2 Description Persons over the age of 65 comprise the fastest growing segment of North Carolina s population. Older adults often have different illnesses, physiology, and care needs. This conference will enable participants to provide more effective evidence-based, quality care and focuses on depression, unique presentations of the older adult, falls risk and reduction, transitions of care, and high risk medications. Who Should Attend? This conference has been planned for Physicians, Nurse Practitioners, Physician Assistants, Registered Nurses and other clinical staff who provide care to older adults in rural North Carolina. Course Objectives Upon completion of this knowledge-based program the participant should be able to integrate into practice new development in the following areas: Recognizing and Treating Depression in Older Adults Define the magnitude and special circumstances of depression in older adults. Recognize atypical signs and symptoms of major depression in older adults. Discuss treatment options for depressed older adults. Is It Just Old Age? Deviations from Normal May Not Be Normal Identify deviations of normal assessment findings in the older adult. Review the unique presentations possible in the older adult. Discuss an evidence-based approach to geriatric assessment. High Risk Medications and Alternatives in the Elderly List potentially inappropriate medications on the Beers list and suggested alternatives. Describe the changes to the 2015 Beers criteria. Apply the Beers criteria to patient cases. Avoiding Slipping, Tripping and Falling: Fall Risk Assessment and Reduction Discuss the importance and benefits of fall prevention activities in the older adult. Discuss strategies to assess for fall risk. Review post fall evaluation and management in the older adult. Describe prevention measures to reduce fall risk in the older adult. Transitions of Care: What s the Secret for Success? Define cross-continuum transitions of care. Discuss the impact of effective transitions of care, including the facilitating and inhibiting factors of care outcomes. Discuss strategies for ensuring successful transitions of care.

Other Professionals EAHEC Contact Hours: Certificates reflecting 5.5 contact hours of education will be awarded upon completion of the evaluation.
