1 Thank you for participating in the WISE program over the last six weeks. Please complete the following questionnaire. Keep in mind there are no wrong answers. We are simply looking for your thoughts and impressions. Your honesty and candidness will help us better understand how our program impacts seniors and will help us to improve our program and serve even more seniors in the future.
