2 Cover Feature The Orthopedic and Spine Institute at Northern Westchester Hospital State-of-the-Art Treatment Close to Home By Jean B. Edwards Patients in need of orthopedic treatment can find expert care at Northern Westchester Hospital s innovative Orthopedic and Spine Institute in Mount Kisco. Now, patients can get state-of-the-art treatment in a unique patient-centered environment without leaving their hometown. No longer will people have to travel to the city to find the most experienced, highly skilled surgeons that are board certified and have trained at highly respected teaching hospitals, says Victor Khabie, M.D., Co-Director of the Orthopedic and Spine Institute. We can provide a wide range of treatments and minimally invasive procedures right here that are customized to meet the unique needs of each patient, he explains. We use a multidisciplinary team that integrates Orthopedic, neurosurgery, pain management, internal medicine and physical therapy along with a team of anesthesiologists, radiologists and nurses to provide the most comprehensive care possible, adds Evan Karas, M.D. Dr. Karas also serves as Co-Director of the institute. Patients Come First Dr. Khabie and Dr. Karas have established exceptionally high standards of care at the institute. We aspire to have the premier Dr. Victor Khabie debriding a rotator cuff utilizing an arthroscopic shaver and a HD camera.
3 Photos by Don Dempsey, White Light community hospital-based Orthopedic and spine surgery program, says Dr. Karas. Our treatment philosophy is one of continuous quality improvement to ensure that our patients receive the best treatment available. We believe our unique, patient-centered approach of providing full-service orthopedic treatment with compassion and comfort will set us apart from any other program. Every patient at the Orthopedic and Spine Institute will be assigned to a care team that will oversee the patient s every need from presurgical assessments and education to surgical treatment, postoperative care and rehabilitation. We consider the patient and the family to be an important part of the care team and they will play an active role as we work together to develop a personalized plan, based upon individual patient needs, explains Dr. Karas. This personalized approach enables us to develop a single plan of care that integrates all related departments, thereby ensuring that patients have all of the necessary information to make an educated choice when surgery is needed. Dr. Karas says that patients and their families will also find a caring, respectful and nurturing environment at the Orthopedic and Spine Institute. There is no question that this type of atmosphere can reduce patients anxiety and promote faster healing. Leading-Edge Technology Along with highly trained surgeons, Northern Westchester Hospital s Orthopedic and Spine Institute offers the latest technology and treatments for patients. Electronic medical records and a system to verify medication orders are in place to ensure patient safety. In addition, all patient rooms are equipped with a computer screen to enable the treatment team, including the patient and his/her family, to review imaging results, lab work and other information at the patient s bedside. Comprehensive imaging tools to help diagnose patients are available including MRI, PET, CT, ultrasound and digital X-rays. The hospital s radiology department is accredited by the American College of Radiology. This distinction ensures that the facility, equipment and personnel have met the highest nationally accepted standards when it comes to performing and interpreting medical images and that the equipment and the facility meets or exceeds quality assurance and safety guidelines. We are very fortunate that Northern Westchester Hospital supports our desire to bring the latest technology to the region once it has been thoroughly researched and tested, explains Dr. Karas. I have the opportunity to use some of the newest bioabsorbable implants during sports medicine surgery. These are usually constructed of plastic, and they are designed to be Arthroscopic rotator cuff repair with double-row fixation [INSET] Arthroscopic rotator cuff tear absorbed by the body without producing an immune response, he explains. In addition, the newer biocompatible products don t require the use of cement when they are inserted. They are engineered so they lock into place, and the bone will actually grow into the prostheses, he explains. We re passionate about striving to improve the services we deliver and keeping abreast of the latest technologies and procedures that can help our patients, says Dr. Karas. In fact, we re continuously involved in testing a number of different knee and hip replacement prostheses to ensure we can provide our patients with the best options to increase the quality of their lives. I have friends who work at hospitals in Manhattan that have shared with me how long it takes to get new technology approved, says Dr. Khabie. They are required to go through various inefficient committees. At Northern Westchester Hospital, decisions are made through a streamlined process that incorporates outcomes data and enables key decision makers from the medical and administrative teams to make determinations quickly and efficiently. The philosophy here is that if new technology will benefit patients, provide better outcomes and has been thoroughly tested and analyzed, we move forward to implement it. Surgical Procedures Available The Orthopedic and Spine Institute at Northern Westchester Hospital is comprised of three specialty areas: sports medicine, total joint replacement and spine surgery. When less invasive therapies don t work, the experts at the Orthopedic and Spine Institute can offer a multitude of surgical treatments. Those include surgery to treat the anterior cruciate ligament (ACL), fractures of the elbow, knee, shoulder and hip, tennis elbow, rotator cuff and meniscus tears, frozen shoulder
