2 To Our Community Members: Although the healthcare landscape is always changing, one constant remains: For close to 100 years, Deborah has been the leader in New Jersey and the region for patient care and innovative health solutions. Deborah s dedication to the people and communities we serve remains as strong today as ever. In 2013, our Community Health Needs Assessment Survey set forth certain points of emphasis for serving the health needs of Deborah s surrounding communities. It is with great delight that I share with you this update on our Community Health Improvement Plan ( CHIP ). This report highlights just some of the many ways we are reaching out to and reaching into our communities to meet health needs. In our CHIP, Deborah has identified five important priority areas of emphasis: 1. Addressing health needs in Deborah s immediate service area; 2. Expanding outreach throughout Deborah s primary and secondary service areas; 3. Expanding access to recently implemented programs; 4. Establishing new programs to meet identified health needs; and 5. Pursuing partnerships to meet identified health needs. In the last two years, we have made considerable progress in achieving meaningful improvement in these areas. I am also proud to report that from the initiation of our CHIP in January of 2014 through December 2015 and our other efforts, Deborah has invested/provided more than $37.5 million in benefits to the community for improved access and improvement in the health of our residents. While we take great pride in our accomplishments to date, we are always striving to do more. Our commitment to excellence and our commitment to the community and each of you is ongoing. In the years to come, we will continue to evaluate our CHIP to make sure we are fully serving the health needs of our surrounding communities, changing and improving the plan whenever needed. 3 Deborah Heart and Lung Center Community Health Improvement Plan (CHIP) Update Deborah's Implementation Plan contains five (5) focus sections: 1. Addressing the Needs in Deborah's Immediate Service Area 2. Expanding Outreach throughout Deborah's Primary and Secondary Service Areas 3. Expanding Access to Recently Implemented Programs 4. Establishing New Programs to Meet Identified Health Needs 5. Pursuing Partnerships to Meet Identified Health Needs Detailed spreadsheets with specific data is available. CHNA Priority Area 1: Addressing the Needs in Deborah s Immediate Service Area Identified Needs: Deborah s 2013 Community Health Needs Assessment ( CHNA ) identified high blood pressure, adult diabetes and aging-related health problems as the top three health issues facing residents living in Deborah s immediate service area of Browns Mills/Pemberton Township. Deborah identified residents lack of awareness of available resources at Deborah as a possible impediment to community members receiving education and accessing care for the identified health issues. CHIP Action: Organized and Hosted a Community Day In collaboration with other local community providers, Deborah hosted a Community Day in 2014 to improve awareness of the three identified health issues and related health services. The Community Day helped educate area residents regarding their ability to access Deborah s services, the recent expansion of Deborah s service line, and enhancements in Deborah s outpatient clinic area. At the Community Day, Deborah provided a variety of educational programs, including information on available hospital services, diabetes education, heart health and disease programs and pediatric health outreach. Deborah offered attendees health screenings, including blood pressure, body mass index, glucose, balance and pulmonary function testing. Deborah offered tours of its clinic areas to help the community better understand the services available there. Food and other fun activities were also part of the Community Day offerings. CHIP Action: Organized the Garden Farmers Market To both highlight and address the link between diet and high blood pressure, adult diabetes and agingrelated health problems, Deborah sponsored the 2014 Heart of the Garden Farmers Market in partnership with Pemberton Township. The ten-week farmers market brought healthy, locally-grown fresh fruit, - 1-
