6 What do we mean by Compliance? We define proper patient compliance as taking the right dose of the right medication at the right time for the prescribed duration of the treatment. 6

7 Compliance / Adherence / Concordance What s a Word s Worth. Compliance is defined as the extent to which patients follow medical instructions. In contrast adherence is determined as the extent of the conformance between the actual and with the health care provider agreed patient s behavior. With more chronic diseases and more powerful and complex therapies, greater understanding and active participation by patients will ultimately yield greater efficacy and safety. 8

9 Compliance / Adherence / Concordance What s a Word s Worth concordance was introduced, implying that the prescriber and patient should come to an agreement about the regimen that the patient will take. It is not clear, however, whether this seemingly laudable aim is actually beneficial to the patient, and although it is generally assumed to be, the true benefit to harm balance of this strategy has not been determined... 10 Adherence Key to Outcomes The gap between best care and usual care is large for many important diseases. In particular, poor adherence remains a significant, inadequately addressed, cause of the care gap. Healthcare Quarterly, 8(1) 2005:

Irrespective of causation, the ultimate result of care gaps is the same - less than optimal clinical outcomes and associated lost opportunities for improved quality of life and productivity. On average practitioners spend less than 60 seconds per prescription for conveying compliance related information to their patients. 15 Understanding the Barriers for Compliance Product related: Numerous attributes of the drug product that can influence adherence include the drug s efficacy, side effects, dosing and ease of administration. Patient related: Memory is critical to taking medication on the correct schedule and in the correct way. Any impairment in memory or cognitive functioning will adversely affect adherence. Patient Physician Communication related: up to 60 percent of patients cannot correctly report what their physicians expected of them 10 to 80 minutes after receiving the information.
