1 PROVINCIAL LEAD AND LHIN LEADER BIOS May 4, 2006 Dr. Bernard Lawless, Provincial Lead Dr. Bernard Lawless completed a residency in general surgery at the University of Western Ontario (UWO) and then did Fellowship training in trauma surgery at Vancouver General Hospital. He returned to UWO to complete a Fellowship in critical care medicine before moving to South Africa to work at Groote Schuur Hospital in Cape Town for further experience in trauma surgery and penetrating traumatic injuries. After returning to Toronto, Dr. Lawless completed a Master s degree in Health Science (Health Management, Policy and Evaluation) at the University of Toronto. Dr. Lawless currently practises general surgery and traumatology at St. Michael's Hospital and critical care medicine at Toronto East General Hospital. Dr. Michael Miletin, Central West LHIN Leader Dr. Michael Miletin is currently the medical director of the division of intensive care and respirology at William Osler Health Centre in Etobicoke and Brampton. Dr. Miletin received his M.D. degree from the University of Toronto in He completed his residency in internal medicine and a subsequent fellowship in respiratory medicine at the University of Toronto, and then spent a year at the University of California at San Francisco where he received fellowship training in critical care medicine. Dr. Miletin has been on staff at William Osler Health Centre since His interests include the standardization of care processes, continuous quality improvement, and the implementation of research findings and clinical guidelines into daily evidence-based practice.

2 Dr. Redouanne Bouali, Champlain LHIN Leader Dr. Redouanne Bouali is currently head of the critical care department at the Ottawa Hospital and chair of the critical care division at the University of Ottawa. He graduated in medicine at the University of Algiers in March 1987, obtained the Swiss Federal Diploma in 1995, and graduated in internal medicine at the University of Lausanne in 1996 and in Critical Care in Dr. Bouli continued his education with a clinical fellowship at the CHU-Lausanne in internal medicine and emergencies. He then moved to Toronto for two years of clinical fellowship in critical care. He was appointed as chief intensivist of Le Centre hospitalier de l Université de Montréal from 2000 to 2005 and promoted in 2001 as co-chair of the internal medicine department and in 2004 as program director of critical care of University of Montreal. Dr. Bouali is one of the founders of the Canadian Collaborative Group for Safety and Quality Improvement and is a leader in the Quebec and Canada Continuous Quality Improvement branches. He is also chair of the organization Committee of the World Congress of Burn Injuries (ISBI) Montreal Dr. John Muscedere, South East LHIN Leader Dr. John Muscedere has been a critical care physician for 14 years with extensive experience in both academic and community critical care areas. He is currently an intensivist at Kingston General Hospital and assistant professor of medicine at Queen s University in Kingston, positions that he has held since August His research interests are quality improvement and knowledge translation in the intensive care unit (ICU). He is currently leading a project to develop clinical practice guidelines for the prevention, diagnosis and treatment of ventilatorassociated pneumonia. He is also chair of the intensivist advisory group for Trillium Gift of Life Network. Before relocating to Kingston, he was director of the ICU at Hotel-Dieu Grace Hospital in Windsor from 1993 to He also served as a member of the Ministry of Health Critical Care Restructuring Task Force.

3 Dr. Michael Scott, North West LHIN Leader Dr. Michael Scott is currently director of critical care at Thunder Bay Regional Health Sciences Centre, a practising intensivist and anesthesiologist, and a member of the Ontario Critical Care Expert Advisory Panel. He was appointed director of critical care at Thunder Bay Regional Hospital in 1998, and has developed and implemented the intensivist-directed critical care service at Thunder Bay Regional Health Sciences Centre. Previously, Dr. Scott was chief of anesthesiology from at Thunder Bay Regional Hospital. He continues to be an investigating coroner since 2001 for the Thunder Bay Region. Mississauga Halton LHIN Leader To Be Confirmed Dr. Thomas Stewart, Toronto LHIN Leader Dr. Thomas Stewart is associate professor of medicine and anaesthesiology at the University of Toronto. He is also director of critical care medicine at Mount Sinai Hospital and the University Health Network in Toronto. Dr. Stewart has been a key advisor on the ministry s critical care transformation strategy since the inception. He received his medical degree with honors from the University of Ottawa in Dr. Stewart completed residencies in internal medicine and critical care medicine at the University of Toronto. He has received multiple honors, awards and grants. Dr. Stewart s research focuses includes lung protective modes of mechanical ventilation, unconventional approaches to mechanical ventilation, critical care medicine networks, leadership skills and implementing change. Dr. William Plaxton, Waterloo Wellington LHIN Leader Dr. William Plaxton graduated from Upper Canada College in 1986 with a view to entering the medical profession. He graduated from the medical school at the University of Western Ontario in 1994 and completed training in internal medicine and fellowship training in critical care medicine and trauma. He joined the critical care unit of St. Joseph s Hospital in London and concurrently went back to school full-time to complete a Master s of Business Administration at The Richard Ivey School of Business. Dr. Plaxton is currently the chief of critical care medicine at Grand River Hospital and is the acting joint chief of medicine for Grand River and St. Mary s Hospitals.

