4 WHAT IS AN ANNUAL PHYSICAL EXAMINATION (APE)? A physical examination is an evaluation of the body and its functions using inspection, palpation (feeling with the hands), percussion (tapping with the fingers), and auscultation (listening). A complete health assessment also includes gathering information about a person's medical history and lifestyle, doing laboratory tests, and screening for disease. 1 INTERNAL USE ONLY 4

It is a vital part of a preventative illness measure taken to ensure a long and healthy life for the employees. 2 It provides an opportunity to answer questions and teach good health practices. Detecting a problem in its early stages can have good long-term results. 1 INTERNAL USE ONLY 5
6 WHAT IS AN ANNUAL PHYSICAL EXAMINATION (APE)? Preventive care to offset a chronic disease should be woven into all aspects of life, including where and how we live, learn, work, exercise, as well as our health care. Getting a yearly check-up is important in maintaining good health, and should be an integral part of anyone s health care routine. 1 INTERNAL USE ONLY 6
7 IMPORTANCE OF AN ANNUAL PHYSICAL EXAMINATION A preventative measure to ensure a long and healthy life for you and your family. 3 To catch early signs and symptoms that could lead to a serious illness. It keeps healthcare costs affordable by preventing illness/es to progress. 2 INTERNAL USE ONLY 7
8 WHAT TO EXPECT DURING APE 4 History This is your chance to mention any complaints or concerns about your health. Your doctor will also likely ask you about important behaviors like smoking, excessive alcohol use, sexual health, diet, and exercise. Questions will also include past medical history, medications taken, past operations and immunizations, family history and details of present Illnesses, if there are any. 2 Vital Signs These are some vital signs that will be checked by your doctor: Blood Pressure: Less than 120 over 80 is a normal blood pressure. Doctors define high blood pressure (hypertension) as 140 over 90 or higher. Heart Rate: Values between 60 and 100 are considered normal. Many healthy people have heart rates slower than 60, however. Temperature: 98.6 degrees Fahrenheit is the average, but healthy people can have resting temperatures slightly higher or lower. INTERNAL USE ONLY 8
9 WHAT TO EXPECT DURING APE 4 General Appearance Your doctor gathers a large amount of information about you and your health just by watching and talking to you. How is your memory and mental quickness? Does your skin appear healthy? Can you easily stand and walk? Heart Exam: Listening to your heart with a stethoscope, a doctor might detect an irregular heartbeat, a heart murmur, or other clues to heart disease. Lung Exam: Using a stethoscope, a doctor listens for crackles, wheezes, or decreased breath sounds. These and other sounds are clues to the presence of heart or lung disease. Head and Neck Exam: Opening up and saying "ah" shows off your throat and tonsils. The quality of your teeth and gums also provides information about your overall health. Ears, nose, sinuses, eyes, lymph nodes, thyroid, and carotid arteries may also be examined. INTERNAL USE ONLY 9
10 WHAT TO EXPECT DURING APE 4 Abdominal Exam: Your doctor can use a range of examination techniques including tapping your abdomen to detect liver size and presence of abdominal fluid, listening for bowel sounds with a stethoscope, and palpating for tenderness. Neurological Exam: Gives information regarding nerves, muscle strength, reflexes, balance and mental. Dermatological Exam: Skin and nail findings could indicate a dermatological problem or disease somewhere else in the body. Extremities Exam: Your doctor will look for physical and sensory changes. Pulses can be checked in your arms and legs. Examining joints can assess for abnormalities. INTERNAL USE ONLY 10
11 WHAT TO EXPECT DURING APE 4 Male Physical Exam An annual physical exam for men might also include: Optional Testicular exam: A doctor can check each testicle for lumps, tenderness, or changes in size. Most men with testicular cancer notice a growth before seeing a doctor. Optional Hernia exam: The famous "turn your head and cough" checks for a weakness in the abdominal wall between the intestines and scrotum. Optional Penis exam: A doctor might notice evidence of sexually transmitted infections such as warts or ulcers on the penis. Optional Prostate exam: A doctor checks for changes in the size and consistency of the prostate. INTERNAL USE ONLY 11
12 WHAT TO EXPECT DURING APE 4 Female Physical Exam A woman's annual exam might include: Optional Breast exam: Feeling for abnormal lumps may detect breast cancer or benign breast conditions. The doctor will also check the lymph nodes in the underarm area and look for visual abnormalities of the breasts. It will include detecting abnormal nipple discharges and breast lumps that can be an early sign of breast cancer or benign breast conditions. 2 Optional Pelvic exam: The pelvic exam allows examination of the vulva, vagina, and cervix. Routine checks for sexually transmitted infections are often done. A Pap test can screen for cervical cancer. INTERNAL USE ONLY 12
13 WHAT TO EXPECT DURING APE Laboratory Tests The following tests are included during the APE: Urinalysis Fecalysis Chest x-ray Complete Blood Count Basic Blood Chemistry (FBS, Total Cholesterol, Creatinine, BUA, BUN, HDL, LDL and Triglyceride) Please note that hours overnight fasting is required. Nothing by mouth form rest. Other Tests Electrocardiogram (ECG): for members 35 years old and above or if indicated Pap smear: for female members 35 years old and above or if prescribed Eye Refraction: to test the acuity of our vision Rectal Exam: detects the cause of symptoms such as rectal bleeding, pelvic pain, change in bowel habits and checks hemorrhoids or growths. 14 WHAT TO EXPECT DURING APE Drug Test In compliance with the Drug Free Workplace Policy (see policy in MIO) and with the Republic Act No The Comprehensive Dangerous Drugs Act of 2002, employees will be randomly selected to undergo drug test. INTERNAL USE ONLY 14
