things some residents find more difficult to do, in order to help maintain maximum independence and be more fully engaged in life. Our people The team at Meadowside takes great pride in knowing they are making a difference to the quality of life for people and their desire, kindness and understanding to support others is what makes the home, and indeed all the Trust s homes extra special. We are committed to investing in our people with ongoing learning and development, so they are equipped with the necessary skills they need in order to deliver the highest standards of care possible in a kind and caring way. 7

8 Enriching lives Everyone at Meadowside is supported to live as fulfilling and enriching a life as possible. Our leisure and lifestyle organisers work in partnership with residents to organise a variety of lively and interesting one-to-one and group activities and these might include pursuing personal hobbies, talks and in-house entertainers. The home has excellent links within the local community and children from a nearby school visit regularly to take part in activities. As a dementia friendly community, we also offer rewarding activities including cognitive stimulation therapy, singing and reminiscence activities to support residents living with dementia. Coming together at mealtimes for tasty, nutritious food is not only wonderful for maintaining good physical health, but is also great for social interaction. We provide a wide choice of delicious and tempting meals, and food is cooked freshly on-site, using the highest quality ingredients, locally sourced wherever possible. 8

We listen to the views and concerns of those we care for; involve them, and their families, carers and friends, in any decision-making about the type of care and support they receive. In addition to both resident and family meetings, we also send out a next-of-kin survey in which we ask for feedback on how we are doing, and using the feedback we get, enhance the quality of our services. For us, it s about celebrating the uniqueness in everyone and providing person centred care. 10
