4 Venogram of a left ovarian vein Colour doppler image of a refluxing left ovarian vein The Whiteley Protocol In order to achieve a consistent and optimal standard of care, the clinic has developed The Whiteley Protocol, a formal protocol for all staff to follow outlining how all patients should be investigated and treated standard of excellence We employ specialist Vascular Technologists/Sonographers to work alongside our consultant surgeons at all times, which reduces the risk of recurrent varicose veins As our research has shown, the biggest causes of recurrent varicose veins are: Inadequate diagnostic scanning, resulting in: Veins being missed at the time of treatment Incompetent perforator veins not being treated Pelvic vein reflux not being diagnosed Regrowth of veins after stripping under general anaesthetic The Whiteley Protocol involves the complete patient pathway, from initial consultation, investigations and through all phases of treatment until discharge. It is constantly monitored and updated on the results of our research, treatment developments, experience, patient feedback and new developments coming into the world of venous surgery. Only the doctors and sonographers working at The Whiteley Clinic are taught The Whiteley Protocol. Those that leave the clinic, although familiar with The Whiteley Protocol, soon fall behind due to the rapid changes we make as a result of our active research programme and quality assurance. 4
5 NICE clinical guidance Since we introduced endovenous surgery in the UK over a decade ago, The Whiteley Clinic has been at the forefront of varicose vein research and treatments. And now the NICE Clinical Guidance 168 echoes everything we have been saying Patients should be referred to a specialist vascular service a team of expert consultants People with a wide range of conditions symptomatic primary or recurrent varicose veins, thrombophlebitis, skin discolouration, venous eczema or leg ulcers can now insist on a referral to a vascular service Assessment of varicose veins must be done using duplex ultrasound Endovenous thermal ablation should be the first line of treatment Compression stockings should not be used to treat varicose veins leading the way 5
6 location and facilities Our London clinic is located right in the heart of the medical area of London W1, conveniently situated between Bond Street and Oxford Circus tube stations and opposite the Royal Society of Medicine givng patients the best The facility has two operating theatres for endovenous treatment of varicose veins and other vein problems as well as x-ray (C-ARM) equipment for our world leading pelvic vein embolisation treatments. In addition, the Clinic is one of the few clinics in the UK with IVUS (intravascular ultrasound) which allows us to perform ultrasound inside pelvic veins for more accurate diagnosis. private insurance The Whiteley Clinic is recognised by the majority of private health insurers. 7 the team Professor Whiteley has assembled one of the best teams of consultant surgeons and specialist vascular technologists (sonographers) to practice at our London clinic Prof Mark Whiteley was appointed a Consultant Vascular Surgeon in 1998 and performed the first endovenous keyhole surgery for varicose veins in the UK in March He trained at St. Bartholomew s Hospital in London and was a lecturer in surgery at Oxford University. As the founder of The Whiteley Clinic and The Whiteley Protocol, his aim was to bring the very best treatments for varicose veins and vein problems, such as thread veins and leg ulcers, to the UK public. David Beckett is an Interventional Radiologist with a special interest in minimally invasive vascular procedures. He trained at St. Georges Medical School and qualified in He went on to gain his radiology experience in hospitals in Birmingham and Sheffield, where he spent two years as an Endovascular Fellow at the Sheffield Vascular Institute before being appointed Consultant Interventional Radiologist at the Royal Bournemouth Hospital in David joined The Whiteley Clinic in early 2010 and in addition to the endovenous treatment of varicose veins, he specialises in a sophisticated procedure called Pelvic Vein Embolisation which is used to treat Pelvic Congestion Syndrome. Mr Gabriele Bertoni was born and grew up in Milan, Italy. He qualified and trained in Italy and also in Oxford, UK. He was appointed Consultant Vascular Surgeon in Italy in 2008 and in the UK in During his PhD studies in vascular anatomy, he developed an interest in the pathophysiology of the venous system. He is now treating his venous patients following The Whiteley Protocol, both in Italy and in the UK. Professor Mohideen Jameel is a Consultant Vascular Surgeon and Honorary Professor at the University of Salford, Manchester. He qualified in Sri Lanka and gained his surgical training in Lancashire and Sussex before joining the Academic Vascular Surgery Department at St. Mary s Hospital, London and the Imperial College of Science, Technology and Medicine, London. He obtained his senior vascular surgical training in Oxford and Nottingham and worked as a Clinical Lecturer in Vascular Surgery at John Radcliffe Hospital, Oxford and the Queen s Medical Centre, Nottingham. A Consultant Vascular Surgeon at Kingston, Surrey from 1998 to 2000, he became Consultant Vascular Surgeon in Wigan and South Lancashire Vascular Network from He has now adopted the Whiteley Protocol for the treatment of varicose veins. Tim Fernandez-Hart gained a MSc in Vascular Ultrasound from King s College in 2008 and became an accredited Vascular Scientist in He spent two more years working full time in the NHS before moving to The Whiteley Clinic in Charmaine Harrison was chief Sonographer at the Alfred Hospital in Melbourne before moving to the UK. She joined the team at The Whiteley Clinic in March As well as her specialist interest in vascular ultrasound and venous procedures she has retained her experience in musculoskeletal ultrasound and gynaecological pathology. Dr Previn Diwakar qualified in Medicine from Charing Cross & Westminster Medical School. In 2007, he was appointed Consultant Endovascular Interventional Radiologist at the Kent & Canterbury Hospital where he is Clinical Lead for Interventional Radiology. In 2012, he adopted The Whiteley Protocol to treat varicose vein patients at The Whiteley Clinic. 7
