Boala fi catului gras non-alcoolic (NAFLD), cea mai frecventă cauză a bolii hepatice cronice la copii, cuprinde un spectru de afecţiuni variind de la steatoză simplă până la steatohepatită (NASH). Acidul ursodeoxicolic (UDCA) a fost utilizat la pacienţi adulţi cu NASH, demonstrându-şi efi cacitatea în doze crescute. Scop. De a evalua efectul tratamentului cu doze crescute de UDCA asupra parametrilor funcţiei hepatice la copii cu NASH. Metode. Lotul de studiu a fost constituit din 18 copii cu NASH cu vârste cuprinse între 2,5 şi 17 ani. NASH a fost operaţional defi nit ca fi catul difuz hiperechogenic la ecografi e cu valori persistent crescute (> 35 UI/l) ale alanin aminotransferazei (ALAT) sau aspartat aminotransferazei (AST), după excluderea cauzelor virale, drog sau alcool induse, colestatice sau genetice ale bolii hepatice. Pacienţii au fost trataţi cu acid ursodeoxicolic (20 mg/kg/zi), timp de şase luni. Rezultate. Tratamentul cu UDCA a determinat scăderea semnifi cativă a valorilor transaminazelor hepatice. Nivelurile ALT au variat între 68,5 (50-166) iniţial vs 47 (29-104) după şase luni, p < 0,001, AST între 43 (29-101) vs 34 (17-75), p < 0,001, şi GGT între 41,5 (11-150) vs. 22 (8-55). La sfârşitul tratamentului, niveluri ALT s-au normalizat la 4 dintre pacienţi. De asemenea, concentraţiile de colesterol au fost semnifi cativ reduse după tratament. Concluzii. Tratamentul cu doze crescute de acid ursodeoxicolic imbunătăţeşte parametrii funcţiei hepatice la copii cu NASH. Cuvinte cheie: steatoză hepatică, acid ursodeoxicolic, transaminaze hepatice INTRODUCERE În ultimele două decade, boala ficatului gras non-alcoolic (NAFLD) a înregistrat creşteri epidemice ale prevalenţei şi devine cea mai comună cauză de boală hepatică cronică la copil (1). NAFLD cuprinde sub aspect histologic un spectru care variază de la steatoza simplă [ficatul gras non- alcoolic (NAFL)] la steatoză ce asociază inflamaţie cu leziuni celulare [steatohepatita non-alcoolică (NASH)], iar evoluţia poate implica fibroză (2). Unele cazuri pot progresa spre ciroză cu hipertensiune portală şi chiar hepatocarcinom la vârsta adultă. Obezitatea, dislipidemiile şi insulino-rezistenţa sunt cei mai comuni factori de risc asociaţi cu NAFLD la copil. Cunoscându-se riscurile caracterului potenţial progresiv al steatohepatitei, multe cercetări s-au focusat pe terapia acestei afecţiuni. Pierderea în greutate, exerciţiul fizic, controlul diabetului şi al dislipidemiilor constituie principalele direcţii de tratament la copil (3). Asociate acestor măsuri de modificare a stilului de vieţă, o serie de agenţi farmacologici cum ar fi metforminul, vitamina E, acizii graşi omega 3, acidul ursodeoxicolic s-au dovedit a avea efecte benefice la pacienţii cu NAFLD şi NASH (4). Acidul ursodeoxicolic (UDCA), prin efectele sale hepatoprotective şi antiapoptotice, poate avea un efect benefic în evoluţia şi progresia steatohepatitei. Trialurile clinice la adulţi au arătat că doza standard de acid ursodeoxicolic (13-15 mg/ kg) este ineficientă în tratamentul NASH, dar doze Adresa de corespondenţă: Drd. Otilia E. 2 75 crescute (28-32 mg/kg pe zi) pot avea rezultate favorabile asupra nivelurilor transaminazelor (5). Acţiunea acidului ursodeoxicolic este doză-răspuns dependentă, iar doze crescute de 20 mg/kg pe zi au avut efecte benefice asupra modificărilor valorilor biochimice şi fibrozei hepatice la copii cu colangită sclerozantă primară sau colestază din fibroza chistică (6,7). Obiectivul studiului a fost