4 Recommended Vaccines for Young Children DTaP HIB Protects against diphtheria, lockjaw and whooping cough Protects against meningitis and pneumonia Polio Protects against polio which can cause paralysis MMR Protects against measles, mumps and rubella (German measles) Varicella Protects against chickenpox Hepatitis B and A Protects infection of the liver caused by a virus Pneumococcal Conjugate Protects against a serious kind of pneumonia (and decreases the frequency of ear infections) Influenza Protects against a highly contagious infection of the nose, throat, and lungs Rotavirus Protects against the most common cause of vomiting and severe diarrhea

8 Toddlers and Pre-Schoolers Each child shall have a Form DH680 on file documenting age-appropriate doses of required immunizations At 18 months to 4 years, children should have at least the following: Immunization Guidelines 2013, pp DTaP 3 Hib 3 Polios 1 MMR 3 Hepatitis B 1 Varicella 4 Pneumococcal Conjugate vaccine
9 Pneumococcal Conjugate vaccines Pneumococcal Conjugate vaccine is required child care and family day care home attendees (Effective January 1, 2012 for children 2 to 59 months of age) The number of doses required is dependent on the child s age at the time of administration and the type of vaccine used as based on the American Academy of Pediatrics (ACIP)

11 Certificates of Immunization DOH 680 forms - must be signed and dated in the appropriate part of the card to be valid Part A-1 (Kindergarten and/or 12th grade) Part A-2 (7 th Graders) Part B (Pre-Schoolers) Part C (Any child with a serious health problem)
12 DH 680: Part A-1 Certificate of Immunization for Kindergarten 12 th Grade Used for: Children entering into Kindergarten either as a new student or as a transfer from another school, or preschool children and children in day care who have met all immunization requirements for Kindergarten Must be dated and signed or electronically certified

14 DH 680: Part B Temporary Medical Exemption Immunization Guidelines 2013, p 8 Preschool children, children in day care, and school children who are incomplete for immunizations in A-1 Children in the process of receiving immunizations Temporary Medical Exemptions MUST have the: Physician s or clinic s name The provider s signature An expiration date (15 days after next immunization appointment)
15 Updating Temporary Medical Exemptions Immunization Guidelines 2013, p 8 When immunizations are needed, two weeks before expiration: Make a photo copy of the child s DH 680 form to maintain in the records Provide the original DH 680 form to the parent to take to the physician/clinic for updating Follow-up with the parent to assure that the DH 680 form is returned prior to its expiration date Return the updated DH 680 form to the child s record once the required immunizations are obtained and documented If appropriate documentation is not provided by the expiration date, the child is out of compliance
16 OOPS Improperly signed cards Immunization Guidelines 2013, p 9 If a preschool, child care facility, or family day care home attendee has: received at least 4 DTaPs, 3 Polios, 1 MMR, 1 VZV, appropriate HIB, and appropriate pneumococcal conjugate shots, and these shots are posted on the DH 680 form, and the card is signed/certified by a physician or clinic the form will not be judged as out of compliance if the form is signed on wrong section.
17 DH 680: Part C Permanent Medical Exemption Immunizations Guidelines 2013, p 10 Documentation of medical problems that do not allow for immunization(s) The medical problem must be valid, specific to a vaccine, and documented in Part C Must be dated and signed by a medical doctor
18 Religious Exemptions Immunization Guidelines 2013, p 12 All children who attend a child care center, family day care, or preschool and claim a religious exemption from receiving immunizations must have a signed and dated Religious Exemption from Immunization form Religious Exemption forms can only be issued from the County Health Department
19 Ensuring proper documentation... Immunization Guidelines 2013, p 9 It is the school, child care facility or family day care home operator s responsibility to ensure proper documentation is on file. This responsibility includes notifying parents of expiring temporary medical exemptions.
