3 Session Rationale o This session includes the general TCM diagnostic procedures, diagnostic differentiation and treatments for sleep disorder, including insomnia and somnolence. o There will be 1-hour practical tutorial for students to develop TCM assessment, diagnosis and treatment skills in regard to sleep disorder. o The aim of this session is for students to develop theoretical knowledge, critical analysis and a clinical perspective in the TCM assessment, differentiation and treatment strategy for sleep disorder. 5 Bio-med perspective: What is a sleep disorder? sleep-disorders A sleep disorder is a condition that prevents you from getting restful sleep and can cause daytime sleepiness and problems in functioning. Signs you may have a sleep disorder include persistent difficulty going to sleep or staying sleeping, irregular breathing or movement during sleep, and feeling sleepy during the day. 6 Types of sleep disorders o There are many different types of sleep disorders. They're often grouped into categories that explain why they happen or how they affect you, or be grouped according to behaviors, problems with your natural sleep-wake cycles, breathing problems, difficulty sleeping or how sleepy you feel during the day. o Some common types of sleep disorders include: Insomnia, Sleep apnea, Restless legs syndrome (RLS), Narcolepsy Endeavour College of Natural Health 6

11 The Functions of the Heart o Governs Blood/Xue & Controls the blood vessels (two ways governing the Blood in TCM): 1. Transformation of Gu-Qi into Xue takes place in the Heart 2. Heart is responsible for the circulation of Blood (just the same way as Western Medicine). o Manifests in the complexion o Houses the (Mind)Shen o Relates to Joy o Opens into the tongue o Controls sweat The Heart s main function is the govern Blood and the blood vessels and to house the Mind (Shen). 14 Heart & Shen (Mind) Shen & sleep o Simple Questions chapter 8 states: The Heart is the Monarch and it governs the Mind. o The Spiritual Axis chapter 71 says: The Heart is the Monarch of the 5 Yin organs and 6 Yang organs and it is the residence of the Mind. Explanation: Heart housing the mind depends on adequate nourishment of Xue and conversely, the Heart s role of governing blood depends on the mind. Heart Xue roots the mind, embraces it and makes it peaceful & happy. Mental restlessness, depression, anxiety and insomnia can deplete the Heart Xue further leading to palpitations, pale complexion, and weak & irregular pulse. Endeavour College of Natural Health 14
15 Heart and Joy o Heart relates to Joy A normal state of Joy is not a cause of disease, but a beneficial mental state that favours a smooth functioning of the Internal Organs and their mental faculties. Joy as excessive excitement and mental stimulation can injure the Heart o Joy as an inordinate craving stirs up the Minister Fire, which ascends and over stimulates the mind. o Excessive joy leads to over stimulation of the Heart. o Sudden joy causes Yang-Qi to float, the blood vessels and the Heart dilate and the pulse becomes slow, slightly overflowing and Empty Endeavour College of Natural Health 15
16 Other Heart related Relationships o Heat o Sweat o Dreams. If the Heart is weak and the mind has no residence it will float with the inability to fall asleep, or disturbed sleep or excessive dreaming. When the Heart is weak one dreams of fires, smoke and mountains, when the Heart is in excess one dreams of laughing. 27 Sleep: A TCM perspective o TCM views the human sleep-wake cycle is A dynamic yet integral physiological state, where almost all sleep disorders stem from either a deficiency of Qi or a surplus of Qi in the body organs (mainly kidney and heart). o TCM explains the sleep process using unique theory of Yin and Yang, Qi circulation, and spirit activity: The Yin-Yang natural Rhythm: Sleep help recharge the body to maintain balance between Yin and Yang in the body. The human body is programmed to live in harmony with the rhythm of nature: When Yang-qi is at its limit and Yin-qi is abundant, one s eyes are closed. When Yin-qi is at its limit and Yang-qi is abundant, one is awake. we naturally go to bed at the Yin predominate time (night) while wake up at the Yang predominate time (day) Endeavour College of Natural Health 27