evaluarea efectelor dozelor relativ crescute (20 mg/kg/zi) de acid ursodeoxicolic asupra parametrilor funcţiei hepatice şi ecografici la copii cu steatoză hepatică non-alcoolică. MATERIAL ŞI METODĂ Lotul de studiu a inclus un număr 18 copii, cu vârste cuprinse între 2,5 şi 17 ani, cu media de vârstă de 9,39 ± 4,13, selectaţi dintre pacienţii cu diagnosticul de steatoză hepatică non-alcoolică, monitorizaţi în Clinica II Pediatrie a Spitalul Clinic de Urgenţă pentru Copii Sf. Maria Iaşi în perioada ianuarie 2010 decembrie Pacienţii au fost supravegheaţi între 8 luni şi 2 ani. NAFLD a fost operaţional definit ca ficatul difuz hiperechogenic la ecografie cu valori persistent crescute (> 35 UI/L) ale alanin transferazei (ALT) sau aspartat transferazei (AST), după excluderea cauzelor virale, drog sau alcool induse, colestatice şi genetice ale bolii hepatice. Criteriile de includere le-au constituit valori ale alanin aminotransferazei 50UI/L şi ecografia semnificativă. Pacienţilor li s-a administrat acid ursodeoxicolic, 20 mg/kg/zi, în două doze divizate la micul dejun şi seara, timp de şase luni. Toţi pacienţii au fost consiliaţi în ceea ce priveşte modificarea dietei prin consumul unei alimentaţii sănătoase şi efectuarea de exerciţii fizice regulate la evaluarea iniţială, dar nu a fost instituită nici o măsură dietetică spe cifică. Monitorizarea în dinamică s-a efectuat prin evaluări ale funcţiei hepatice, glicemiei, profilului lipidic şi aspectului ecografic la intervale de 3 luni. Parametrii sindromului metabolic au fot apreciaţi după criteriile IDF, 2007 (8). Pentru evaluarea gradului de fibroză iniţială s-a efectuat Fibroscan, valorile de referinţă fiind considerate cele de la adult (9). S-a cuantificat modificarea valorilor transamina zelor hepatice la finalul perioadei de tratament. Analiza statistică s-a realizat cu ajutorul programului SPSS 19. Semnificaţia statistică s-a stabilit pentru o valoare p < 0,05. REZULTATE Caracteristicile clinice şi biologice ale pacienţilor la începutul perioadei de urmărire sunt redate în Tabelul 1. Nivelurile enzimelor hepatice au fost cele specifice pacienţilor cu steatohepatită non-alcoolică, cu aminotransaminase moderat crescute şi un raport AST/ALT mai mic de 1. TABELUL 1. GGT a înregistrat valori peste 1,5 x VN la 33,3% dintre pacienţi. La 38,8% dintre pacienţi, valorile glicemiei à jeun au fost peste 100 mg/dl, iar la 11 pacienţi, gli cemia a fost în limite normale. Doi dintre pacienţi s-au încadrat în parametrii sindromului metabolic, prezentând circumferinţa abdominală mai mare de percentila de 90, glicemia à jeun 100 mg/dl, trigliceride 150 mg/dl şi tensiune arterială sistolică peste 130 mmhg. La 61,1% dintre pacienţi s-a regăsit cel puţin o componentă a sindromului metabolic. Evaluarea fibrozei prin Fibroscan a relevat prezenţa fibrozei F2 la 2 dintre pacienţi, stadiu F1 la 6 pacienţi şi F0 la 10 pacienţi. A existat o corelaţie semnificativă statistic între gradul fibrozei şi nivelul ALT (p < 0,05) şi AST (p < 0,01). De-a lungul perioadei de urmărire, nu au fost raportate modificări semnificative ale schimbării sti lului de viaţă. Pe timpul celor 6 luni, greutatea corporală a pacienţilor a rămas relativ constantă. IMC a variat nesemnificativ de la momentul începerii urmăririi la 6 luni de tratament, 23,02 ± 6,32 kg/m 2 vs. 22,71 ± 5,78 kg/m 2 (p = 0,12). La sfârşitul perioadei de administrare a UDCA s-a observat o reducere a valorilor transaminazelor he patice. Mediana (intervalul) valorilor absolute

