1 Newlife Fertility Price List Our objective at the Newlife Fertility clinic is to instill a deep sense of quality and personal touch to the level of care experienced by every patient. We ensure that treatment costs are kept to a minimum and you are only advised the absolute necessary tests relevant to you the individual. Our aim at Newlife is to guide and support you through a very personalised Fertility Journey and to help you achieve your ultimate goals. CONSULTATIONS (All consultations are with an IVF Specialist Newlife is a Doctor led clinic) Free 15-minute Consultation for Non Newlife Patient Includes 1 appointment only, Should you wish to become a patient, an Initial Consultation will need to follow NO CHARGE Initial Consultation deposit is required at time of booking and this will be taken off the total cost of the appointment Follow Up Consultation NO CHARGE One appointment offered after an unsuccessful IVF /ICSI, frozen embryo, egg donation, surrogacy, or 3 failed IUI cycles Follow Up Consultation 120 Telephone or In house Telephone Follow Up Consultation 15 minutes 50 Applies to all calls that involves speaking to a clinician Follow Up Consultation After Fertility Assessment 150 If wishing to proceed with treatment Should you require a copy of your medical notes there is a consent form that must be signed by both parties if applicable and there is a 40 administration fee. We legally have 40 days to provide you with your copy of notes but will always endeavor to get these to you by recorded delivery as soon as possible
2 FERTILITY ULTRASOUND SCANS AND DIAGNOSTIC TESTS Initial Baseline Scan 150 To assess suitability for treatment and determine treatment plan Ovulation Induction Scan 150 In Treatment Scans This Includes all scans in IVF, IUI & FET Cycles Fees apply if cycles are abandoned prior to procedure NO CHARGE Pregnancy scan 150 Female Fertility Assessment 390 Includes Hormone Blood test with AMH (ovarian reserve) days 1-3 of cycle, Scan & Mini Consultation mid-cycle Male Fertility Assessment 200 Includes Semen Analysis done on site with 2-5 days abstinence period and Mini Consultation once results available Couples Fertility Assessment 480 Includes Female Hormone Blood Tests and Scan, Semen Analysis and Mini Consultation once all results available Hyfosy 475 Tubal Patency Test between days 6-10 of cycle Saline Hysterosonogram 475 Assessment of Uterine Cavity days 6-10 of cycle
3 STIMULATED IVF CYCLE IVF 3500 This includes monitoring scans, nurse consultation, injection technique, theatre charges, anesthetist fees, egg collection under sedation, embryo transfer, embryo glue and first pregnancy blood test (BHCG only) If sedation is required for embryo transfer this is 350 extra ICSI 1100 Not required for all patients Blood Tests FSH, LH, Oestradiol 95 These are required every couple of days over a 2-week period approximately Blood Test Progesterone 45 This is required at times during the treatment, on egg collection day, embryo transfer day and on some occasions a few days after embryo transfer. This is also chargeable upon a positive pregnancy test as will be added to the BHCG test from that day Stimulation Medication Estimation per cycle This is only an estimate as dosages vary amongst patients (Medications cannot be returned by law and therefore will not be subject to refund) HFEA Fee 80 This applies to each treatment cycle even if cycle becomes abandoned at any point Please note there is a recipient fee per cycle, for any donor egg or surrogacy cycles performed at Newlife of 1000
4 FROZEN EMBRYO TRANSFER CYCLE Frozen Embryo Transfer 1100 This includes monitoring scans, embryo thaw, embryo transfer and first pregnancy blood test (BHCG only) If Sedation is required this is 350 extra HFEA Fee 80 This applies to each treatment cycle even if cycle becomes abandoned at any point Blood Test Progesterone 45 Approximately 3-4 progesterone tests are required during an FET cycle This is also chargeable upon a positive pregnancy blood test as will be added to BHCG from that day Medication This may vary depending on whether cycle is medicated or non-medicated and dosages, please ask nurses for costs related to your specific treatment (Medications cannot be returned by law and therefore will not be subject to refund) Please note there is a recipient fee for any donor egg or surrogacy cycles performed at Newlife, if embryos are already frozen and in storage with us the recipient fee is 600 per cycle INTRAUTERINE INSEMINATION Intrauterine Insemination 800 This includes monitoring scans, semen preparation with analysis and Insemination Procedure HFEA fee For Donor Insemination Applies to all donor IUI cycles and is chargeable even if cycle is abandoned Medication Approx This does not include stimulated IUI, includes progesterone pessaries & possible trigger shot if required (Medications cannot be returned by law and therefore will not be subject to refund) IUI Package 1850 You are entitled to a maximum of three cycles, however once a clinical pregnancy (fetal heart beat) is present, you will not be entitled to further cycles within this package. Three cycles should be completed within 12 months. Please note, in the event of abandoned cycle prior to insemination, an abandoned cycle fee relating to the number of scans performed is applicable. This fee is non refundable once treatment begins and cannot be used against any other treatment

Please contact clinic for further information Lab services fee of 200 only applies if SSR done off site. Storage fee is 300 per year Sperm Freeze with Analysis including first year storage 410 In order to freeze you are required to have a virology blood test done at Newlife and complete consent forms, these must be done prior to the freeze. 10 HOLISTIC TREATMENTS (Package Prices available upon request) Counselling Chargeable direct to counsellor Counselling is mandatory at Newlife for anyone using any donor gametes, eggs, sperm or embryos and for all surrogacy cycles. All parties are required to attend. It is also recommended for all other patients undergoing treatment but not mandatory in their case Reflexology / Hypnotherapy / Fertility Abdominal Massage First Appointment of 1.5 Hours 85 Follow Up Appointment of 1 Hour 60 Acupuncture Initial Appointment of 1 Hour 15 Minutes 80 Follow Up Appointment of 45 Minutes 60 Acupuncture & Massage of 1 Hour (Follow Up Patients Only) 75 Pre- Embryo Transfer Acupuncture of 45 Minutes 85 Post Embryo Transfer Acupuncture of 45 Minutes 85 Pre and Post Embryo Transfer 45 Minutes each session 160 (An additional fee of 10 will be added to pre and post transfer embryo transfer acupuncture if you have not been seen before by our acupuncturist) Nutrition Individual First appointment of 1.5 Hours 80 Individual Follow Up of 1 Hour 50 (Once you have had 2 sessions any further follow ups will be charged at 40 for an individual and 60 for a couple)
