1 Weekly Epidemiological Bulletin Flood Response in Pakistan Volume 2, Issue 2 Monday 17 January Highlights Epidemiological week no 2 (7 to 13 Jan ) Between 7 to 13 Jan (epidemiological week no. 2, ), 57 districts in 4 provinces provided surveillance data to the DEWS system. 2,295 fixed health and 11 mobile medical outreach centres provided surveillance data for this week. A total of 637,341 consultations were reported through DEWS of which 32% were acute respiratory infections (ARI), 9% skin disease, 6% acute diarrhoea, and 6% were suspected Malaria. Total 17 alerts in past two weeks were received and responded: 9 were for Measles, 5 for suspected Influenza, while 1 each for Pertussis, Bloody diarrhoea, and Hypothermia. There were two cases of confirmed poliomyelitis reported this week from flood affected districts. Note: All presented data are based on the number of patient consultations and include information on priority diseases under surveillance as well as major health events reported through DEWS. In the graph at right, DEWS reports received in 2010 are compared to the reports of 2009 from disaster affected districts. This week, DEWS received 38,079 reports of acute diarrhea, 6% of total consultations. Samples were negative for Measles. Vitamin A was given, mass vaccination campaign is planned, health education session was conducted, and measles vaccination completed. Active surveillance is on going. 9 Jan 11 Mardan Rauf Khan Amo Khel Village Mayar 20 F Suspected cases were found with flu like symptoms but sample tested at NIH was found negative for Jan Swabi BKMC (Col. Sample collected and sent to NIH. Active surveillance is on going. 15 Jan 11 Haripur Chungi Bandi Mori 45 M Patient with chronic illness and confirmed, hospitalized in Islamabad, died after 5 days. Field investigation ongoing. 15 Jan 11 Peshawar KTH (Nadir Street,Danishabad & Hashim Quarters Peshawar Cantt.) 55, 63 M,M Suspected cases were found with flu like symptoms. Sample collected and sent to NIH. Active surveillance is on going. 12 Jan 11 Pertussis Sindh Ghotki MCH Centre (Saindino Ghoto Village) 3, 4, 18m, 3 F, F, M, M During active search 4 suspected cases were found. All the Patients were improving. Health education was imparted. Active surveillance was done. 12 Jan 11 BD Haripur Afghan Refugee Camp 4, 18m, 5, 2, 18m, 3, 7m M, M, F, M, M, M, F Suspected cases were reported and upon field investigation 6 suspected cases were found. Samples collected and sent to NIH were negative for shigella and cholera. Water samples were collected and tests are under process. Active surveillance was done. 13 Jan 11 Hypothermia Sindh Dadu UC Khan Jo Goth 16, 5, 55, 80 F, M, F, M Upon field investigation 4 deaths were found during cold weather. Further investigation found epilepsy and pneumonia deaths and two deaths due to hypothermia from exposure. Partners are requested to assist with food and shelter. 15 Jan 11 Tetanus Sindh Ghotki Bhandki Village UC I (Jinnah Clinic) 11 M Upon field investigation 1 suspected case was found in the private health facility. TIG was provided by WHO. Further analysis of public health issues is under process. 4 Province Khyber Pakhtunkhwa This week 14 districts reported to DEWS from province, 515 health centers reported 135,931 patients consultations to DEWS. ARI is the leading disease in province. ARI cases reported 61,224 (45%), 1% lower as compared with last week. Seven alerts were received and investigated this week; 5 alerts were for Influenza, while 1 each for measles and bloody diarrhoea. One alert of suspected measles was received and responded this week. Malaria cases are still reported from flood affected districts. This week a total of 4,746 malaria samples were tested of which 159 were positive (slide positivity rate 3.35%), while falciparum rate was 45.2%. 5 Province Sindh This week 21 districts reported to DEWS from Sindh province and 683 health centers. A total of 276,026 patient consultations were reported during the reporting period of week 2,. In Sindh, ARI cases reported were 82,340 (30%), 2% lower as compared with last week Nine alerts were received and responded this week; 7 were for suspected Measles, while 1 each for Pertussis, and hypothermia. In Balochistan, ARI cases reported were 19,067 (30%), 6% lower as compared with last week. Malaria cases are still reported from different areas of Jaffarabad, Naseerabad and Kech districts. A total of 659 malaria samples were tested of which 204 were positive (slide positivity rate 30.9%). Falciparum Rate was 60.2%. In Naseerabad, a total of 259 samples were tested for malaria, of which 61 were positive (slide positivity rate 23.5%), while Falciparum rate was 86.8%. 6 Focus on: Rubella It is noted that the first case of Rubella this season was confirmed during investigation of a measles outbreak in Kashmore last week, so it is appropriate to review our understanding of the importance of this epidemic prone disease. Rubella is a contagious viral disease, occurring most often in children, which is transmitted via the respiratory route, and symptoms usually appear 2 3 weeks after exposure. The disease is usually mild, with low fever, nausea and a transient rash. However, when a woman is infected with the rubella virus early in pregnancy, she has a 90% chance of passing the virus on to her fetus. This can cause the death of the fetus, or it may cause Congenital Rubella Syndrome (CRS). Even though it is a mild childhood illness, CRS causes many birth defects. Deafness is the most common, but CRS can also cause defects in the eyes, heart, and brain. It is estimated that there are 110,000 cases of CRS globally each year. There is no specific treatment for rubella, but the disease can be prevented by immunization. Many countries are using the MMR (Measles, Mumps, Rubella) as a childhood vaccine, but it is not used by the public health system in Pakistan. As in many other developing countries, mild childhood rubella infections provide natural immunity which prevent most CRS. Immunization of adolescent girls against rubella can also be undertaken to prevent CRS. Acute Respiratory Infection (ARI) As the weather changes especially in the northern areas, the numbers of Diarrheal cases decreased while Acute Respiratory Infection (ARI) cases increased very rapidly more than 30% of all cases were of ARI during last few weeks, which is a threat to the community especially the children. To reduce the morbidity and mortality due to ARI, WHO is supporting the district health system to ensure 24 hour care to children hospitalized with pneumonia and similar illnesses in the flood affected districts. The interventions to control ARI can be divided into four basic categories: immunization against specific pathogens, early diagnosis and treatment of disease, improvements in nutrition, and safer environments. The first two fall within the purview of the health system, whereas the last two require multisectoral involvement. The Federal EPI program with donor support is providing vaccination while WHO is providing technical and financial support in early diagnosis and treatment of disease. It is planned that 76 ARI units in public health hospitals will be supported for 2 to 3 months in flood affected districts. The ARI units supported by WHO are in public health facilities designated in consultation with the district and provincial health departments and organized under the supervision of the hospital administration and pediatrician. The existing health staff and additional staff, where needed, are involved in two days training on case management of ARI imparted to them by expert trainers on WHO guidelines. In this manner, these ARI units are providing quality services for the hospitalized children 24 hours round the clock. Essential medicine for ARI and some equipment which includes Nebulizers, Ambu Bags, Oxygen concentrators and Oxygen cylinders will also be provided to the special ARI units where needed. The objective of this weekly epidemiological bulletin is to provide a snap shot on selected health events reported from the communities affected by the current flood in Pakistan. While every attempt is made to present the weekly trend of the epidemic prone diseases, the information presented in the bulletin needs to be interpreted in the context that precise information on the reference populations is not always available, The bulletin also includes information collected by DEWS teams established during earlier emergencies, including 2005 earthquake, 2007 floods and 2008 ID crises. The primary focus of DEWS is the early detection of epidemic prone diseases, to facilitate a rapid public health response.