3 76 ALT s-a ameliorat semnificativ de la valoarea iniţială 68,5 (50-166) la 47 (29-104), p < 0,001, la 6 luni. Media reducerii valorilor ALT a fost de 27,22 ± 18,56. Nivelurile ALT s-au normalizat la 4 dintre pacienţi la finalul perioadei de tratament. Reducerea aspartat aminotransferazei a urmat un trend similar reducerii ALT, dar într-un grad mai redus. Mediana (intervalul) valorilor AST a variat semnificativ de la 43 (29-101) la 34 (17-75), p < 0,001. Media reducerii valorilor AST a fost de 15,05 ± 11,96. Tratamentul cu acid ursodeoxicolic s-a asociat, de asemenea, cu o scădere semnificativă (p < 0,05) a nivelurilor GGT, mediana variind între 41,5 (11-150) vs. 22 (8-55). Modificări statistic semnificative (p < 0,01) au înregistrat şi valorile colesterolului, mediana variind între 167,5 ( ) vs 154,5 ( ). Modificările colesterolului nu s-au corelat cu modificările IMC, sugerând că efectele metabolice asociate ad ministrării de UDCA nu pot fi atribuite modificării greutăţii corporale pe perioada tratamentului. Nu s-au înregistrat modificări semnificative ale valorilor bilirubinei totale, fosfatazei alcaline, glice miei sau trigliceridelor. Aspectul ecografic nu a variat la finalul celor 6 luni de administrare a UDCA. DISCUŢII Argumentele epidemiologice, care relevă o incidenţă în creştere a NAFLD, de cele mai multe ori în relaţie cu prevalenţa fără precedent a obezităţii, au impus o nevoie imperioasă pentru implementarea unor terapii specifice şi sigure. Acidul ursodeoxicolic este un acid biliar natural, hidrofilic, care constituie apoximativ 3% din conţinutul bilei umane, cu numeroase proprietăţi hepatoprotectoare şi imunomodulatoare. UDCA stabilizează membranele mitocondriale şi reduce eli be - rarea de enzime hidrolitice din hepatocitele de te - riorate. 4 77 reactive de oxigen. În acest context, UDCA ar putea ameliora disfuncţia şi leziunea celulară hepatică determinată de stresul oxidativ în NASH (10). Efectele de modulare a căilor de semnalizare antiapoptotice ale UDCA ar putea constitui un efect benefic în steatohepatita non-alcoolică (11), în care apoptoza celulară s-a dovedit a fi crescută. În plus, studiile au arătat că TNF-α ar avea o contribuţie semnificativă la dezvoltarea insulino-rezistenţei şi nivelurile serice ar fi crescute la pacienţii cu NASH. Cel puţin la pacienţii cu colestază cronică, UDCA a determinat scăderea nivelurilor de TNF-α, asociat cu îmbunătăţirea parametrilor biologici şi histologici (12). Administrarea de acid ursodeoxicolic a condus în studiul nostru la scăderea semnificativă a nivelurilor transaminazelor la pacienţii cu NAFLD, iar la 22,2% dintre pacienţi s-a produs o normalizare a nivelurilor ALT. Neavând confirmarea bioptică a steatohepatitei, am considerat ca marker al injuriei hepatice valorile mai mari de 1.5xVN ale ALT. În acest context putem presupune că reducerea nivelurilor ALT ar putea reflecta o îmbunătăţire a inflamaţiei hepatice. Nivelurile ALT sunt considerate ca parametru reprezentativ pentru evaluarea îm bunătăţirii necroinflamaţiei hepatice, având valoare predictivă asupra modificărilor histologice. Corelaţia cu evoluţia IMC arată că scăderea în greutate nu poate fi considerată cauza implicată în scăderea transaminazelor. De asemenea, la pacienţii urmăriţi s-a înregistrat o scădere semnificativă a nivelului colesterolului. Dislipidemia fiind recunoscută ca factor de risc în apariţia steatozei, scăderea valorilor colesterolului ar putea ameliora încărcarea grasă a ficatului în evoluţie. Datele din literatură