5 OVERVIEW: Building Balance Program Led by a physiotherapist 1 PTA & trained volunteers 2 days/week x 6 weeks Max 12 participants

6 OVERVIEW: Building Balance Program Balance & Strengthening Exercise (as individualized as possible) Education Home exercise (booklets)

7 Target Population Community-dwelling seniors, ideally BEFORE they have had their first fall, when balance is just starting to decline Admission Criteria (1 or more of the following): Fearful of falling 1 or more near falls in the past 3 months 1 or more falls in past year

17 Bone Fit Bone Fit is an evidence-informed exercise training workshop for health professionals. Bone Fit teaches the design & adaptation of safe exercise programs for people with osteoporosis/those at risk of fractures due to bone loss. Workshop demonstrations include simple transitional movements, strengthening exercises for the back, activities of daily living and modifications to Yoga/Pilates for people with or at risk of osteoporosis.

Exercises to help prevent falls Patient booklets and Tear off pads for practitioners to give to patients- newest version available through OC

29 Recommendations to manage osteoporosis with exercise: Strength exercises at least 2 times per week to keep you strong and fit Posture exercises every day to keep you standing tall and not stooped Balance exercises every day to keep you stable on your feet, walk more easily. Good balance helps prevent falls. Aerobic physical activity that is weight bearing (moderate to vigorous intensity) at least 150 minutes per week to improve your overall health. It can reduce your risk of disease. It may improve your bone strength.

42 Safety Starts at Home Safety Check Project CHATS - HAMP Program, in collaboration with South Simcoe Police with be providing safety checks in the months of September and October Working on funding partnership with Town of Bradford West Gwillimbury for this joint initiative. Comprehensive outdoor/indoor safety check of home at no cost to clients. South Simcoe Police will provide outdoor checks assisting with Crime prevention for example motion lighting and cutting back shrubs.

43 Safety Starts at Home Town of BWG would provide funds for batteries, latches, smoke detectors and CO detectors. CHATS Home Adaptation and Maintenance Program will provide: Checks of batteries for CO and Smoke Detectors Free in-home Safety Assessments Maintenance of property Adaptations (bath chairs, grab bars, bath mats, hand held shower heads) Increased community awareness about falls prevention aligning with the South Simcoe Police and Township Resources.

44 Safety and Falls Prevention One of CHATS strategic goals is to continue to develop partnership with other community partners to ensure safety and falls preventation for all clients.
