2 #About the Conference With just one geriatrician for every 10,000 adults over 75, primary care physicians are being called on to provide geriatric care for a rapidly aging population This conference will explore hot issues that are impacting clinical practice in geriatric medicine and challenging providers caring for Hawaii s older adults When is my patient no longer capable of making informed medical decisions? How do I screen for cognitive impairment quickly in the outpatient setting? What do I need to know about the POLST? What are the top 10 evidence-based recommendations in geriatric medicine? What are the risks / benefits of Vitamin D, Calcium, Statins and novel Anticoagulants? What technology is available to improve my geriatric practice? What are appropriate therapies for the elderly patient? How do I assess for elder abuse? Physicians and other health care professionals who care for the elderly will be empowered to engage patients in the decision making process and put into practice Choosing Wisely - the new guidelines for appropriate treatment decisions in geriatric medicine #Educational Objectives At the end of this educational activity, participants will be able to: Session I: Discuss three ways to estimate prognosis utilizing new technology tools; and describe its application to practice Assess for cognitive impairment efficiently in the outpatient setting Explain recent changes in the Physician Orders for Life-Sustaining Treatment (POLST) Initiate advance care planning discussions in the PCP office Implement new guidelines on common geriatric treatment issues Session II: Prescribe appropriately for geriatric patients based on new evidence Recognize when to order physical therapy, occupational therapy, and speech therapy Determine when a patient is no longer capable of making informed medical decisions Assess level of care (determine when to consider placement, when a patient is not safe to live alone, and when to call APS)
