ACL REPAIR KIERA MANTYLA

ACL ACL, or anterior cruciate ligament, 1 of the 4 main ligament of the knee Connects the femur to tibia ACL provide 80-90% of leg s stability and essential for movement About 400,000 ACL injuries each year in the United States

ACL INJURY Causes of injury usually from sudden movement of the knee such as suddenly stopping and changing direction, impact to side of knee. Estimated recovery time is 8 to 12 months with current repair techniques Said you can hear a pop and will cause extreme pain when torn

ACL IN SPORTS 1 of the top 10 most common/severe sport injuries Especially high risk for lateral-motion sports, football and basketball Cuts the Athlete s season short and even their careers in some cases Athletes with ACL injuries: Tom Brady, Adrian Peterson, and many others

CURRENT REPAIR TECHNIQUE Most common technique is using a graft Graft reconstruction surgery takes about hour to hour and half. Graft can be either own tissue or a donor tissue Could be from patella tendon, quadriceps tendon, or hamstring tendon Athletes more common to use own tissue for graft

BRANCH-ENHANCED ACL REPAIR BEAR Martha Murray is Engineer behind this research Technique uses stiches and bridging scaffold(a sponge injected with patients blood) This stimulates the torn ACL to heal Blood will stimulate clotting and promote healing 6-8 weeks the torn ligament will grow into the sponge and ends will connect, then replace the sponge

BEAR VS. RECONSTRUCTION BEAR Experimental, some unknown risks/benefits Remaining ACL tissue is saved Incision to insert scaffold (arthroscopically) Doesn t require graft ACL Reconstruction Standard fix for acl, proven to be safe Remaining ACL is removed Incision to insert graft (arthroscopically) Requires graft of patient s tendon

CLINICAL TRIAL Study have been done in pigs In pig s knee study 3, 6 and 12 months after surgery bridge technique is as strong as reconstruction Pigs had less arthritis with this method But doctors were unsure this would be the same in humans US drug administration cleared the study to continue for human testing

CLINICAL TRIAL Also did a study involving 20 patients, 10 received BEAR treatment The first surgery was February of 2015, to this date all patients are doing just as well as patients with reconstruction surgery Still early for the patients to tell if this will be a long term fix No bad reaction to the sponge bridge from the 10 patients 10 months after surgery all patients have new healthy ACLs

CLINICAL TRIALS CONTINUE Starting the next face of trail, with 100 patients ages Random 2/3 with get BEAR, and other 1/3 will get reconstruction surgery During the study will do follow ups 1-2 weeks, 6 weeks, 3 months- questionnaire and physical exam 6 months: lab work, MRI, physical test 1-year: strength test, physical test, questionnaire 2,6,10 year: MRI, X-ray, physical testing

FUTURE Believe to make recovery time less 6-9 months, currently needed for reconstruction Hopes this will be the new treatment used by all professional athletes Murray that this technique of BEAR can be applied to heal deteriorating articular cartilage
