2 Introduction Your GP or hospital doctors have prescribed for you a course of intravenous antibiotics. They will be given via a cannula into your vein and can now be given in your home or in a community clinic if you do not require any further medical support or hospital admission. The duration of the treatment will depend on the course of IV antibiotics that are prescribed and how you respond to the treatment. Advantages of Home IV Therapy There are many benefits of receiving your treatment in a community setting. Here are a few. Advantages You will not be required to stay in hospital overnight and you will be able to be stay at home. You will be reviewed daily by the specialist nurses from the IV Therapy Team Being treated at home or in the community clinic will enable you to resume your normal day to day living. Further Information The cannula will need to stay in place for the duration of your treatment but we will teach you to care for this (to avoid infection). Each day, a time will be negotiated where you must be at home or in the community clinic. This can only vary slightly on a day to day basis but we will take into consideration your needs as well as our service requirements. There is no reason why you can t carry on doing most things while you are having treatment. If you are unsure of anything, please ask the team. Returning to work during your treatment may or may not be possible depending on the number of times you require treatment each day and your type of work. We will discuss this with you and help you make a decision about this. 2

3 Medications Once an assessment has been completed and if you are happy to receive treatment at home or in the clinic, your GP or hospital doctor will give you a prescription. This prescription should be taken to the hospital outpatient pharmacy (Lloyds ground floor of the eye hospital) if you are being discharged from the hospital. If you are being referred to our service by your GP (your GP, will inform you of where these chemists are). If you or your family are unable to collect your medication from the community pharmacy, please let your doctor know. It may be possible to get it delivered. Safe Storage of Medicines Once you have collected your medications they must be stored correctly: Ensure medicines are stored in a cool, dark, dry place out of direct sunlight Keep medicines out of sight and reach of children Medicines should remain in their original packaging If possible store in a cupboard in a clean plastic box with a lid. Ensure medicines are not left on the floor Avoid storing in warm places such as bathroom cabinets or near radiators. What happens next? Once the referral by your GP or hospital doctor has been received by the community IV therapy team, they will contact you to arrange a time and date for your first visit. These visits can be to your home but there is an option for you to attend a community clinic to receive your treatment there instead. We will discuss this option with you. 3

4 IV Therapy Nurse The IV Therapy Nurse will attend each day to administer your IV antibiotics until the course is completed. This could be once or twice a day. Once the IV course is completed, the nurse will direct you in how to take your oral doses (tablets or medicine) for the following days if these have been recommended by your doctor. During each visit, the nurse will observe the cannula in your arm to ensure there are no signs of infection. They will also be monitoring your condition by checking your blood pressure, temperature and heart rate. You may also have to have a blood test and the results will be communicated back to your doctor. Once the course has been completed, your GP or hospital doctor will be informed. Cannula Care Advice. Your cannula will be secured with a dressing and a bandage will be placed over it to protect it. Please leave the dressing and bandage in place and keep the area clean and dry. The nurse will check the cannula site at each visit to ensure there are no problems. 4

5 Here are some potential problems that can occur and advice as to what to do. Advantages Cannula falls out Leaking around site Blood in line Infection around cannula site The dressing is no longer securing the cannula in place, or the cannula falls out completely. This could be due to the area being knocked or the dressing not being secure. If the dressing is wet underneath it will affect the adhesive of the dressing and be less secure. This could lead to the cannula falling out. Blood can be visibly seen within the line Any redness, pain, swelling around the cannula site, or along the arm which has been used for treatment Disadvantages - Place a piece of gauze or a clean tissue over the site and apply pressure for minimum 5 minutes and then apply a plaster. - If the site continues to bleed or becomes painful please contact the IV-ACT team or out of hours number. 6 Symptoms you may experience if your condition is deteriorating are: Increased temperature Confusion Unstable blood glucose levels (if you are a diabetic patient) Feeling generally unwell. If you experience any of the above or development a rash or start with diarrhoea or vomiting then these symptoms should be reported immediately.