4 Cover Feature cases, bone spurs and a variety of pediatric and adolescent sports injuries. Total joint replacement is available for hip, knee, shoulder, fingers, ankles and elbows when arthritis or years of use cause the joint to wear away. Surgery for scoliosis, as well as lumbar and cervical herniated discs is also available. Whether the patient is a professional athlete or a weekend warrior, the experts at the Orthopedic and Spine Institute can treat a full range of conditions and injuries to help them recover faster so they can get back in the game. Dr. Khabie has significant experience in treating ACL knee injuries, which are common to athletes. The ACL ligament connects the femur to the tibia and keeps the knee joint from slipping out of place. When the ligament is injured, the knee becomes unstable and can buckle. Because Dr. Khabie can perform surgery using a minimally invasive approach, patients have less pain and recover faster. During arthroscopic ACL surgery, Dr. Khabie makes several small incisions around the knee and inserts the arthroscope into one of the incisions. A camera at the end of the arthroscope transmits pictures from inside the knee to a monitor. We re very fortunate because Northern Westchester Hospital just purchased a high-definition Dr. Evan Karas: left knee meniscectomy done arthroscopically using a biter to remove the torn portion of the meniscus camera and recently upgraded to monitors with high-definition flat panel screens, says Dr. Khabie. The new camera transmits magnified images of the surgical area to the high-tech screens, he explains. These images are 25 to 30 times larger than their actual size, so the visual acuity is incredible, he says. We think this new equipment is a marked improvement from some of the older technology that is being used at other facilities. Once injured, the ACL cannot simply be repaired by sewing it together again. This requires Dr. Khabie to use autograft tissue from another part of the patient s body, such as the patella or hamstring. One of the latest techniques we are performing at Northern Westchester Hospital involves the use of allograft tissue from a cadaver to help reconstruct the knee during ACL surgery, explains Dr. Khabie. This approach eliminates the need to harvest the patient s own tissue. The tissue from the cadaver simply becomes part of the patient s body, and it will last a lifetime. As a Harvard-trained, leading expert in orthopedic sports medicine, nearly 90% of the work Dr. Khabie does involves sports medicine surgery. A former athlete himself, Dr. Khabie played varsity football while a student at the University of Pennsylvania. He has served as assistant team physician to some of the top sports teams including the Los Angeles Lakers, Dodgers and Kings hockey team. He is also the ringside physician for the New York State Athletic Commission where he cares for professional boxers. Dr. Karas, an orthopedic and sports medicine specialist, focuses on joint replacements and knee and shoulder surgery. At Northern Westchester Hospital, we have all of the technology available that helps us maximize outcomes for our patients, he says. Our goal is to help patients get the very best treatment and get them rehabilitated quickly. We use the very same protocols and procedures as they do at the major teaching institutions in New York City. What sets our Orthopedic and Spine Institute apart from other places is the personalized care we provide to every patient.

6 NWH Orthopedic and Spine Institute surgical team total joint replacement surgery. The Orthopedic and Spine Institute will be addressing ways to provide the best care for our patients so we can maximize outcomes. With information captured through electronic medical records, we will be able to perform clinical studies. For example, we will be able to examine how many patients came to the institute with a particular diagnosis and what the outcomes were for the treatments they received. We believe that by using evidence-based medicine, we will be able to provide better care and we ll have the results to prove it, he says. Evan Karas, M.D., and Victor Khabie, M.D., Co-Directors of The Orthopedic and Spine Institute at Northern Westchester Hospital Future Trends Dr. Karas notes that there is an increased focus and importance on measuring outcomes in the field of orthopedic surgery. At our new institute, we are very interested in measuring the quality of the treatments we provide and how we can improve upon these treatments to help our patients, he says. In the past, there hasn t been a lot of good research done, but that s all changing. In the future, we will put much more emphasis on evidenced-based surgery by evaluating certain control studies so we can decide which approach is best for our patients. He continues, Today, a big debate in orthopedic surgery is how to prevent problems after Teaching Other Physicians In addition to hosting a variety of clinical conferences for physicians, the Orthopedic and Spine Institute will also serve as a location for physicians to advance their skills. Many of our physicians at the institute are truly leaders in their field, and we want to invite other physicians to come here to learn new techniques while utilizing the latest technologies, explains Dr. Karas. Physicians will have the opportunity to perfect their surgical techniques on cadavers, and we hope they will take this experience back to their hospitals so they can help other surgeons improve their performance. We believe this will be a win-win situation for everyone. At Northern Westchester Hospital s Orthopedic and Spine Institute, we believe we can provide specialized care that is just as good as or better than any available in New York City, says Dr. Khabie. We have the top surgeons, the latest technology and a caring and compassionate team that is fully committed to providing the best patient outcomes. n Reprinted from Lower Hudson/Bronx md news