4 vegetables and organic meats to the community. The weekly event also offered health screenings for high blood pressure and body mass index (BMI), as well as diabetes and nutritional education. Two hundred twenty-seven screenings were performed. The farmers market was not offered in 2015 because of the inability to attract sufficient vendors and the ready availability of local farm stands throughout Burlington County. Deborah will continue to offer health screenings at different local events. CHIP Action: Providing On-Site Screenings To further allow community access to health screenings and to help community members understand more fully the many services offered by Deborah, in 2014 we offered onsite Peripheral Arterial Screening, blood pressure screening and ankle-brachial testing. What s Next: Deborah s 2016 CHIP goals include continuing to explore opportunities to work with our local community in our immediate service area to improve health awareness and outcomes related to the three identified health needs noted above, and to educate the community about Deborah s related services. CHNA Priority Area 2: Expanding Outreach throughout Deborah s Primary and Secondary Service Areas CHIP Goal: In addition to other outreach efforts, Deborah set a goal in its CHNA to conduct an additional 700 patient screenings in This number is a 20% increase compared to the average number of screenings in previous years. The additional screenings were targeted in the following four areas: high blood pressure, adult diabetes, adult heart disease and cancer Annual Screenings 6,470 5, CHIP Annual Screening Goal: 4,200 Annual Screenings Prior to 2014: 3,500-2-
5 CHIP Action and Results: We are pleased to report that in 2014, sixty-six outreach events screened a total of 5,789 people, exceeding our goal of 4,200 screenings. These screenings resulted in important health interventions. For example, screenings at the Deborah s Women s Health EXPO identified individuals receiving referrals for further care in the following areas: 60 balance, 14 sleep, 18 pulmonary, 10 peripheral arterial disease, 6 cardiac (with an additional 28 people recommended for additional cardiac follow-up), 1 pediatric hypertension and 30 diabetes (with an additional 12 identified for follow-up testing). In 2015, sixty-two outreach events screened 6,470 people, another substantial year-over-year increase. Outcomes results from Deborah s Women s Health EXPO identified individuals receiving referrals for further care in the following areas: 23 balance, 33 sleep, 10 pulmonary, 24 peripheral arterial disease, 19 cardiac, 16 diabetes and 9 lung cancer. Outcomes results from the New Jersey Education Association s annual convention post-screening questionnaire reinforced the benefits of Deborah s screening efforts. Of the 537 individuals receiving blood pressure screenings, 110 had abnormal results, with 86 percent of those indicating they would follow-up for additional care. Of the 314 individuals receiving pulmonary screening, 107 had abnormal results, with 74 percent of those indicating they would follow-up for additional care. What s Next: In 2016, Deborah will continue to maintain a vibrant community screening outreach and educational component, documenting the outcomes and impact of those screening events. Deborah also will continue to work towards our CHNA goal of identifying a suitable location for the placement of a second automated blood pressure machine in our primary service area. Deborah s blood pressure monitoring machine at the Ocean County Mall provided 42,120 tests in 2014 and 38,500 tests in Our goal in 2016 is to find a suitable location for a second machine in Burlington County that will yield similar results Use of Blood Pressure Machine Ocean County Mall 42, , CHNA Priority Area 3: Expanding Access to Recently Implemented Programs CHIP Goal: Our CHNA identified Deborah s Diabetes Center as a focal point for our efforts to expand the community s access to our recently implemented programs and targeted continued growth of the services rendered as a key goal. CHIP Action: The Diabetes Center s mission is to improve the quality of life for persons affected by diabetes or pre-diabetes. This is accomplished by providing current diabetes resources to people affected by diabetes, significant individuals in their lives and the healthcare professionals serving them. The Center - 3-
6 offers the services of an Endocrinologist, a Registered Nurse Certified Diabetes Educator (RN-CDE) and a Registered Dietician (RD). Services include medical management by a physician, individual diabetes self-management training provided by an RN-CDE and medical nutrition therapy provide by an RD. Comprehensive class instructions are also provided on a weekly basis by an RN-CDE and an RD. The Center s patient population consists of people eighteen years and older who have diabetes or pre-diabetes symptoms. We encourage patients (and their family members) to participate actively in their own care. Results: In both 2014 and 2015, the Diabetes Center met its goal of expanded growth. In 2014, the Center provided 24 classes, 165 medical nutrition therapy visits, 462 diabetes self-management training visits and 1,716 office visits. In 2015, the Center provided 6 classes, 142 medical nutrition therapy visits, 354 diabetes self-management training visits and 2,771 office visits. What s Next: In 2016, Deborah will continue to grow education and diabetes self-management services at the Diabetes Center. Deborah will also continue to work towards our CHNA goal of opening a second diabetes services location in our secondary service area. CHNA Priority Area 4: Establishing New Programs to Meet Identified Health Needs CHIP Goal: Establish a number of new programs to meet the needs of the residents in our service areas. CHIP Actions: Pulmonary Oncology Program Deborah recently launched a new Pulmonary Oncology Program, bringing an oncologist to the Center to provide Deborah pulmonary patients a continuum of care. The oncologist works closely with Deborah pulmonologists and thoracic surgeons to ensure patients are educated about cancer diagnosis and treatment. This new initiative, along with increased cancer screenings, is an important step in addressing the need identified in our CHNA. - 4-
