1 Regional Clinical Co-Lead (Physician) Role Opportunity The South West Hospice Palliative Care Network (SWHPCN) in partnership with the South West LHIN and the South West Regional Cancer Program are seeking a Regional Clinical Co- Lead, to join the SWHPCN leadership team. The Regional Clinical Co-Lead will report to the CCO RVP and the LHIN CEO (through the Chief Clinical Lead). The Regional Clinical Co-Lead will collaborate with local clinicians and other partners to improve hospice palliative care across all patient populations, illness trajectories and health care settings. Together, with the South West Hospice Palliative Care Network (SWHPCN) Lead, they will be responsible for the delivery of regional work plans and completion of annual objectives. The Clinical Co-Lead will fulfill a critical role in improving hospice palliative care across all patient populations, illness trajectories and health care settings. The Clinical Co-Lead will provide strong, visionary clinical leadership at the regional level as a recognized leader in the region in hospice palliative care, demonstrating in-depth and current knowledge of clinical and system level issues. The Clinical Co-Lead will participate as an active member of the SWHPCN to facilitate regional planning, palliative care network development, performance measurement, and quality process improvement. The SWHPCN is looking for a strong professional with a proven track record in leadership that is passionate about evidence based care and quality improvement. Applicants must have experience and expertise in the area of palliative care and be members in good standing of the College of Physicians and Surgeons as defined by the Regulated Health Professionals Act. 2 Regional Physician Co-Lead Role Statement Background In March 2016, the Ontario Palliative Care Network (OPCN) was officially launched. The OPCN is a disease-agnostic network developed to serve the needs of all Ontarians. The OPCN is an organized partnership of community stakeholders, health service providers and health systems planners who together are developing a coordinated, standardized approach for delivering hospice palliative care services in the province. The OPCN introduced a new governance structure for hospice palliative care, one that is accountable to the Ministry of Health and Long- Term Care (MOHLTC), under the leadership of four provincial partners: the Local Health Integration Networks (LHINs), Cancer Care Ontario (CCO), Health Quality Ontario (HQO) and the Quality Hospice Palliative Care Coalition of Ontario (the Coalition). The OPCN will build on the great work happening at the local level and align with best evidence to leverage what already exists. It will support providers and facilities to adapt a provincial, standardized approach to their local reality and environment. The Declaration of Partnership will continue to be the roadmap for advancing high-quality, high-value hospice palliative care in Ontario under the OPCN. Parliamentary Assistant John Fraser s recent Palliative and End-of-Life Care Provincial Report confirmed the Declaration is a priority. The role of the OPCN is to ensure implementation of the Declaration of Partnership priorities at both the provincial and regional levels. The OPCN has been working to evolve 14 Regional Palliative Care Networks (RPCNs) through broad consultation with providers and stakeholders. The OPCN s collaboration with the RPCNs is critical to the success of advancing high quality, high value, and patient-centered hospice palliative care. In the South West, the South West Hospice Palliative Care Network has been operational for a number of years, and has been given the mandate of evolving the work of the OPCN at the regional level. The South West Hospice Palliative Care Network (SWHPCN) is an alliance of community agencies, hospitals, long-term care homes, and other stakeholders who are all committed to continuously improving hospice palliative care services and delivering on the vision for integrated hospice palliative care. Within the SWHPCN, there are five collaborative tables, each
3 related to a sub-lhin area. These areas are; Elgin, London-Middlesex, Grey Bruce, Huron Perth and Oxford. Along with the overall Network mandate, these regional collaborative tables are also focused on change ideas aimed at improving hospice palliative care needs specific to their community. The OPCN, including the SWHPCN, will embrace the following principles outlined in the Declaration: Improve access to hospice palliative care in the community Provide more support for primary care providers Improve hospice palliative care first for those who are in their last year of life, including those with advanced chronic disease Work to raise awareness of hospice palliative care in general Improve quality of care by optimizing the best of what is currently available while building for the future Regional Clinical Co-Lead Roles To ensure appropriate leadership in this important work, the SWHPCN is seeking a Palliative Physician Co-Lead to join the HPC leadership team. This position represents a 0.2 FTE commitment of time and is a key clinical leadership partner in moving forward with the implementation of the Declaration recommendations in the South West. The Regional Palliative Care Physician Co-Lead will report to the CCO RVP and the LHIN CEO (through the Chief Clinical Lead). Together, with the SWHPCN Executive Team and local and regional care providers, the Palliative Physician Co-Lead will play a key role in achieving regional deliverables to work with local partners to improve hospice palliative care across all patient populations, illness trajectories and health care settings. Key to success in this role will be collaboration with and amongst clinical members of both the South West LHIN and the South West Regional Cancer Program Leadership structures. Status: The Regional Clinical Co-Lead must be Physician in good standing with the College of College of Physicians and Surgeons of Ontario and have experience in primary care health delivery as well as palliative care. The Co-Lead must be able to articulate the role of primary care in supporting patients and families during their palliative care experience and identify opportunities to continue to improve care for people in their last year of life.
