1 Mount Sinai Hospital 600 University Avenue, Suite 455 Toronto, Ontario M5G 1X5 Tel: (416) Fax: (416) Assistant Professor Department of Surgery University of Toronto GI SPECIAL PROCEDURE COLONOSCOPY From your pharmacy, please purchase: 3 Dulcolax laxative tablets, 2 packets of Pico-Salax and 1 Fleet enema Monday Take 3 Dulcolax laxative tablets on Monday night 7:00 p.m. Tuesday DO NOT EAT ANY SOLID FOOD ON TUESDAY CLEAR FLUIDS ONLY ALL DAY. Clear fluids include: water, clear fruit juices (i.e. apple, cranberry), bouillon, plain jello. Drink at least 2 litres of clear fluids. No solid foods, milk or milk products. AT 12:00 NOON Mix 1 packet of Pico-Salax with 5oz or 150ml of cold water. Stir well for 2-3 minutes. Sometimes the mixing of Pico-Salax will make the water hot. If this occurs, wait until it cools before you drink it. Drink that followed by at least 3 more cups of any clear fluid. (1cup = 8oz / 250ml) AT 6:00PM You will need to mix and drink the other packet of Pico-Salax in the same way. Then drink 3 more cups of clear fluid. More fluids are better. Drink as much as you can. It is very important to drink lots of clear fluids for this test. AT 9:00PM Use Fleet Enema Plan to be near a bathroom for the entire evening and night. If you are taking your laxative solution and develop symptoms of nausea or vomiting, stop the preparation process for an hour or so. Then if you feel better, try to pick up where you left off. After midnight you may continue to drink water only until 6 hours before your colonoscopy. Please take all your usual medications with water. If you are on iron medication, please stop ten days prior. If you are on Aspirin stop 7 days prior. If you are on a blood thinner such as Coumadin (warfarin) please discuss with your physician well in advance of test. Your appointment has been made for: Please be at Mount Sinai Hospital main floor Admitting Department at on the day of colonoscopy. Entrance from Murray Street. You must have an arranged lift home from a family member or friend after the procedure as you will be drowsy and will not be able to drive yourself, take a taxi or public transport. Your procedure will be cancelled if this lift is not prearranged. You can be picked up approximately two hours from the time of the appointment booking in the GI Unit on the 6th floor. Please be sure to call to cancel or reschedule your appointment 72 hours before your procedure. Failure to cancel or not attend your scheduled procedure will result in a charge.

If you are a diabetic, please talk to your doctor or call the endoscopy triage nurse at about how to take your medication in order to prevent low blood sugar. One Week Before the Procedure: Start a LOW fiber diet for at least 5 days prior to your procedure. See attached. Do not take iron for at least 5 days prior to your procedure. Purchase 2 Crystal Light (32 oz. each) or 2 Gatorade (32 oz. each) of any color except red. Purchase 2 Dulcolax LAXATIVE tablets and a 238 gram bottle of MiraLax from the pharmacy. No prescription is needed. Purchase 1 Fleet saline enema from the pharmacy. No prescription is needed. If you do not regularly have a daily bowel movement, you need to purchase additional MiraLax and take 17 grams once daily for the week prior to the procedure. One Day Before the Procedure: DO NOT EAT FOOD. DRINK CLEAR LIQUIDS ONLY. Drink as much clear liquid as possible throughout the day. Water, tea, coffee (no milk or cream), soda, broth, clear juice, popsicles, and JELL-O are examples of clear liquids. DO NOT INGEST RED LIQUIDS. Between 2 and 4 p.m. take 2 Dulcolax tablets with a full glass of water. If Your Procedure is Scheduled BEFORE 11 AM in the Morning: Three hours after taking the Dulcolax, mix HALF of the bottle of MiraLax into the first 32 oz. of Crystal Light or Gatorade and the other HALF of the bottle of MiraLax into the second 32 oz. of Crystal Light or Gatorade. Shake the solution until MiraLax is dissolved. Drink 8 oz. every minutes until the entire solution in BOTH bottles is gone. The solution will cause you to have loose stools, which is the expected result. Even if you have liquid