4 Activity Goal Identify patients who are candidates for intrathecal therapy (IT) pain management and devise and modify IT pain treatment plans to incorporate an algorithmic approach that balances efficacy and side effects, integrates an evidence-based titration strategy and addresses common barriers to care. Learning Objectives At the end of this CE activity, participants should be able to: Apply evidence-based, best practice criteria for appropriate patient selection for intrathecal pain management. Implement a slow dose-titration strategy for intrathecal pain management that maximizes efficacy while minimizing the potential for side effects. Integrate evidence-based trialing strategies into the decision-making process when developing an individualized treatment approach to pain management. The following learning objectives pertain only to those requesting CNE credit: Review the evidence-based criteria for selecting patients for whom intrathecal pain therapy is appropriate. Analyze the strategies for a slow dose-titration for intrathecal pain management. Discuss with colleagues the development of an individualized treatment approach for intrathecal pain management. Caraway, MD, PhD Medical Director Center for Pain Relief, Tri State, PLLC Huntington, WV Biography Dr. Caraway is the CEO of The Center for Pain Relief, Tri-State, PLLC in partnership with St. Marys Regional Medical Center in Huntington, West Virginia to deliver state-of-the-art health care. Although the clinic provides comprehensive, multidisciplinary services, the emphasis of the practice is interventional pain management. He is also the founder of David Caraway, MD, PhD, PLLC, located in Charlottesville, Virginia. This firm provides consulting, lecturing, medical-legal review and compliance planning with the mission of elevating the field of pain management through education, research, development and deployment of new therapies while promoting best practice strategies. The corporation has provided consultation services to medical societies, workers compensation carriers, private practitioners, hospitals, legal firms, regulatory offices, insurance companies, medical device manufacturers and the pharmaceutical industry. Dr. Caraway is a graduate of the University of Virginia School of Medicine where he received his MD and of the University of Virginia Graduate School of Arts and Sciences receiving a PhD in Biophysics. He also holds a BS in Chemical Engineering from the University of Virginia School of Engineering. He received post-graduate training in anesthesiology and pain management from the University of Virginia. Dr. Caraway is board certified by the American Board of Anesthesiology. Dr. Caraway serves as the Executive Vice President of Strategic Planning and Board of Directors member for the American Society of Interventional Pain Physicians and is member of the Board of Directors of the North American Neuromodulation Society. He is a member of and frequent lecturer at the national meetings of the American Society of Anesthesiologists and the American Academy of Pain Medicine. Dr. Caraway serves as an advisor to the Neuromodulation Therapy Access Coalition and various patient advocacy groups. Dr. Caraway is nationally recognized as an expert in the treatment of chronic pain. He lectures regionally, nationally and internationally in the field of Interventional Pain Medicine. He has authored and contributed to numerous publications in this field including recently published guidelines for the appropriate use of implantable therapies. 4

6 Gladstone C. McDowell II, MD Medical Director Integrated Pain Solutions Columbus, OH Biography Dr. McDowell is the Medical Director of Integrated Pain Solutions in Columbus, Ohio. Dr. McDowell attended medical school at the University of Texas Health Science Center at San Antonio. He completed his residency in Anesthesiology at the University of Colorado in Denver. Dr. McDowell completed a Urologic Oncology Fellowship at the University of Texas, MD Anderson Cancer Center in Houston. He performed a Pain Management Fellowship in the Department of Anesthesiology at the Ohio State Pain Control Center. Dr. McDowell is the Former Medical Director of Grant Pain Management Center and Director of Pain Services for Grant Medical Center, Ohio Health. He is on the Board of the Texas Medical Institute of Technology, Community Emergency Health Initiative, and the Ohio Clinical Advisory Board of the National Multiple Sclerosis Association. Dr. McDowell has held faculty appointments at the National Patient Safety Congress meetings and National Institute for Heatlhcare Fraud. He has been the Primary Pain Consultant for Careleaders and TMIT. He recently taught at the FBI Health Care Fraud Managers Meeting and served on the 2007 and 2011 International Polyanalgesic Consensus Panel. Dr. McDowell is certified as a Patient Safety Officer by the Institute for Healthcare Improvement in He is a member of the American Academy of Pain Management, American Society of Regional Anesthesia, and North American Neuromodulation Society. Dr. McDowell s research is published in peer-reviewed medical journals. He has authored book chapters regarding Urology and Anesthesiology.
