Current levels and recent trends in health inequalities in the EU: Updates from the EU Report Anne Scott London Health Observatory Expert Working Group on Social Determinants and Health Inequalities Luxembourg, June 2012

Introduction Geographic inequalities in mortality levels and trends - Between EU countries - Between 268 regions (NUTS2) of the EU - Between regions within countries Indicators Life expectancy at birth, age 50, age 65 Age-specific death rates, infant mortality rate Summary measures of inequality: Highest compared with lowest: Absolute gap, rate ratio Gini coefficient of inequality Source: Eurostat database Socio-economic inequalities in self-perceived health - Social determinants: Education, income, material deprivation Indicator: % of population reporting poor or very poor general health Summary measure of inequality: Slope index of inequality Source: EU Survey of Income and Living Conditions (EU-SILC) 2010

Slope indices of inequality (SIIs) for poor or very poor general health and material deprivation, 2010 Men Women % with poor or very poor general health Slope index of inequality = 20.5 (95% Confidence interval: ) Data points Relative rank from high to low material deprivation % with poor or very poor general health Slope index of inequality = 21.6 (95% Confidence interval: ) Data points Relative rank from high to low material deprivation

Estimated odds of reporting poor or very poor general health by socioeconomic indicators, 2010 Education One variable in model: Deciles of EU income* Three variables in model: Material deprivation Items lacking * Purchasing power standard

Summary: Inequalities in mortality Inequalities in life expectancy at birth between EU member countries have decreased since 2000 for women, but changed only slightly for men This reflects decreases in inequalities in the mortality of children aged <15 that are partly offset for women, and fully offset for men, by increases in inequalities at older ages, especially at ages Inequalities in mortality between regions across the EU as a whole are larger than those between countries but trends in inequalities between EU regions are more muted: convergence between countries for women has been offset by divergence within countries At the country level, the trend in regional inequalities in mortality differs between EU member states, increasing greatly in some and decreasing markedly in others; and sometimes (e.g. Poland, Greece) exhibiting divergent trends in different age groups

Summary: Inequalities in general health The 2010 EU-SILC individual-level data reveal gradients in self-perceived general health by education, income and deprivation The steepest gradients are those between material deprivation and poor or very poor general health The gradients for men and women are similar; but they are closest for the relationship between household income and poor or very poor general health and the association between low education and poor health is more marked (steeper gradient) for women than it is for men The multivariate analysis suggests that, for each of the three socioeconomic indicators, after adjusting for the other two: the elevated risks for income are substantially reduced those for education are somewhat smaller the risks associated with items of deprivation remain high indicating the strength of the relationship between material deprivation and poor health among individuals living in the EU
