2 INTRODUCTION Nursing and midwifery students on clinical practice may be exposed to blood and body fluids as a result of accident or inexperience, and therefore are at risk of contracting or passing on a blood-borne virus or other infectious diseases. You have a responsibility to protect yourself and to protect patients from health hazards, and must be fully immunised. The Student Immunisation Record form is the most complex requirement to complete and may take weeks so we suggest you make a start on this as soon as possible. The form can only be completed by a registered General Practitioner (GP), who is not a family member of the student, and will not be accepted if completed by any other party. The examples on the following pages will show you how your GP should fill out the immunisation form correctly. This will ensure that you do not need to make extra appointments with your GP to have data rectified on your form. Please note that we will not accept serology reports or vaccination lists that aren t listed on your Immunisation form by your GP. The student administration team process these forms and aren t medical professionals so we cannot determine serology results or vaccination details. This is why the UQ Immunisation form has been created and must be completed by a GP. If you do not complete your immunisation form correctly you will be asked to resubmit which will delay your placement. Please make sure you read through this whole document carefully as it provides important information.

3 IMMUNISATION REQUIREMENTS The following Immunisations requirements MUST be completed before attending placement: Name of disease DTP (Diphtheria, Tetanus, Pertussis (whooping cough)). MMR (Measles, Mumps, Rubella) Chickenpox (Varicella) Hepatitis B Requirements Completed childhood vaccination course. Must have had a booster within 10 years. Have at least 2 doses of the vaccination OR serology results confirming immunity. Have at least 2 doses of the vaccination OR serology results confirming immunity. Have completed appropriate course of vaccinations AND serology to confirm immunity. You may start placement conditionally after you have had 2 doses of the vaccine, but must update your form each time when you receive your 3 rd vaccination and your serology results. Your Hep B vaccination must follow the below schedule: First Dose Second Dose Third Dose Serology Hep B vaccination schedule and serology This is the initial dose. 1 month after 1 st dose (you cannot receive this less than 1 month after your 1 st dose) 6 months after 1 st dose (you cannot receive this dose less than 6 months after your 1 st dose) To be completed 4 weeks after your 3 rd dose (you cannot complete your serology lest than 4 weeks after your 3 rd dose) Tuberculosis Screening If you do not provide us with an updated form with your 3 rd dose and immunity results before the start of your 2 nd year you will be pulled from placement. Must have completed recent test (skin or IGRA/Quantiferon Gold for TB blood test) within the last 2 years. If you test positive you will need to request a chest x-ray from your GP to determine if you are clear of any TB infection. You will not be permitted on placement until you test negative or are cleared of any infection. Your GP will need to be screened for Hep B & B and HIV infection as these infections can limit the type of surgical procedures the student can be involved with. Your GP will need to fill out this information on page 3 of your immunisation form.

4 The following immunisations are RECOMMENDED to be completed prior to placement Name of disease Influenza Hepatitis A Requirements Should have vaccination each year in autumn. This is Strongly Recommended and can affect your placement location if you do not receive this. If you elect not to have the influenza vaccination you are still required to complete the Student Influenza form each year and indicate that you wish not to receive the vaccination. Not routinely required recommended for Health Care Workers who work in remote and Indigenous communities or with Indigenous children in NT, QLD, SA or WA, and other specified healthcare workers in some jurisdictions

5 EXAMPLE 1 - INCOMPLETE IMMUNISATION FORM DTP vaccination more than 10 years old. Only 1 Chickenpox vaccination listed. You are required to have 2 to start placement or have positive immunity. TB test completed more than 2 years ago and received a positive result. Students must be negative or clear of TB. Only 1 Hep B dose listed. Students need to have at least 2 to start placement conditionally and provide evidence on immunisation form of 3 rd dose and positive immunity by Semester 1, Year 2.

6 EXAMPLE 2 INCOMPLETE IMMUNISATION FORM MMR serology date listed but not results of test. Your GP needs to write the results (positive/negative) AND the date on the form. Only one dose of Chickenpox given. Students require 2 doses or positive immunity to start placement. TB test date listed but not results. Your GP needs to write the results (positive/negative) on the form. Hep B serology results missing. Students need to provide evidence of Hep B doses AND serology results on their immunisation form.

7 EXAMPLE 3 COMPLETE IMMUNISATION FORM DTP given within the 10 year timeframe Two doses of MMR and Chickenpox listed. TB report date within 2 years and results listed stating student does not have TB. Three doses of Hep B vaccine given AND serology report date and results showing immunity. GP needs to write level amount not immune.

8 EXAMPLE 4 COMPLETE IMMUNISATION FORM DTP given within the 10 year timeframe. MMR and Chickenpox test results dates listed AND immunity status. TB report date with 2 years and results listed stating student does not have TB. At least 2 Hep B doses listed. You may start placement conditionally after you have had 2 doses of the vaccine, but must update your form each time when you receive your 3 rd vaccination AND your serology results. If you do not provide us with an updated form with your 3 rd dose AND immunity results before the start of your 2 nd year you will be pulled from placement.

9 REASONS WHY YOUR STUDENT IMMUNISATION RECORD FORM MAY BE REJECTED Missing immunisations (as listed above in examples). Form has not been signed by you or your GP. You have not submitted ALL pages of the form. You attach serology reports to the form (Your doctor needs to write your doses and serological results on the Immunisation form). You upload each page of your form as a separate document (We cannot save your form to your student file like this). Your GP has listed future dates for vaccinations. (Vaccination dates cannot be listed on the form until they have been given).