privind eficacitatea UDCA în NASH sunt variate şi discordante. În 1996, Laurin et al. a evidenţiat, pe un lot de 24 de adulţi cu NASH, că UDCA în doze de mg/kg/zi a determinat o scădere semnificativă a nivelurilor alanin aminotransferarzei (-30%), GGT (-45%), şi, de asemenea, scăderea gradului histologic al steatozei (13). În urma unui studiu randomizat efectuat la adulţi, Lindor et al., 2004, au arătat că tratamentul cu UDCA (13-15 mg/kg) versus placebo pentru 2 ani a determinat aceleaşi scăderii ale niveluri ALT în ambele grupuri. Din punct de vedere histologic s-a înregistrat doar o ameliorare a steatozei, similară în ambele grupuri (14). Studiul condus de Dufour et al., 2006, a arătat efecte semnificative ale asocierii UDCA cu vitamina E, dar nu şi ale UDCA versus placebo. Rezultatele negative au ridicat întrebarea dacă administrarea unor dozele crescute ar avea beneficii în NASH (15). Ratziu et al., 2011, într-un studiu randomizat la adulţi, a arătat că adminstrarea de doze crescute de UDCA (28-35 mg/kg/zi) a determinat scăderi semnificative ale nivelurilor transaminazelor, glicemiei şi insulinemiei la un an după tratament. Studiul nu a avut şi end-point histologic, dar s-a realizat o evaluare a markerilor non-invazivi ai inflamaţiei şi fibrozei prin Fibrotest, înregistrându-se, de asemenea, o scădere semnificativă (5). Un alt studiu realizat la adulţi, cu punct de urmărire histologic, a arătat că administrarea de mg/ kg/zi de UDCA a adus îmbunătăţiri ale nivelurilor transaminazelor, dar acestea au fost nesemnificativ statistic în comparaţie cu valorile grupului de control. Studiul nu a relevat nici îmbunătăţiri ale aspectul histologic după 18 luni de tratament (16). Dozele convenţionale de UDCA folosite pe un lot de copii obezi nu au arătat îmbunătăţiri asupra nivelurilor transaminazelor şi nici asupra aspectului ecografic al ficatului (17). Pe lotul nostru de copii, ad ministrarea unor doze mai mari de acid ursodeoxicolic a determinat scăderi ale nivelului transaminazelor hepatice, chiar dacă nu s-au produs normalizări ale alaninaminotransferazei decât la 22,2% dintre pacienţi. Nu au existat efecte adverse grave; s-au înregistrat doar discrete dureri abdominale sau accentuarea tranzitului intestinal, care nu au impus întreru perea terapiei. Din cauza intervalului relativ scurt de urmărire, nu am evaluat gradul de fibroză şi la finalul tratamentului, posibilitatea apariţiei unor modificări importante ai parametrilor fibrozei fiind redusă. Totuşi evaluarea fibrozei necesită supra veghere în dinamică din cauza caracterului progresiv al steatohepatitei non-alcoolice. Una dintre limitele studiului de faţă este numărul relativ mic de pacienţi. Deşi lotul este mic şi există oarecare neomogenitate a parametrilor dislipidemiei şi BMI, efectul terapeutic şi siguranţa preparatului constituie argumente ale utilizării, în condiţiile în care la copil nu se pot utiliza alţi agenţi farmacologici utilizaţi la adult. CONCLUZII Dozele crescute de acid ursodeoxicolic pot avea un efect benefic în tratamentul afectării hepatice cronice la copii cu steatohepatită non-alcoolică. O analiză mai detaliată prin studii randomizate controlate a efectelor acidului ursodeoxicolic în doze crescute în steatoza hepatică non-alcoolică la copil ar putea aduce date suplimentare asupra indicaţiilor de tratament la vârsta pediatrică. De asemenea, sunt necesare studii care să urmărească modificările metabolice asociate inflamaţiei pentru a defini diversele mecanisme de acţiune.