4 Dr. Donna McRitchie, Central LHIN Leader Dr. Donna McRitchie is the director of critical care at North York General Hospital and an assistant professor in the department of surgery at the University of Toronto. She completed her Bachelor of Science, MD and Master of Science degrees at the University of Toronto as well as her specialty training in General Surgery. Her fellowship training included subspecialty training in trauma at the University of Maryland Shock Trauma Centre in Baltimore, and critical care training at Harvard University in Boston. She has worked in trauma and critical care in Toronto for more than 10 years, first at St. Michael s Hospital and now at North York General Hospital. Dr. Howard Clasky, Central East LHIN Leader Dr. Howard Clasky graduated from the University of Toronto medical school in He subsequently received his certification in internal medicine and emergency medicine. He completed a two-year critical care fellowship in 1999 and then assumed his current position as director of the ICU at the Scarborough Hospital (General Division). His primary focus is performance improvement in critical care. Dr. Clasky is also a part-time medical director for Criticall. Dr. Michael Sharpe, Erie St. Clair LHIN & South West LHIN Leader Dr. Michael Sharpe graduated from medical school in 1980 and received his speciality training in anesthesiology and critical care at the University of Western Ontario. He is currently a fulltime intensivist at the London Health Sciences Centre (University Hospital) and professor of anesthesia and perioperative medicine at the University of Western Ontario. He is also a member of the critical care advisory board for Trillium Gift of Life Network and is chair of the organ/tissue management committee at the London Health Sciences Centre.

5 Dr. Peter Kraus, Hamilton Niagara Haldimand Brant LHIN Leader Dr. Peter Kraus joined the Hamilton Health Sciences Corporation as chief of critical care medicine in July Prior to coming to Canada in 1993, he held the position of deputy head/director of the ICU at Baragwanath Hospital in Johannesburg, South Africa. He and his family relocated to Regina where he worked initially as an intensivist at the Pasqua Hospital before his role expanded to becoming site director of two ICUs in the city. Dr. Kraus is also cochair of the Hamilton Health Sciences Corporation s clinical ethics committee, and sits on a number of committees including the medical advisory committee, the McMaster critical care advanced training committee, and the Hamilton Health Sciences ICU research advisory committee. Provincially, Dr. Kraus is a member of the critical care expert advisory panel to the Ministry of Health and Long-Term Care, and most recently, site leader for the ministry s critical care response team, Hamilton General Hospital. Dr. Giulio DiDiodato, North Simcoe Muskoka LHIN Leader Dr. Giulio DiDiodato is the clinical director of the critical care unit at the Royal Victoria Hospital in Barrie. He also provides more than 30 weeks a year of intensivist service in various ICUs across the province, along with providing consultative services in infectious diseases and internal medicine. His clinical and administrative experience is unique and invaluable to understanding the challenges that exist in the organization and delivery of critical care services in the different provincial regions and community hospital environments. Dr. DiDiodata completed his fellowships in critical care medicine and infectious diseases in Dr. David Boyle, North East LHIN Leader Dr. David Boyle has practised as an anesthesiologist-intensivist for 13 years at the Hôpital Régional de Sudbury Regional Hospital. Administrative experience included being chief of staff of the Sudbury Regional Hospital Dr. Boyle was also executive director of project development responsible for leading the Northern Ontario School of Medicine through their initial accreditation ( ). He has been an active member of the Provincial Critical Care Committee since Dr. Boyle is actively involved in postgraduate and undergraduate medical education. He is an associate professor of clinical sciences at the Northern Ontario School of Medicine. -30-
