Questions about Compliance/Adherence The needs of patients regarding packaging, such as accessibility, convenience and medication control have been sufficiently taken into account by the pharmaceutical industry Ο I agree Ο rather agree Ο rather disagree Ο disagree 3

Agenda Compliance / Adherence / Concordance What s a Word s Worth Understanding the barriers for Compliance The Compliance ROI Strategies to Improve Compliance a Cost Effective Way to Improve Compliance/Adherence 5
What do we mean by Compliance? We define proper patient compliance as taking the right dose of the right medication at the right time for the prescribed duration of the treatment. 6
Compliance / Adherence / Concordance What s a Word s Worth. Compliance is defined as the extent to which patients follow medical instructions. In contrast adherence is determined as the extent of the conformance between the actual and with the health care provider agreed patient s behavior. Deutsche Agentur für Health Technology Assessment des Deutschen Instituts für Medizinische Dokumentation und Information DIMDI, Köln
Compliance / Adherence / Concordance What s a Word s Worth Compliance doesn t acknowledge that patient input is routinely given and that it s necessary. With more chronic diseases and more powerful and complex therapies, greater understanding and active participation by patients will ultimately yield greater efficacy and safety. 8
Compliance / Adherence / Concordance What s a Word s Worth concordance was introduced, implying that the prescriber and patient should come to an agreement about the regimen that the patient will take. It is not clear, however, whether this seemingly laudable aim is actually beneficial to the patient, and although it is generally assumed to be, the true benefit to harm balance of this strategy has not been determined... Concordance also carries the implication that patients should take greater responsibility for their management, even though not all are willing to do that. 9 Compliance, concordance, adherence Jeffrey K Aronson, Editor-in-Chief, British Journal of Clinical Pharmacology University Department of Clinical Pharmacology, Radcliffe Infirmary, Woodstock Road, Oxford OX2 6HE, UK
Adherence Key to Outcomes The gap between best care and usual care is large for many important diseases. In particular, poor adherence remains a significant, inadequately addressed, cause of the care gap. Healthcare Quarterly, 8(1) 2005:
Adherence Key to Outcomes About half of all patients with chronic diseases stop refilling prescriptions by one year. Irrespective of causation, the ultimate result of care gaps is the same - less than optimal clinical outcomes and associated lost opportunities for improved quality of life and productivity. Healthcare Quarterly, 8(1) 2005:
Compliance Prevention WHO: Through innovation, health care systems can maximize their returns from scarce and seemingly non-existent resources by shifting towards activities that emphasize prevention and delay in complications. 12
Understanding the Barriers for Compliance Compliance/Adherence depends largely on information about the health condition and the trust between the patient and the health care provider. On average practitioners spend less than 60 seconds per prescription for conveying compliance related information to their patients. Heuer H.O., Heuer S., Lenneke K.,: Compliance in der Arzneitherapie. Wissenschaftliche Verlagsgesellschaft mbh. Stuttgart
Understanding the Barriers for Compliance Consultations usually end with a prescription and most patients are left alone with their questions and concerns regarding therapy, benefits and risks of the medication, interaction with other drugs and side effects. Heuer H.O., Heuer S., Lenneke K.,: Compliance in der Arzneitherapie. Wissenschaftliche Verlagsgesellschaft mbh. Stuttgart
Understanding the Barriers for Compliance Product related: Numerous attributes of the drug product that can influence adherence include the drug s efficacy, side effects, dosing and ease of administration. Patient related: Memory is critical to taking medication on the correct schedule and in the correct way. Any impairment in memory or cognitive functioning will adversely affect adherence. Patient Physician Communication related: up to 60 percent of patients cannot correctly report what their physicians expected of them 10 to 80 minutes after receiving the information.
