1 January 31, 2019 Effective April 1, 2019, Blue Cross of Idaho is implementing changes to the Commercial and Medicare Advantage prior authorization list for medical and pharmacy services. Prior authorization changes may not apply to all contract types. Check member eligibility and prior authorization requirements before performing services.