4 Roles and Responsibilities Work in close partnership with their Regional Clinical Co-Lead counterpart and the SWHPCN Director to achieve regional deliverables defined or outlined in the regional work plan Work collaboratively with the other Regional Clinical Co-Leads, and the Provincial Clinical Co-Leads to align priorities, accountabilities and deliverables at both the provincial and regional levels and ensure consistency among regions Provide strong, visionary clinical leadership at the regional level as a recognized leader in the region in hospice palliative care, demonstrating in-depth and current knowledge of clinical and system level issues Participate as an active member of the SWHPCN to facilitate regional planning, palliative care network development, performance measurement, and quality process improvement Participate as an active member of the Physician Leadership teams at both the South West LHIN (under the leadership of the Chief Clinical Lead) and at the South West Regional Cancer Program (under the leadership of the RVP, CCO). Promote and strengthen collaboration within the region through the SWHPCN, the LHIN, the South West Regional Cancer Program and at provincial committees or councils Work closely with regional partners to develop yearly hospice palliative care education plans Work closely with the regional hospice palliative care resources (regional primary care leads, nurses, Palliative Pain and Symptom Management Consultants, and others) to ensure a standardized approach is being employed regionally with regards to hospice palliative care education and implementation of OPCN Clinical Advisory Council recommendations for clinical improvements in the regions Facilitate the implementation of the South West quality initiatives (e.g. guidelines, and standards,) in alignment with the OPCN direction. The Co-lead will be a member of the SW LHIN Clinical Quality Table. Build health service provider skills to increase primary level hospice palliative care at the local level through partnerships with Physician Leads,, local providers and others Facilitate knowledge transfer of provincial hospice palliative care initiatives and strategies to stakeholders throughout the region and act as a voice of the South West at provincial tables Participate in provincial and regional committees as required
5 Accountability/Reporting Completion of an annual regional work plan the annual regional work plan will be approved and supported by the SWHPCN Leadership Committee. Report to the CCO RVP and the LHIN CEO (through the Chief Clinical Lead) on a quarterly basis against annual deliverables Engage with regional and provincial leadership as required Engage with the OPCN Provincial Clinical Co-Leads to ensure alignment on shared priorities and deliverables Qualifications Family Practice Physician in good standing with the College of Physicians and Surgeons of Ontario Clinical experience in providing palliative care. Proven track record of clinical leadership at a hospital and/or university level Knowledgeable of clinical, administrative and system-wide issues with respect to palliative care services and an aptitude for systems thinking Knowledge and understanding of the health system in the South West region Excellent interpersonal skills and able to build effective relationships with clinical and administrative stakeholders Actively seeks opportunities and challenges for personal learning, character building, and growth. Foster and supports and challenges others to achieve professional and personal goals. Set Direction: Inspires vision by identifying, establishing, and communicating clear and meaningful expectations and outcomes. Take Action to Implement Decisions: Acts in a manner consistent with the organizational values to yield effective, efficient public-centered service. Access and Evaluate: Measures and evaluates outcomes. Holds themselves and others accountable for results achieved against benchmarks and correct the course as appropriate.
