2 Elective Cardioversion - Information for Patients Elective cardioversion is a procedure for terminating abnormal heart rhythms such as atrial fibrillation and atrial flutter, and may be an alternative to long-term drug therapy. The aim is to convert the heart to a normal heart rhythm (sinus rhythm) by delivering a controlled electrical impulse to the heart. Under a general anaesthetic, the shock is given to the heart via two paddles placed on the chest wall. As you are asleep you will not feel anything. Before the procedure A few days before your cardioversion you will attend hospital, where you will be assessed and have an ECG recorded. The procedure will be explained to you and its implications discussed. It will be checked whether you have been taking your medication regularly and you will have a blood test. This may require repeating the day before the procedure. On the day of the procedure Please attend the Millbrook Unit at Macclesfield District General Hospital at.. am. If you are unable to attend for any reason please ring the cardiology nurse on and leave a message You will need to arrange for someone to bring you who can drive you home after the procedure and you will need someone to stay with you overnight. Do not bring any valuables with you. At the appointed time report to the Millbrook Unit which is easily accessed through the Fieldbank Road entrance. The Treatment Suite is situated through the double doors just inside the main entrance to the Millbrook Unit. Sit down on the seats situated to the left and someone will attend you as soon as possible. YOU MUST HAVE NOTHING TO EAT OR DRINK FOR 6 HOURS PRIOR TO THE PROCEDURE. Take all your normal tablets except Digoxin (if you are prescribed it) with a small sip of water unless told differently. A cannula will be inserted into your hand or arm to administer the anaesthetic drugs and you will be given some oxygen to breathe in. Once fully anaesthetised you will receive a synchronised shock to your heart but you will not feel anything. You should be asleep only for about 5 minutes. 3 Occasionally you may get a red mark where the paddles have been. Cream will be applied if necessary. Once you are fully awake you can have a drink and a light snack and will be able to go home after a couple of hours. You will be told whether the cardioversion was successful or not on the same day. Out-patient appointments and any medication changes will be discussed with you before you go home. Following discharge The drugs we give you for a general anaesthetic may make you clumsy, slow and forgetful for about hours. We strongly advise in this time that you: Do not drive or operate machinery during this time, including electric kettles and cookers Are not left alone but have a responsible adult to stay Do not take a hot bath or shower unattended Do not drink alcohol Do not sign any important documents or make any major decisions We would advise you to: Rest at home Eat light meals Drink fluids regularly Contact CCU on the number opposite if you have any worries concerning the procedure Contact your GP at any time if you feel unwell Observe any changes to your medication that have been advised Potential risks of cardioversion The most serious risk is of triggering a stroke, which occurs in less than 1% of patients. By checking that your level of anticoagulation has been adequate and stable for a period of at least four weeks this risk will be minimised. Serious complications from anaesthesia are very rare but overall risks depend on your general health. Any increased risk due to your personal health and circumstances will be discussed with you at you pre-cardioversion assessment. There is about a 95% chance that this procedure will be successful initially and will return your heart rhythm to normal, but you might revert to your original abnormal rhythm in the days or weeks after the procedure. Six weeks after the procedure your ECG will be checked and it is expected that about 50% of patients will still be in a normal rhythm. Success is difficult to predict on an individual basis. 4 Important points to remember Ensure you attend for you pre cardioversion check and blood test or the procedure can not be performed Report to the Millbrook Unit at the appointed time Have nothing to eat or drink for 6 hours prior to the procedure Do not drive or operate machinery for 24 hours until the effects of the anaesthetic have completely worn off If you have any queries or are unable to attend please ring the Cardiac Nurses on Leaflet Ref: Published: 09/15 Review: 09/18 Page 4