15 SOME REMINDERS DURING APE Play an active role during your APE. Make sure you speak up with any questions or concerns you have about your health, regardless of how minor they may seem. Take doctor s warnings seriously and do not hesitate to ask for more informational resources. The easiest way to prolong life and keep healthcare costs affordable for everyone is to take measures to prevent illness whenever possible. INTERNAL USE ONLY 15
16 SOME REMINDERS DURING APE Annual exams and a healthy lifestyle are two big preventative measures you can control in taking good care of yourself. Plastic containers (for the pea-size fecal specimen) will be delivered to your office. Please do not forget to bring your Intellicare card and Indra ID on your scheduled APE. In compliance with and for the issuance of the 2017 Health and Sanitation Permit, the Pasig City Local Government is requiring all employees to complete their APE. Given this, ALL EMPLOYEES are required to undergo the APE. INTERNAL USE ONLY 16
17 SOME REMINDERS DURING APE 2 Registration Please ensure that you have reserved slots for you and your dependents on your preferred schedule of APE Please bring your Intellicare card and Indra ID Female Patients Physical Exam must be accomplished prior to having the Chest X-ray A waiver must be signed prior to the Chest X-ray procedure For patients with delayed menstrual cycles or are suspected pregnant please inform the x-ray mobile team Female Patients with Pap Smear in their package No sexual contact for at least 3 days (72 hours) prior the examination The last day of menstruation should be at least 10 days prior to the examination date Do not use vaginal douches for at least 3 days prior to the procedure INTERNAL USE ONLY 17
18 SOME REMINDERS DURING APE 2 Patients with ECG in their package Refrain from putting lotions, cream, oil or powder at the chest area Patients who will undergo a Chest X-ray Remove all metals and other gadgets prior to the x-ray Avoid wearing shirts with buttons or zipper Females are encouraged not to wear undergarments with underwire Mobile x-ray van will not be available between 12 noon until 1pm. Patients with Blood Chemistry in their package hours overnight fasting (no food and water by mouth) from rest INTERNAL USE ONLY 18
19 SOME REMINDERS DURING APE 2 How to collect Urine Specimen Use an approved empty container for collection Wash and dry your hands thoroughly Remove the container cap and set it aside. Do not touch the inner surface of the container to avoid any possible contamination. Wash your genital area (lower parts) with a toilet paper or a mini towel a. For women: wipe from front to back between the folds of skin b. For men: retract the foreskin (if uncircumcised) and clean the glands (head of the penis) INTERNAL USE ONLY 19
20 SOME REMINDERS DURING APE 2 The intent is to collect midstream during the urination process a. Pass a small amount of urine into the toilet. The initial stream may be contaminated with skin and urethral bacteria b. Midway, urinate into the container. The container should only be full or 2/3 full Replace the cap and make sure it is closed properly Wash your hands after collecting the specimen Bring the specimen to the medical technicians for submission Note: For female members, it should be at least 7 days from the last day of menstrual period. INTERNAL USE ONLY 20
21 SOME REMINDERS DURING APE 2 Proper collection of stool sample Stool sample should be collected in a clean container The specimen should not be contaminated with toilet paper, water, urine or other materials The specimen should be submitted to the laboratory within 2 hours from the time of collection A small amount of stool (pea/thumb size) is enough for the analysis Note: Completion of APE on the next schedule or at an Aventus clinic will be allowed only for those female members with menstrual period and/or suspected pregnant during their scheduled APE. INTERNAL USE ONLY 21
22 FREQUENTLY ASKED QUESTIONS 2 Is it safe to get an X-ray if my monthly period is delayed or if I m irregular? Before having an xray, please proceed for the Physical Examination. Indicate the date of your last menstrual period in the Medical Form provided and inform the examining doctor. Depending on the circumstance, the doctor may advise you to postpone taking the procedure. Is it safe to get another X-ray if I had one 3 months ago? The level of safety depends on the type of machine, kind of xray test you need and the dose of radiation you will be exposed to. The Aventus machines are safe for repeat chest xray and the radiation exposure is minimal. If I have my menstruation, do I go through Urinalysis? Please inform the medical staff if you have your menstruation. Urinalysis is not performed during menstruation. INTERNAL USE ONLY 22
23 FREQUENTLY ASKED QUESTIONS 2 Can I get a Pap Smear even if I am a virgin? If you are a virgin, please inform the examining doctor during the Physical Examination. The instrument used to collect the cervical cells may stretch and tear your hymen (a thin tissue covering the vaginal opening, which is present in some women who have never had sexual intercourse). Slight bleeding may also occur. What can I expect from a Pap Smear procedure? The doctor will instruct you to lie on your back, with your knees up and bent and feet on the rests. An instrument called a speculum is used to open the vagina so that the walls and the cervix can be clearly seen. A sample of the mucus and cells will be taken from the cervix. During the test, breathe slowly and concentrate on relaxing your stomach and leg muscles. Some discomfort may be experienced. INTERNAL USE ONLY 23
24 FREQUENTLY ASKED QUESTIONS 2 Do I need to fast for Complete Blood Count (CBC)? No, fasting is only required when a blood chemistry panel is to be conducted along with the CBC. How much stool specimen is enough for a stool exam? A small amount of stool (pea or thumb size) is enough for analysis. This is placed in the provided container and sealed properly. Will blood extraction cause black and blue? Black and blue or hematoma may happen after blood extraction. To avoid, apply adequate pressure 1-2 minutes after extraction. You may also: leave the pressure dressing on for at least 8 hours; avoid lifting heavy objects with the arm; apply ice pack for about 20 minutes during the first 24 hours following the formation of the bruise or hematoma; you may apply warm, moist compresses for about 20 minutes during the second 24 hours after collection. INTERNAL USE ONLY 24
