1 The Urbandale Club s 2016 Slate of Nominees include five individuals interested and eligible for our four open Board Trustee positions with terms running from July 1, 2016 through June 30, Members of the Urbandale Club will be able to vote to determine who fills the open Board Trustee positions at our Annual General Meeting on June 15, 2016 at 7:00 PM in the Urbandale High School, Media Center. The nominees are presented below in alphabetical order by last name. Jeff Braaten Residence Valdez Drive, Urbandale, IA Three U8, U11 (JUSC), U14 Volunteer Coach, Urbandale Club (F licence) (2013, ) JUSC Tournament Volunteer (2013, 2015) Beautiful Savior Lutheran Church WDM Secretary ( ) Urbandale Middle School PTO Board Secretary (2014 Present) Business Analyst, Hirsh Industries (2003 Present) Demand planning for all manufactured products. Utilizing advanced statistical forecasting models, sales information, and business intelligence. Collect and analyze data of customer needs, and buying habits to identify potential markets and factors affecting product sales. Assistant Manager, Kohl s Department Store ( ) Responsible for ensuring total store execution according to Kohl's Best Practices. Executive responsibilities include providing leadership for the store in the following areas: Associate development, communications, and analysis of business operations, expense management, and general management of the store as part of the store management team. Bachelor of Arts Sociology (minor in psychology) I have had at least one child in the club for the past 8 years. I now have three children utilizing the soccer club. I feel that it is time to give back. I have vast experience with board and council positions and understand how to work with other members to benefit the greater good.

2 Urbandale Club Jeremy Epperson Residence 3300 Melanie Drive, Urbandale, IA Two U6 and U9 Volunteer Coach, Urbandale Club (past four years) My soccer background includes: playing competitive soccer throughout my childhood and high school years. I played on a select club from the Omaha area that competed throughout the Midwest and West regularly. Being a part of a soccer club taught me many valuable lessons that I still utilize today. I have d as an account manager, senior sales representative, sales manager, and most recently as a real estate appraiser. At times throughout my career, I have been responsible for the management of over 25 sales reps, managed contracts with fortune 500 companies, and worked closely with executives all the way to the end user of my services. I feel strongly that the Urbandale Club is an important part of our community and one that provides many benefits to the community on and off the field. The Urbandale Club is an elite soccer program that provides coaches with all of the necessary tools to teach children how to properly play the game. It provides an option for players in the community of all ranges of abilities. It has avenues for the elite player to possibly advance their soccer career through Sporting Iowa to the player who simply enjoys playing the game with their peers in a less competitive atmosphere. The club teaches many valuable lessons that will help develop kids into great players, and hopefully great assets for the city. I am requesting to be a member of the Board of Trustees for the Urbandale Club, because I think it is a very important organization that provides an excellent service to our community. I feel that I would bring an open mind, a love of the sport, and a desire to continually improve the experience for our parents, coaches, volunteers, and most importantly our players. 2

3 Urbandale Club Victor Fageroos Residence st Street, Urbandale, IA Two U11 and U14 (both JUSC) Volunteer Coach, Urbandale Club (E Licence) ( ) Board Trustee, Urbandale Club (2013 Present) Served on the Urbandale Club board as the U9/U10 age group coordinator for three years and I am currently president of the club. During my time on the board I have been working on establishing fees to better help the club operate in a fiscally responsible manner. I have also been part of a marketing committee to grow our membership. I have also organized two successful membership cookouts. During my time as president, I have worked on moving two of our recreational programs to newly formed ISA leagues, handling parking lot hassles, and transitioning from a poor apparel supplier to a better apparel supplier. I am also working with the City of Urbandale for soccer complex improvements and negotiating a lease for the club. Volunteer coach basketball and volunteer as a Boy Scout Leader ly, I am an engineering associate. I have a two year degree in mechanical design engineering as well as continuing education certificates in a variety of engineering processes, including lean sigma. I am also listed as an inventor on six U.S. patents. The work at my current employer allows me to see a variety products so work is never boring. My goal is to make Urbandale Club and it s partners the best club in the state for kids to play soccer. I hope that all kids and parents in our programs have the best experience possible and get to meet new friends through soccer. I hope to continue listening to different ideas and helping make the best decisions for our members. I know first hand that the club needs good volunteers and I hope that I have been a good volunteer for the club. 3

4 Urbandale Club Janet Miller Residence th Street, Urbandale, IA One U5 Throughout high school and early in my working career I spent a lot of time volunteering. I have volunteered with Special Olympics, Salvation Army, at our Church just to name a few. I started playing soccer when I was 6 years old and played until I was 14. I was on several teams including tournament teams through the 8 years I played. I am currently the general manager at Carey Cleaning Solutions. We recondition semis for resale and do industrial onsite cleaning. I currently manage 7 employees. In the past I have managed up to 10 at one time. I have completed the Master Graduate with Rapport Leadership International by graduating from Leadership Breakthrough 1, Leadership Breakthrough 2, and Power Communication. For the past several years I have been working in sales and business development. I enjoy getting to play a key role in actually making things happen. My core values are integrity, open honest communication, and service. This is my daughter's first year in soccer and SHE LOVES IT! She absolutely loves playing soccer and so did I when I was younger. I have a strong desire to be active in the community and I can't think of any better way than to also be contributing to my daughter's passion for soccer. I know she will be playing soccer for a long time. I want to get back into contributing to the community. I think it is important to teach my kids that it isn't about just showing up on game day. There is so much more work put in to create the fun environment they get to be apart of. I believe my involvement on the Board of Directors is just one way for me to show this to them. Being on the board of directors is also a great way to contribute to the community and help continue improving the program. 4

Interested in U7/U8 group coordinator position. I feel USC is one of the best soccer programs in our state and I am happy with progress I see in our children. With the tech world we live in our children would be glued to TV or ipad forever unless we take them outside. I am very impressed and glad to be part of such a great soccer organization and look forward in helping more. 5