Nonalcoholic fatty liver disease (NAFLD), the most common cause of chronic liver disease in children, encompasses a range of disease from simple steatosis to steatohepatitis (NASH). Ursodeoxycholic acid (UDCA) has been used in adult pacients with NASH, demonstrating its effi cacy in high doses. Purpose. The aim of our study was to assess the effect of high-dose ursodeoxycholic acid therapy on liver functions in children with NASH. Methods. We prospectively studied 18 children with NASH aged 2.5 to 17 years. NASH was operationally defined as diffusely hyperechogenic liver at ultrasonography with persistently elevated (> 35 UI/l) alanine aminotransferase (ALT) or aspartate aminotransferaze (AST) after exclusion of viral, alcohol-induced, druginduced, cholestatic and genetic causes of liver disease. Patients were treated with ursodeoxycolic acid (20 mg/ kg per day) for six month. Results. Treatment with UDCA signifi cantly decreased hepatic transaminases. ALT levels ranged from 68.5 (50-166) at baseline vs. 47 (29-104) after six month, p <0.001, AST from 43 (29-101) 34 (17-75), p < 0.001, and GGT between 41.5 (11-150) vs. 22 (8-55). At the end of treatment, ALT levels normalized in 4 patients. Moreover, cholesterol levels were signifi cantly reduced after treatment. Conclusions. Treatment with high-dose ursodeoxycholic acid improves liver function parameters in children with NASH. Key words: NASH, ursodeoxycholic acid, liver enzymes INTRODUCTION In the last two decades, non-alcoholic fatty liver disease (NAFLD) has reached epidemic prevalence and became the most common cause of chronic liver disease in children (1). NAFLD comprises a spectrum of clinico-histological entities ranging from simple steatosis [Nonalcoholic fatty liver (NAFL)] to steatosis associated with inflammation and cell injury [non-alcoholic steatohepatitis (NASH)], that can involve fibrosis development (2). Some cases may progress to cirrhosis and even hepatocellular carcinoma with portal hypertension in adulthood. Obesity, dyslipidemia and insulinresistance are the most common risk factors associated with pediatric NAFLD. Givin the pro gressive nature of steatohepatitis, several researches has focused on the treatment of this condition. Weight loss, exercise, control of diabetes and dys lipidemia are the main line of treatment in children (3). Associated with life style interventions, a num ber of pharmacological agents such as metformin, vitamin E, omega 3 fatty acid, ursodeoxycholic acid have been shown to have beneficial effects in patients with NAFLD and NASH (4). Ursodeoxycholic acid (UDCA), with its hepatoprotective and anti-apoptotic effects may be beneficial in the management of steatohepatitis development and progression. Clinical trials in adults have shown that the standard dose of urso deoxycholic acid (13-15 mg/kg) is ineffective in the treatment of NASH, but higher doses (28-32 mg/kg per day) may have favorable results on transaminase levels (5). Ursodeoxycholic acid ac tion is dose-dependent and higher doses of 20 mg/kg per day had beneficial effects on biochemistry and liver fibrosis in children with primary sclerosing cholangitis or cholestasis in cystic fibrosis (6, 7). The objective of our study was to evaluate the effects of higher doses (20 mg/kg/day) of ursodeoxycholic acid on liver function parameters and ultrasound in children with non-alcoholic fatty liver disease. MATERIALS AND METHODS The study group included 18 children, aged 2.5 to 17 years, with a mean age of 9.39±4.13, selected from patients diagnosed with non-alcoholic fatty liver, monitored in 2nd Pediatrics Clinic, Emergency