7 Results: In 2014, the Multi-Disciplinary Oncology Clinic saw 312 patients: 47 patients underwent thoracic procedures, 77 received EBUS evaluation and 158 had PET scans. There were 280 physician-only patient visits in the clinic. In 2015, the clinic saw 225 patients: 46 underwent thoracic procedures, 53 received EBUS evaluation and 116 had PET scans. New Lung Screening Program: Deborah also recently initiated a new program sponsored by The Deborah Foundation. The program provides low-dose CT scans for early detection of lung cancer to identify and treat early stages of lung cancer. In 2014, the program screened 84 people. There were 44 negative results with a recommendation for follow-up in one year. There were 40 positive results, resulting in 3 referrals to Deborah, 5 referrals to the Multi-Disciplinary Oncology Clinic, 6 PET scans, 24 complex CT scans and 1 surgery. In 2015, there were 67 people screened with 26 positive results. World Asthma Day Event: On World Asthma Day in 2015, in conjunction with the American Lung Association, Deborah offered an onsite event for 100 community members to raise public awareness of asthma and lung cancer. Deborah offered pulmonary function testing, pulse oximetry testing to measure the oxygen level of blood and alpha one testing to screen for a genetic condition that can result in serious lung disease in adults. Asthma and lung cancer education were also provided. The Power of Breath Event: Also in 2015, Deborah held The Power of Breath event to raise awareness of the dangers of tobacco use and the link to lung cancer. Forty-four pulmonary function tests were performed. The event was held in conjunction with the Burlington County Health Department, the American Lung Association, Tobacco-Free New Jersey South, Prevention Plus and the Burlington County Regional Community Development Council. What s Next: Deborah will continue to grow the pulmonary oncology program and increase community awareness of the services offered. Deborah will also continue to work to partner with other organizations to raise awareness of the dangers of tobacco use and to expand access to smoking cessation programs. CHNA Priority Area 5: Pursuing Partnerships to Meet Identified Health Needs CHNA Goal: In accordance with our CHNA, Deborah continues to work to build its relationship with the New Jersey Institute for Successful Aging (NJISA) of the Rowan University School of Osteopathic Medicine. The NJISA is a nationally recognized Center of Excellence that since 1989 has served older individuals and their families, providing medical care in varied settings and offering training for health professionals of multiple disciplines. NJISA s leadership in education, research, clinical care, community service and public policy fosters our commitment to helping the growing number of older individuals in New Jersey age successfully. The increased availability of NJISA programs, especially in Deborah s immediate service area and in Ocean County, would help clarify and address some of the health problems associated with aging identified in the CHNA. CHIP Actions: As previously noted in this document, Deborah is continuing to seek partnerships to offer an automated blood pressure monitors at a second location within our primary service area. - 5-
8 Deborah has established a number of successful health partnerships which include: Partnership with Burlington County College to coordinate onsite campus flu clinics Partnership with Burlington County Sheriff s Department to provide NARCAN to county officers to address an identified need related to substance abuse in the community Partnership with Pemberton Township for enhancing community outreach Partnership with the American Lung Association for several outreach events to raise awareness of smoking dangers and to offer resources to help smokers stop smoking Partnership with Ocean County Health Department for enhanced participation in their Chronic Disease Management Programs and ongoing work on the county s CHIP Partnership with Burlington County Health Department for enhanced participation with their ongoing health outreach efforts What s Next: In 2016, Deborah will continue to build on these successful community partnerships and work to establish new partnerships to meet the needs of the community. - 6-
