1 Nutrition Procedures/Referrals Guidance Measurements All Options Head Start & For children 24 months and over Measurements to be taken: At Enrollment and Spring (for a total of 2 measurements 6 months apart) Enter only measurements you have taken into PROMIS (Health>Physicals/Tests>Medical Tests/Screenings>Height/Weight), as well as in Child s File Do not enter child s measurements from Physical Exam Form unless it is the information from 18 month Wellbaby check. Center Based, Home Based Victoria & FCC Option For children 24 months and under Enter measurements provided on Well Baby Check (height, weight and head circumference) according to EPSDT schedule into PROMIS (Health>Physicals/Tests>Medical Tests/Screenings>Height/Weight), as well as Child s File If parent does not bring Well Baby check on time, please take (height, weight and head circumference) and record in child s file on the Supplemental Data Form and in PROMIS. The following Growth Charts must be printed from PROMIS in the Growth Chart section and placed in child s file under Health and Nutrition Print the following Growth Charts: Children younger than 2 years Select age group: Birth to 36 Months in PROMIS Children older than 2 years Select age group: 2-20 years in PROMIS Head Circumference Weight by Height Height by age Print these Growth Charts: BMI Weight by Height Height by age
2 SCREENING ABNORMAL BMI- Overweight and Obese: - Older than 2 Years of Age - Look at Height by Weight results in Short Stature: - All age groups - Look at Height By Age results in BMI:-Underweight - Older than 2 Years of Age - Look at Height by Weight results in Head Circumference: - Younger than 2 Years of Age - Look at Head Circumference results in 85% or above GIVE parent High BMI handout and attach Letter from Nutrition OFFER to make a referral to Nutrition Specialist. If accepts send an Internal Request for Follow-up Services. Send IRFUS to Nutrition Specialist (See Internal Request for Follow-up Services SOP). If declines, make sure to DOCUMENT conversation on the Family Contact Log as well as under Services in PROMIS. DOCUMENT all this this information on the Family Contact Log as well as into PROMIS. Please refer to Screenshot, How to input High/Low BMI in PROMIS 5% or below SUBMIT IRFUS to Nutrition Specialist (See Internal Request for Follow-up Services SOP). In addition, CONTACT Nutrition Specialist via telephone or to review results with parent 4% or below GIVE parent Low BMI handout and attach Letter from Nutrition OFFER to make a referral to Nutrition Specialist. If accepts send an Internal Request for Follow-up Services. Send IRFUS to Nutrition Specialist (See Internal Request for Follow-up Services SOP). If declines, make sure to DOCUMENT conversation on the Family Contact Log as well as under Services in PROMIS. DOCUMENT all this this information on the Family Contact Log as well as into PROMIS. Please refer to Screenshot, How to input High/Low BMI in PROMIS Below 2% or above 98% SUBMIT IRFUS to Health Specialist and Nutrition Specialist (See Internal Request for Follow-up Services SOP). In addition, CONTACT Health Specialist and Nutrition Specialist via telephone or to review results with parent. If results are within the normal range: Offer parent/guardian a general nutrition handout if you choose. Please inform parents that there is a Nutrition Specialist in the agency available to answer any questions they may have now or in the future.
3 Hematocrit or Hemoglobin And Head Start Hematocrit or Hemoglobin to be taken at 7-9 months, months and 2 years of age and every year after with Physical Exam. Take results from Physical Exam and document results into PROMIS under (Health>Physicals/Tests>Medical Tests/Screenings>Blood count) Results are to be recorded on the Supplemental Data Form in the child s file if they were not on the initial Physical Exam form that was received. SCREENING NORMAL ABNORMAL Hemoglobin > 11.0 <11.0 Hgb GIVE parent Iron and Anemia handout and attach Letter from Nutrition Must SUBMIT an Internal Request for Follow-up Services. Send IRFUS to Nutrition Specialist (See Internal Request for Follow-up Services SOP) Ask parent/guardian when the next follow-up test is scheduled for and DOCUMENT this information on the Family Contact Log as well as under Services in PROMIS. Be sure to follow-up with parent on that date for new results. Hematocrit >34% <34% Hct GIVE parent Iron and Anemia handout and attach Letter from Nutrition Must SUBMIT an Internal Request for Follow-up Services. Send IRFUS to Nutrition Specialist (See Internal Request for Follow-up Services SOP) Ask parent/guardian when the next follow-up test is scheduled for and DOCUMENT this information on the Family Contact Log as well as under Services in PROMIS. Be sure to follow-up with parent on that date for new results.
4 Lead For children 24 months and under Lead test to be taken at 12 and 24 months Take results from Physical Exam and document results into PROMIS under (Health>Physicals/Tests>Medical Tests/Screenings>Lead) If there is no documentation that a blood lead test was performed at 12 month of age, a blood lead test must be performed as soon as possible. A second lead test will be required at 24 months of age or soon thereafter. Not required for children greater than 3 years of age if test was done at 24 months. If child s primary health care provider refuses to give child the 2nd blood lead test, a written statement must be obtained from the provider. SCREENING NORMAL ABNORMAL Lead 5 ug/dl and under 5 ug/dl and greater SUBMIT IRFUS to Nutrition Specialist (See Internal Request for Follow-up Services SOP).
5 Other Parent has requested a meeting with Nutrition Specialist. Parent has concerns regarding child s eating habits, weight, or any other nutrition-related issue SUBMIT a referral to Nutrition Specialist (See IRFUS Policy & Procedure). If the issue is urgent, contact Nutrition Specialist immediately, via telephone or . There are answers on Nutritional Information form that triggered a referral(*) There is a nutrition condition mentioned on the physical that is not mentioned above