6 79 Hospital for Children St. Maria Iasi from January 2010-December Patients were monitored between 8 months and 2 years. NAFLD was operationally defined as diffuse hiperechogenic liver on ultrasound with persistently abnormal (> 35 IU / L) alanin aminotransferase (ALT) or aspartat aminotransferase (AST), after excluding viral, drug or alcohol induced causes, cholestatic and genetic liver disease. Inclusion criteria were alanine aminotransferase 50UI/l and significant ultrasound. Patients received ursodeoxycholic acid, 20 mg/ kg/day in two divided doses, breakfast and evening, for six months. All patients were counseled regarding dietary changes and regular exercise at initial assessment, but no specific dietary measures was been established. Dynamic monitoring was performed by assessment of liver function, blood glucose, lipid profile and ultrasound every 3 months. Metabolic syndrome parameters were appreciated by IDF criteria, 2007 (8). To assess the degree of fibrosis was performed Fibroscan, reference values from adults being considered (9). We quantify changes in hepatic transaminases at the end of treatment. Statistical analysis was performed using SPSS 19. Statistical significance was established for p-value < RESULTS Clinical and biological characteristics of patients at the beginning of follow-up are shown in Table 1. The levels of liver enzymes were specific for NASH patients, with moderately elevated aminotransa minase and all patients had an AST/ALT ratio less than 1. TABLE 1. GGT values were over 1.5 x NV in 33.3% patients. In 38.8% of patients, fasting glucose levels were above 100 mg/dl and in 11 patients blood glucose was normal. Two patients were within the parameters of metabolic syndrome, presenting abdominal circumference greater than 90 percentile, fasting glucose 100mg/dl, triglycerides 150 mg/dl and systolic blood pressure over 130 mmhg. At least one component of the metabolic syndrome was present in 61.1% of patients. Assessing the presence and degree of fibrosis revealed F2 in 2 patients, F1 in 6 patients and F0 in 10 patients. There was a statistically significant correlation between the degree of fibrosis and ALT levels (p < 0.05) and AST (p < 0.01). Over the follow-up period, no significant changes were reported in lifestyle. During treatment, body weight remained relatively unchanged. Body mass index varied slightly from baseline, ± 6.32 kg/m 2 vs ± 5.78 kg/m 2 (p = 0.12) after six month. At the end of treatment with ursodeoxycholic acid was observed a reduction of hepatic transa minases levels. Median (range) of absolute ALT levels was significantly improved from baseline 68.5 (50-166) to 47 (29-104), p < 0.001, at 6 months. Average ALT reduction was ± ALT levels normalized in 4 of patients at the end of treatment. Aspartate aminotransferase reduction followed a similar trend to ALT, but in a lesser degree. Median (range) AST varied significantly from 43 (29-101) to 34 (17-75), p < Average reduction AST was ± Ursodeoxycholic acid treatment was also associated with a significant decrease of GGT levels, median ranging from 41.5 (11-150) vs. 22 (8-55), (p < 0.05). Also cholesterol recorded statistically significant changes (p <0.01), the median ranging from ( ) vs ( ). Cholesterol changes were not correlated with changes in BMI, suggesting that metabolic effects associated with the administration of UDCA can not be attributed to changes in body weight during treatment. There were no significant changes in total bilirubin, alkaline phosphatase, glucose or triglycerides. There were no changes in ultrasound appearance of the patient at the end of 6 months administration of UDCA.