diarrhea, please continue taking it. You may have nausea while drinking the solution, but you should drink all of it to ensure an adequate preparation. Continue drinking clear liquids until bedtime. If Your Procedure is Scheduled AFTER 11AM in the Morning: Three hours after taking the Dulcolax, mix HALF of the bottle of MiraLax into the first 32 oz. of Crystal Light or Gatorade and the other HALF of the bottle of MiraLax into the second 32 oz. Shake the solution until MiraLax is dissolved. Drink 8 oz. every minutes until the entire solution in the FIRST bottle is gone. The solution will cause you to have loose stools, which is the expected result. Even if you have liquid diarrhea, please continue taking it. You may have nausea while drinking the solution, but you should drink all of it to ensure an adequate preparation. Continue drinking clear liquids until bedtime. The NEXT morning, at least 5 hours before the procedure is scheduled, drink the SECOND bottle of 32 oz. Crystal Light or Gatorade. Drink 8 oz. every minutes until the entire solution in the SECOND bottle is gone. The Morning of the Procedure: DO NOT EAT FOOD. You may continue drinking clear liquids but MUST STOP 4 HOURS BEFORE THE PROCEDURE. You may take your regular medications unless otherwise instructed by your doctor, with sips of water. Other than this, have NOTHING AT ALL BY MOUTH 4 HOURS BEFORE THE PROCEDURE or your procedure will need to be CANCELLED. Your stool should be clear. If it is not, use 1 Fleet saline enema as instructed on the box. In order for your procedure to be performed, you must arrange for an adult to meet you in the Endoscopy Center and either drive you home or accompany you in a taxi or on public transportation. Your ride must be flexible, as procedures can run longer than expected. You cannot be discharged from the Endoscopy Center until your ride arrives to accompany you home.

3 Low-Fiber Diet A low-fiber diet limits the amount of food waste that has to move through the large intestine. PLEASE FOLLOW THIS DIET FIVE DAYS PRIOR TO THE PROCEDURE Foods Recommended Breads, Cereal, Rice and Pasta: White bread, rolls, biscuits, croissants and melba toast Waffles, French Toast and pancakes White rice, noodles, pasta, macaroni and peeled cooked potatoes Plain crackers and Saltines Cooked cereals: Farina, Cream of Wheat Cold cereals: Puffed Rice, Rice Krispies, Corn Flakes and Special K Vegetables: Tender Cooked and canned vegetables without seeds: carrots, asparagus tips, green or wax beans, pumpkin, spinach, lima beans Fruits: Strained fruit juice Canned fruit, except pineapple Ripe bananas Melons Milk/Dairy: Milk, plain or flavored Yogurt, custard and ice cream Cheese and cottage cheese Fats, Snacks, Sweets, Condiments and Beverages: Margarine, butter, oils, mayonnaise, sour cream and salad dressing Plain gravies Sugar, clear jelly, honey and syrup Spices, cooked herbs, boullion, broth and soups made with allowed vegetables Coffee, tea and carbonated drinks Plain cakes and cookies Gelatin, plain puddings, custard, ice cream, sherbet and popsicles Hard Candy or pretzels Ketchup and mustard Meat and other proteins: Ground, well-cooked tender beef, lamb, ham, veal, pork, fish, poultry and organ meats Eggs Peanut butter without nuts Foods to Avoid Breads, Cereal, Rice and Pasta: Breads or rolls with nuts, seeds or fruit Whole wheat, pumpernickel, rye breads and corn bread Potatoes with skin, brown or wild rice and kasha (buckwheat) Vegetables: Raw or steamed vegetables Vegetables with seeds Sauerkraut Winter squash, peas, broccoli, Brussels sprouts, cabbage, onions, cauliflower, baked beans, peas and corn Fruits: Prunes and prune juice Raw or dried fruit All berries, figs, dates and raisins Milk/Dairy: Yogurt with nuts or seeds Fats, Snacks, Sweets, Condiments and Beverages: Nuts, seeds and coconut Jam, marmalade and preserves Pickles, olives, relish and horseradish All desserts containing nuts, seeds, dried fruit, coconut or made from whole grains or bran Candy made with nuts or seeds Popcorn Meat and other proteins: Tough, fibrous meats with gristle Dry beans, peas and lentils Peanut butter with nuts Tofu
