1 Information Grid People with dementia and their carers enter and exit the health and social care system at different stages of the dementia journey and via a variety of entry points, including the emergency services, financial institutions and Post Offices. They do not necessarily follow a natural trajectory from early stage and early diagnosis through to late stage and long-stay care. For example, not everyone is diagnosed at an early stage of dementia, so information on granting a power of attorney will not be appropriate for a person who no longer has the capacity to grant one. Similarly, someone in the very early stages of dementia is unlikely to want detailed information about choosing a care home until later, although it could form part of the person s forward planning. Timely, relevant and suitable information should be available at whatever the individual s entry point, together with knowledgeable staff (and possibly volunteers) who can assess each individual s understanding and readiness to receive information and provide a verbal explanation if required. While there will be common questions and concerns about the implications of a dementia diagnosis and living with the illness, this Information Grid does not suggest a standard information pack nor does it advocate inundating people with piles of leaflets to make sure all the likely issues are covered. Information should be tailored to the individual s needs. The Grid is divided into seven stages ranging from pre-diagnosis (where someone may be worried about their own or someone else s symptoms) through to end-of-life and beyond (reflecting the need for carers to have information and support in bereavement). These divisions are used for convenience and should not be seen as rigid categories there will be considerable overlap between stages and considerable differences between individuals going through any of these stages. The Grid is intended to be a tool for staff involved at any stage of the journey to alert them to: the sorts of information likely to be needed by both the person with dementia and his/her family at each stage of the journey where staff, people with dementia and carers can source relevant, suitable information services and organisations to which staff can signpost people with dementia and their families which staff and services may be involved in disseminating particular types of information. It should also act as an aide-memoire to remind staff about the issues and questions likely to arise and be prepared for them.

Other information may need to be provided verbally by staff or from locally produced information material. Running through each stage is the need to make information available in different formats and languages as required

MCI is included here as a high percentage (estimates vary) of people with MCI go on to develop dementia. 7 Immediate Post Diagnosis Information at this stage is more likely to be provided face to face either just with the service user or with another (eg family member) present and should offer the chance to talk about the diagnosis. Whoever is providing the information needs to recognise/determine what the person already knows. Immediate post diagnosis doesn t necessarily mean early stage diagnosis could be delayed for many reasons. Some information is more appropriate at early or later stages of the illness.