The Internal Energy Cycle o Energy levels and mood fluctuate during the day, this is due to the body clock that is reflected in the meridians and linked to the organs cycle. o TCM 24 hour body clock is divided into 12 two-hour intervals of the Qi (vital force) moving through the organ-meridian system. 29 Sleep: A TCM perspective (cont.) The Shen (spirit) activity Shen in TCM refers to mind, thought, state of consciousness and mental functioning that keep the mind sharp and alert. Shen is the highest authority of the physical body that orders it to rest or work. In coordination with natural rhythm, Shen hides in the organs at night for recharging, and comes out during daytime to exert its duties. That is why sleeping problems occur when Shen is disturbed. 31 Notes o Sleep disorder mainly include: Insomnia : inability to experience normal sleep Somnolence : is characterized by constant sleepiness, regardless of the time of day or night. o From TCM viewpoints, Sleep disorder is not only due to Heart pathologies. o Almost all types of sleep disorder stem from either a deficiency of Qi or a surplus of Qi in the body organs. 33 TCM insomnia o Define: inability to experience normal sleep. Insomnia means insufficient duration/amount of sleep or sleep of poor quality. In mild cases, it may be difficulty in falling asleep or frequent waking from sleep. In severe cases, it may be inability to fall asleep all night. Headaches, dizziness, palpitation and forgetfulness commonly accompany insomnia. Discussion in class (it s better in practical section): How would you ask your patient presenting with sleep problem appropriate questions in the clinic consultation? 34 TCM Etiology and Pathogenesis o Insomnia is mostly induced by pathogenic factors agitating the Heart, or by malnourishment of the mind. 37 Differentiation of deficiency/strength type insomnia: o The deficiency type (Mainly)Qi and blood deficiency dysfunction of the heart due to malnourishment of Blood and or Yin (Yin deficiency with strong Fire. (This type is commonly associated with restlessness.) o The strength type (mainly) due to Phlegm-Heat causing upward disturbance, and typically shows agitation, irascibility, a bitter taste in the mouth and a dry throat, constipation and dark urine. 38 Key s & s of Insomnia in Common TCM Patterns o Deficiency of the heart & spleen: be able to fall asleep but readily awakens and then unable to fall asleep again o Flaming Fire due to Yin deficiency: difficulty of falling asleep with restlessness, palpitation, aphthous sores in the mouth and mouth dryness in the night. o Insufficiency of the heart and the gallbladder/blood insufficiency and liver hyperactivity: patient falls asleep readily but wakes up with fright and is generally fearful and irascible, with frequent sighing o Dysfunction of the heart: insomnia accompanied with frequent dreams, dizziness, headache and forgetfulness Endeavour College of Natural Health 38
39 Insomnia Acupuncture treatment o Treatment with acupuncture and herbs usually produces a reliable result on most types of insomnia. o Adjunct therapy is useful in most types of insomnia, particularly relaxation or meditation to calm the mind. o Acupuncture Treatment: In general, acu-treatment should be applied 2hs before sleep. 54 Case 1: Insomnia Case Studies A 36-year old male presented with insomnia for several years. He is sometimes unable to sleep at all through the entire night. He frequently had recurrent aphthous sores in the mouth and on the tongue. In addition, he had dizziness and headaches. At the time of presentation he had much thirst with desire to drink large amounts. His tongue was red, with scant coating. His pulse was threadlike and rapid. Endeavour College of Natural Health 54
55 Case study 2 31 years old female presents for assist sleep and stress (onset: <2 years). She reports very difficult to fall asleep ( took ~2 to 3 hours each night to try to fall asleep ). Further questioning also determines that the patient suffers from palpitation, dry mouth and constipation, poor appetite and occasional stomach ache following meals. She has recently had her eye worsen and recently purchased new glasses.
