4 Curriculum Vitae Déon Hayley Page Department of Award Excellence in Clinical Care and Education 2012 Listed in Castle Connelly Top Doctors TEACHING ACTIVITIES: Philosophy of Teaching: My teaching focus is related to issues of aging and end of life care. I teach adeptly in the classroom, in lecture halls but most importantly with patients in the outpatient clinic, hospital, nursing home or home. And, I am expert in teaching learners in many disciplines as well as the public. I have developed expert level teaching in and Palliative over the past 22 years. Notably, I have had the great opportunity to teach hundreds of medical trainees. I have not only educated and personally mentored in many clinical and academic settings; I am now a key faculty member for the Fellows and also teach EVERY single Medical Student at KU in Kansas City and EVERY Internal resident through his or her rotation in. I have been intimately involved with teaching approximately 500 Internal Residents over the past 20 years (approximately 25/year) as is one of the only required rotations for all Internal Residents. I have precepted residents at clinical sites (clinic, hospital, nursing home, house calls), lectured on topics, directed lecture series and clinical experiences, lead group discussions and developed curricula. The curriculum for Palliative Home Care I developed and implemented at both the University of Chicago and the University of Kansas, the first of its kind, has met with rave reviews. Since coming to the University of Kansas I have been able to be a part of every medical student s education through a novel Palliative Care longitudinal simulated patient curriculum which I have been instrumental in crafting and administering with Dan Swagerty and Jessica Kalender-Rich. This endeavor has been successfully supported by grants and the Medical School and we have been able to disseminate our work broadly so that I have National and International Recognition for teaching and Palliative Care. A very, very important part of my career has been leading and mentoring. I have been intimately involved in the education of Fellows for 20 years through clinical precepting, recruitment, leading house call instruction, career counseling and successful mentorship in research. Through all the many trainees of many disciplines and levels that I have been fortunate to teach and Palliative over the last 20 years, I have been able to spread knowledge very broadly as most of these trainees have gone to be teachers themselves and are now able to teach what they have been taught. My goal continues to be that I will be able to teach in many settings from the classroom to clinical setting to the community to Regional and National Forums. Through doing so, we can improve the care of the Older Adult.

University of Kansas 2010-present I have been instrumental in the expansion of the Barney Smith palliative care simulated patient experience. All Medical Students now have a longitudinal experience throughout 5 scenarios from the 2 nd through 4 th years. I developed and teach the didactic portion, Palliative Care Workshop in the Clerkship (Gero900) and have primary responsibility for the third encounter as well as co-lead the fourth and fifth encounters. I review student performance, provide continuous quality improvement to the staff and actors through the skills lab regarding their performance. With Jessica Kalender-Rich, I have also cooperatively developed and lead Barney Smith in which all the Internal interns participate. This has been extremely well received Present As Fellowship key faculty in Internal, I direct much of the Fellows training. I have developed and lead a scholarship workshop that meets once a week. Topics covered include research methods, development of a scholarly project, presentation and teaching skill cultivation, career counseling, administration and communication. I also lead their patient care conference, and core curriculum as well as precept longitudinal clinic. In addition, I am integral in the development and continuous quality improvement of the fellowship experience as well as assisting with scheduling, recruitment and interviews of candidates Present Lead COACH (Care of the Older Adult Continuing at Home) program which I developed where Internal Residents have instruction in home care and make posthospitalization to elderly patients. 21 residents who completed 16 rated it as Superior and 5, Good Present Key faculty in Internal Required rotation. I share responsibility with Dr. Kalender-Rich for administration and scheduling of the rotation, orientation of residents, and development and updating of curriculum.

