1 DIAGNOSTIC CLINICS FOR COGNITIVE IMPAIRMENT LOCATED IN SOUTHEASTERN WISCONSIN Obtaining an accurate diagnosis of progressive memory loss or cognitive changes is often the most important first step made by individuals and families in their efforts to secure the education and support needed to cope with Alzheimer's disease or the symptoms of dementia. Diagnostic clinics provide evaluation and management services to individuals and their families. A typical individual will undergo neuropsychological testing, a comprehensive history and physical examination, laboratory and, if appropriate, x-ray testing during the evaluation. The involvement of a social worker is a routine and essential feature. Mobile Medical Specialist Phone: Park Ave Pewaukee, WI Fax: (262) Note: This service is for member s who have Medicare or Medicaid (or other Medigap or private insurance) and who will not go out (physical disability, cognitive impairment, mental health limitations) or do not have the stamina for trips to an MD office. 3 Veterans & Spouses of Veterans Clement J Zablocki VA Medical Center serving Milwaukee County & surrounding Counties 5000 West National Ave Primary Care Clinic Main phone number is Veterans or veteran s spouses must first be seen at the VA Primary Care Clinic and then one can request a referral to the Neurological & Memory evaluation clinic. NOTE: 1. Medical Providers outside the VA System: If the veteran or spouse also has an established Physician and or other Medical providers outside the VA Medical services system, it does not mean that the member or spouse has to give up these providers. It does mean that the veteran or spouse has to be seen at least 1x a year at the VA Primary Care clinic. 2. Medications thru the VA are often much cheaper than thru Medicare D. Health Care Benefits Overview VA publishes the "Health Care Benefits Overview" booklet, which is designed to provide general information and frequently asked questions about VA s medical benefits package available for all enrollees, and other information such as eligibility for enrollment.
