Please carefully read all infrmatin belw prir t starting the nminatin submissin prcess. General Infrmatin Catherine Wrthingham, PT, PhD, FAPTA, was a change agent wh was effective, respectful, and hnest, and mtivated thers t make an impact within the physical therapy prfessin. She was als a visinary wh demnstrated leadership acrss the dmains f advcacy, educatin,, and research. The purpse f the Catherine Wrthingham Fellw designatin (FAPTA) is t hnr Dr. Wrthingham and inspire all physical therapists t attain the high level f prfessinal excellence and impact in terms f advancing the prfessin she exemplified. The FAPTA designatin is the highest hnr amng APTA s membership categries. Each nminatin packet must include specific evidence f a nminee s ne primary dmain (advcacy, educatin,, r research). The packet must als include evidence f demnstrated achievements f the primary dmain acrss at least tw f the three nn-primary dmains. Althugh nminees ften have made utstanding cntributins in mre than ne dmain, specify nly ne primary dmain. A nminee s curriculum vitae prvides a listing f the nminee s accmplishments. Nminatin and supprt letters shuld g beynd such a listing and shuld elabrate n the accmplishments t help illustrate hw the nminee has: demnstrated ne primary dmain (advcacy, educatin,, r research); made significant cntributins, achievements, r leadership in at least tw f the three nn-primary dmains; and advanced the physical therapy prfessin thrugh frequent and sustained effrts fr a perid f nt less than 15 years preceding the nminatin. Please see the sample descriptins at the end f these instructins f hw a nminee's achievements within a primary dmain and translatin acrss the ther tw f three nn-primary dmains might be reprted. This table is designed t assist with writing letters f nminatin and aid writers with accurately describing why a member shuld be elected as a Catherine Wrthingham Fellw. The examples are intended t prvide greater clarity regarding the depth and breadth f cntributins that distinguish an individual as deserving f the Catherine Wrthingham Fellw designatin (FAPTA). 2 Nminatin Submissin Details/Instructins Hw t Make a Nminatin The nminatin f any physical therapist member r any life physical therapist member as a Catherine Wrthingham Fellw f APTA must be submitted electrnically via APTA s website ( The nminatin may be submitted by any individual member, chapter, sectin r assembly f the Assciatin. Active r life member may nt nminate himself r herself and current Bard f Directrs members are nt eligible t be nminated and shall nt nminate, write letters f supprt, r endrse an individual fr this hnr. Members f the Awards Cmmittee Catherine Wrthingham Fellws Subcmmittee shall nt nminate, write letters f supprt, r endrse an individual fr this hnr. Deadline All materials cmpleted and submitted fr each nminatin must be received by the APTA Hnrs and Awards Prgram by 5:00 pm Eastern time n December 1 fr cnsideratin fr this membership categry. Please nte: Yu d nt have t enter all elements f a nminatin packet at nce. Yu can enter them as yu gather them, but be sure t save the file after each uplad. Nminatr Respnsibilities The nminatr is respnsible fr: Carefully reading the current Catherine Wrthingham Fellws plicy. This plicy may be updated every year, s it is imperative that yu review the current plicy t ensure yu are aware f any changes and accunt fr all f the criteria in yur nminatin packet. Crdinating the nminatin packet t include requesting letters f supprt and cllecting all dcuments needed fr the Catherine Wrthingham Fellw nminatin packet (see list belw). Prviding each letter f supprt writer with: 1) Catherine Wrthingham Fellws Instructins fr Writing a Letter f Supprt and 2) a cpy f the current Catherine Wrthingham Fellws Plicy. Submitting the nminatin packet by ensuring prper and timely cmpletin f all steps and cmpliance with eligibility criteria utlined in APTA s nline nminatin submissin system, including the uplading f all required dcuments by December 1, 5:00 pm Eastern time. Please try t submit early s that if yu have a prblem uplading the nminatin packet, yu will be able t address the issue and submit by the deadline. Page 2

3 Nminatin Packet: Cntents All nminatin packets must include the fur elements utlined belw: A current and cmplete curriculum vitae r resume f the nminee The curriculum vitae r resume shuld include: Identificatin f whether articles are published in refereed r nnrefereed jurnals Differentiatin between invited lectures, presentatins, addresses, and sympsia and whether these were presented natinally r internatinally Identificatin f service t the Assciatin and the level at which the cntributin was made (natinal, chapter, sectin, district) 1 letter frm the nminatr The letter frm the nminatr shuld: Clearly indicate the name f the nminated individual, including all prfessinal designatins Lay ut a clear and cmpelling case fr why the nminee shuld be granted the Catherine Wrthingham Fellw designatin. T d this, the nminatr shuld: Clearly state the nminee s primary dmain in the first paragraph. Summarize the cntent f all f the letters f supprt and include yur wn experience with the nminee t clearly illustrate hw the nminee meets each and all f the fllwing criteria fr the Catherine Wrthingham Fellw f the APTA: Criteria 1: Hw has the nminee demnstrated a primary dmain f advcacy, educatin,, r research? Please highlight and elabrate n the achievements f the nminee that are ntewrthy and standut in terms f their quality and/r set a new standard fr the prfessin.
