Price List March 2018 Fertility treatment, gynaecology, male and female health care At the Agora we specialise in diagnosing and treating the causes of infertility and offer bespoke fertility services to our patients. We aim always to be completely transparent about our fees and have created packaged costs for both fertility investigations and treatments. Packaging our fees in this way means patients receive the most competitive price for their treatment and there are no hidden extras. 2 Investigation Packages Male and Female Fertility Screening Package Includes ovarian reserve blood test, pelvic ultrasound scan, semen assessment, chlamydia test and doctor s consultation. Single Female Fertility Screening Package Includes ovarian reserve blood test, pelvic ultrasound scan, chlamydia test and doctor s consultation. Enhanced Male Fertility Screening Package Includes semen assessment, DNA fragmentation, HBA sperm binding, antisperm antibodies (Or 500 including a consultation with a doctor). Enhanced Male and Female Fertility Screening Package Includes ovarian reserve blood test, pelvic ultrasound scan, thyroid function, chlamydia for the female as well as the enhanced male package as above and a doctor s consultation. Same Sex Couple Fertility Screening Package Includes both partners having baseline scans, ovarian reserve blood tests, chlamydia tests and doctor s consultation. Egg Donor Screening Package Includes ovarian reserve blood test, pelvic ultrasound scan, all screening tests, one counselling session and doctor s consultation. Sperm Donor Screening Package Includes semen assessment, freezing up to 3 samples, all screening tests, one counselling session, nucleic acid test (NAT) release screen, one year s storage and doctor s consultation Common Blood Tests Female Endocrine Profile (FSH, LH, Oestradiol) 115 Male Endocrine Profile (FSH, LH, Testosterone) 160 Thyroid Profile 85 AMH 92 Oestradiol 39 Progesterone 45 Testosterone 42 Prolactin 45 Blood Group & Antibodies 65 Full Blood Count 40 Biochemistry Profile (U+E, LFT) 76 Chlamydia (Urine) 75 CA Other tests are available on request, please contact us for further details. 3 Treatment Packages Our individualised packages of care are designed so each patient receives the best possible treatment. We will provide you with your own detailed cost plan prior to starting treatment, but in the meantime the following is a guide as to how much your recommended treatment cycle may cost. Follicle Tracking Follicle Tracking Cycle (Per monthly cycle) 495 What s included in my Single IUI/DI Treatment? Intrauterine Insemination (IUI) Treatment Your treatment package includes: a nurse consultation, viral screening, rubella test, one counselling session, HFEA fee if DI, ultrasound monitoring of cycle, sperm preparation and insemination, pregnancy scan at 7 weeks. What s included in my Multi-Cycle IUI/DI Treatment Package? Your three treatments include: a nurse consultation, viral screening, rubella test, one counselling session, HFEA fee for each cycle if DI, ultrasound monitoring of each cycle, sperm preparation and insemination within each cycle, pregnancy scan at 7 weeks. Multi-cycle packages must be completed within six months of your first cycle. Please be aware there is no refund due if you do not complete all three cycles. IUI - Intrauterine Insemination (One cycle using partner s sperm) Excludes medication cost and follow-up consultation. IUI Multi-Cycle Package - Intrauterine Insemination (Three cycles using partner s sperm) Excludes medication cost and follow-up consultation. DI - Donor Sperm Insemination (One cycle using donor sperm) Excludes medication cost, imported donor sperm and import costs, known donor screening costs and follow-up consultation. 4 What s included in my IVF Cycle Package In Vitro Fertilisation (IVF) Treatment Your treatment package includes: a nurse consultation, viral screening, rubella test, one counselling session, HFEA fee, ultrasound and endocrine monitoring of cycle, sperm preparation and IVF insemination, egg collection under sedation, blastocyst culture, EmbryoGlue, embryo transfer and pregnancy scan at 7 weeks or follow-up consultation within 3 months. What s included in my Frozen Embryo Transfer (FET) Treatment Package Your treatment package includes: a nurse consultation, HFEA fee, ultrasound monitoring of cycle, blastocyst culture if required, embryo thawing, EmbryoGlue, embryo transfer and pregnancy scan at 7 weeks. Your treatment is specifically tailored to your needs and may not include all of these items. IVF Cycle Package Excludes medication costs and any additional extras not specified. ICSI Cycle Package As IVF package plus ICSI. Frozen Embryo Transfer Package Excludes medication costs, follow-up consultation and any additional extras not specified Abandoned Cycles Abandoned IVF Cycle (Prior to egg collection) 600 Fertility Preservation Treatments What s included in my Egg Freezing Package Your treatment package includes: a nurse consultation, viral screening, one counselling session, HFEA fee, ultrasound and endocrine monitoring of cycle, egg collection under sedation, vitrification of eggs plus storage for 2 years and a follow-up consultation. What s included in my Egg Thawing Package Your treatment package includes: a nurse consultation, HFEA fee, ultrasound monitoring of cycle, thawing of the eggs, ICSI, Embryoglue, embryo transfer and pregnancy scan at 7 weeks. Egg Freezing Package Excludes medication costs and any additional extras not specified. Egg Thawing Package Excludes medication costs, follow-up consultation and any additional extras not specified Surgical Sperm Retrieval What s included in my Surgical Sperm Retrieval Your treatment includes: a nurse consultation, procedure under sedation, laboratory services post retrieval, freezing plus 1 year s storage and a follow-up consultation. 6 Information For Egg Donor Recipients You may have a friend or relative willing to become a known donor for you. If not, and you require an unknown donor, you can join the waiting list at the Agora. There is a 300 non-refundable fee to join this list. The wait for a donor egg can vary between 6-18 months depending upon availability. Please be aware that you may be matched with an altruistic donor or egg sharer, prices are shown below. Egg Donation What s included in my Egg Donor Treatment Package Your treatment package includes: a nurse consultation, viral screening, rubella test, one counselling session, HFEA fee, ultrasound and endocrine monitoring of both donor and recipient cycles, sperm preparation and IVF insemination, egg collection under sedation, blastocyst culture, EmbryoGlue, embryo transfer and pregnancy scan at 7 weeks or follow-up consultation within 3 months. Egg Donation Package 1 (IVF recipient from altruistic egg donor) plus 1195 if requires ICSI. *Egg Donation Package 2 (OD same-sex couple) plus 1195 if requires ICSI. *Egg Donation Package 3 (Known donation) plus 1195 if requires ICSI *NOTE: Egg donor screening tests are not included, please see separate costs. 7 Surrogacy Surrogacy is when a woman carries a baby for a couple who are unable to conceive or carry a child themselves. It is a complicated treatment with most arrangements co-ordinated by a surrogacy agency. If this is something you are considering, you must obtain independent legal advice prior to starting treatment. Costs stated do not include surrogate expenses or legal costs. Surrogacy Treatment What s included in my Surrogacy Treatment Package Your treatment package includes: a nurse consultation, viral screening, rubella test, one counselling session, HFEA fee, ultrasound and endocrine monitoring of donor and / or surrogate cycle, sperm preparation and IVF insemination, egg collection under sedation, blastocyst culture, EmbryoGlue, embryo transfer and pregnancy scan at 7 weeks or follow-up consultation within 3 months.