Variations in liver enzymes at baseline vs. the end of treatment DISCUSSIONS Epidemiological arguments that show an increasing incidence of NAFLD, most often in relation to the unprecedented prevalence of obesity, imposed a pressing need to implement targeted and safe therapies. Ursodeoxycholic acid is a natural, hydrophilic bile acid, which is about 3% of human bile content, with many hepatoprotective and immunomodulating properties. UDCA stabilizes mitochondrial membra nes and reduce the release of hydrolytic enzymes from damaged hepatocytes. Hepatocytes loaded with fat are more susceptible to necrosis and release of reactive oxygen species. In this context, UDCA can improve liver dysfunction and cellular damage caused by oxidative stress in NASH (10). Modulatory effects of anti-apoptotic signaling pathways of UDCA may be a beneficial effect on nonalcoholic steatohepatitis, in which apoptosis was found to be increased (11). In addition, studies have shown that TNF-α have a significant contribution to the development of insulin resistance and TNF-α serum levels were elevated in patients with NASH. At least in patients with chronic cholestasis, UDCA decreased TNF-α levels associated with improved biological and histological parameters (12). Administration of ursodeoxycholic acid in our study resulted in a significant decrease in transaminase levels in patients with NAFLD and 22.2% of patients had a normalization of ALT levels. With no biopsy confirmation of steatohepatitis, we considered as a marker of liver injury higher values of ALT, more than 1.5 x upper limits of normal values. In this context we can assume that the reduction in ALT levels may reflect an improvement in liver inflammation. ALT levels are considered representative parameter for assessing liver necroinflammation improvement with predictive value of histological changes. Correlation with BMI changes shows that weight loss was not the cause involved in transaminase reduction. Also, patients

8 81 showed a significant decrease in cholesterol levels. Dyslipidemia is recognized as a risk factor in the development of steatosis, lower cholesterol levels could improve the development of fatty liver. Data on the efficacy of UDCA in NASH are varied and inconsistent. In 1996, Laurin et al. showed, on a group of 24 adults with NASH, that UDCA in doses of mg/kg/day resulted in a significant decrease in ALT (-30%), GGT (-45%) levels and also decrease histological grade of steatosis (13). Following, a randomized study in adults, Lindor et al., 2004, showed that treatment with UDCA (13-15mg/kg) versus placebo for 2 years reduced ALT levels in the same degree in both groups. Histological there was only an improvement in steatosis, similar in both groups (14). The study by Dufour et al., 2006, showed significant effects of UDCA combination with vitamin E, but not of UDCA versus placebo. Negative results have raised the question whether high doses of UDCA would benefit in NASH (15). Ratziu et al., 2011, in a randomized study in adults showed that high doses of UDCA (28-35mg/kg) resulted in significantly reductions of transaminases, glucose and insulinemia levels, after one year treatment. The study had not histologically end-point, but an assessment of noninvasive markers of inflammation and fibrosis by Fibrotest, also recorded a significant decrease (5). Another study in adults with histologically tracking point showed that administration of mg/day of UDCA has brought improvements in transaminase levels, but these values were not statistically significant compared with the control group. The study showed no improvements in histology after 18 months of treatment (16). Conventional doses of UDCA used on a group of obese children showed no improvement on transaminase levels or on liver ultrasound appearance (17). In our group of children, higher doses of ursodeoxycholic acid decreased hepatic transaminase levels, although normalization of alanine aminotransferase aws only in 22.2% of patients. There were no serious adverse effects; only discrete abdominal pain or diarrhea, which did not require discontinuation of therapy. Due to the relatively short follow-up, the degree of fibrosis at the end of treatment was not assessed, the probability of significant changes of fibrosis parameters being reduced. Nevertheless, the assessment of fibrosis requires dynamic monitoring due to the progressive nature of non-alcoholic steatohepatitis. Limitation of this study is the small number of patients. Even if the group is small and there is some heterogeneity of dyslipidemia and BMI parameters, therapeutic effect and safety of UDCA are arguments of using it in children, given that in these patients can not use other pharmacological agents used in adult. CONCLUSIONS Higher doses of ursodeoxycholic acid may have beneficial effects in the treatment of chronic liver disease in children with non-alcoholic steatohepatitis. A more detailed analysis in randomized controlled trials of the effects of high-dose ursodeoxycholic acid in pediatric non-alcoholic fatty liver could get additional data on its indications for children treatment. Also, studies are needed to monitor metabolic changes associated to inflammation to define different mechanisms of action. REFERENCES 1.