Professional Service (Patient Care): I am proud of my service achievements and rewarded by the provision of care and leadership I have been able to provide through extensive experience and expertise in patient care, service to the community and service to the local as well as academic community at large. I have established career level expertise in the area of clinical care of the Older Adult. My primary practices in the outpatient and home setting has led to mastery of longitudinal care of the frailest, most complicated patients with a multiplicity of health, functional and social challenges. I have led the start-up of Clinic in Internal in the Medical Office Building and have worked with staff and leadership to establish a distinguishable practice. I see patients in this setting 3 half days per week and have earned above average patient satisfaction scores 4.61 (average 4.51) and consistently accumulate RVU totals above the mean even though I have a minimum of 2 learners with me. I superceded my Target RVUs last year by 30% (see table). I have been one of the first physicians in Kansas City to provide an academic house call practice at present I make post-hospital transitional house calls 1-2 days per month. My service experience and expertise also extend to service in the community volunteering and teaching topics related to aging. I have been a popular speaker for lay audiences. I have served in Leadership capacity as Medical Director of our clinic at University of Chicago and now, at KU as Section Leader of in the Division of General and Geriatric. I have been an invited speaker in national settings. And, I have a varied service record on national committees though currently directing my efforts at the Education Committee of the American Academy of House Call Physicians.

17 Curriculum Vitae Déon Hayley Page 17 PATIENT CARE At University of Chicago Outpatient clinic (Windermere Senior Health Center/Outpatient Senior Health Center) I have provided longitudinal care and Comprehensive Assessments of frail elderly patients 3-5 half day sessions per week. I maintained a full, vital practice of these complex geriatrics patients with multiple medical and social issues and was consistently one of the geriatricians who saw the most patients. PEACE Palliative Care outpatient consultation clinic I developed, initiated and lead this multi-disciplinary consultation clinic where patients with advanced disease were evaluated by Geriatrician, nursing and social worker expert in complex care of the elderly patient. One half-day per week. Nursing home (Montgomery Place) Care of patients in skilled nursing as well as long term care. I maintained a panel of about 15 patients. Hospice (CareMed) I provided consultative care of hospice patients. Our ongoing census was approximately 15 patients Inpatient General Internal I was a teaching Attending for approximately one month per year. Our average daily census was approximately 15 patients. Inpatient Consultations I provided consultations to inpatient teams about geriatrics issues. I was on service about 6 weeks per year and averaged about 5 new consults per week Inpatient Palliative Care Consultations I provided consultations to inpatient teams regarding palliative care and symptom management. I was on service about 6 weeks per year and averaged about 9 new consults per week. House Calls I started a multidisciplinary palliative home care service providing consultative and longitudinal care to homebound elderly. This was completed in conjunction with an advance practice nurse and social worker and we maintained an ongoing census of about 40 patients. At University of Kansas Outpatient longitudinal and consultation clinic 2010-Present clinic 3 half day sessions per week seeing elderly patients in longitudinal care as well as consultations. This clinic in Internal was a brand new endeavor at the University of Kansas and I am happy to report my patient satisfaction scores are above average. As well, I have many referrals from within the medical center to care for patients with complex medical and social issues. Every quarter, I have made well above the average RVU for. House Calls Once monthly visits to homebound elderly. I am one of the only physicians in Kansas City who makes housecalls on a regular basis. Nursing home

18 Curriculum Vitae Déon Hayley Page 18 I am the back-up physician for patients at a skilled nursing/long term care facility. I round about once a month. Consultant in Palliative Care With multidisciplinary team, provide consultations and active assistance with care of the very ill and dying patients to inpatient teams regarding palliative care and symptom management. RESEARCH AND SCHOLARLY ACTIVITIES: Opening Statement: My scholarly and research interests relate to optimization of care of the frail elderly, many of whom have dementia and/or are near the end of life. My work has primarily focused on the development and study of programs to reach frail elderly and teach about this care. There is perfect overlap between the program development and then provision of care to frail elderly and teaching about it. As seen under Teaching/Education and Service, I was involved in the design of PEACE (Palliative Excellence in Alzheimer Care Efforts), a clinical and research program which integrated palliative care early in the course of dementia. This program became nationally recognized and we were able to study and publish many aspects of this care. I obtained funding and initiated my own program, PATCH (Palliative Access Through Care at Home) to provide excellent care and palliative care for homebound elderly in Chicago and teach about this care to medical trainees. I successfully developed the curriculum and program and studied the effects of the program on patient care. Patients were highly unlikely to die in a nursing home and very pleased with palliative support at home At the University of Kansas, I have built on earlier work in the areas of care of the frail elderly near the end of life. I not only obtained funding and developed a teaching house calls program, COACH (Care of the Older Adult Continuing at Home), but have studied and presented this work in National forums. And, we have studied our results teaching the Palliative Care standardized patient Barney Smith and are disseminating that work broadly as well. 1.
