3 ABOUT US Washburn County Health Department has served the community for 82 years beginning in Washburn County Health Department continues to meet the statutory requirements for a Level II Health Department in the State of Wisconsin following the 140 Review completed in 2014 (next due in 2019). The Health Department underwent several staffing changes in Joan Romanowski started as the Account Clerk II in January, assisting with financial reporting and payroll, among other duties. Dianna Stumph (RN, BSN) retired in June after 25 years as a Public Health Nurse. The vacant Public Health Nurse position was converted to a Public Health Specialist position in response to the movement of Public Health away from providing direct clinical services and towards prevention and education services. The Public Health Specialist position was filled in September by Cassidy Watson (MPH). Cassidy manages several grants related to opioid abuse prevention, carbon monoxide poisoning, maternal and child health, and the Washburn County Community Health Improvement Plan. Many of the grant efforts include community outreach throughout Washburn County. 4 SERVICES COMMUNITY HEALTH IMPROVEMENT PLAN Local health departments are required under Wisconsin Statute 251 to regularly undergo the Community Health Improvement Process, or the process of conducting a Community Health Assessment followed by developing and implementing a Community Health Improvement Plan. A Community Health Improvement Plan is a long-term, systematic effort to address problems based on the results of community health assessment activities and the community health improvement process (see figure below). Washburn County, in partnership with Spooner Health, conducted a Community Health Assessment in A Community Health Improvement Plan was developed to run from targeting five health priorities: 1) Alcohol, Tobacco, and Other Drug Abuse, 2) Mental Health, 3) Access to Healthcare, 4) Chronic Disease Prevention, and 5) Nutrition, Obesity, and Physical Activity. In an effort to better partner with Spooner Health, the existing Community Health Improvement Plan will be extended through 2021 so that the Health Department can be on the same Community Health Assessment schedule as Spooner Health. Spooner Health conducted their own 2018 Community Health Assessment, which will provide the opportunity for a mid-point evaluation and adjustments to the Washburn County Community Health Improvement plan. 5 PUBLIC HEALTH EMERGENCY PREPAREDNESS Washburn County Health Department engages in a continuous planning process with community partners in preparation to respond to public health emergencies, from large scale disease outbreaks to acts of bioterrorism to ice storms. The Health Department is part of a Regional Preparedness Coalition which provides for continuity of planning across county lines. Volunteer Reception Center (VRC) Training was again conducted with our local volunteer groups, using lessons from the 2017 Barron tornado. Shell Lake Area United Methodist Church graciously allowed us to use their church as a VRC site. Many thanks to our volunteer groups: Faith Lutheran Church, Shell Lake Lions, and Spooner/Trego Lions, as you are a vital part of our Emergency Response Team. The Health Department is a member of the Western Wisconsin Public Health Readiness Consortia (WWPHRC), a multi-county coalition that supports public health emergency preparedness. Through the WWPHRC, the Health Department conducted a tick education campaign, a severe weather education campaign, and a handwashing education campaign. WWPHRC Members Note: Star denotes St. Croix Tribal Health Department ENVIRONMENTAL HEALTH Under Wisconsin Statute 254, the Health Department is responsible for investigation and abatement of known Human Health Hazards to the public. Washburn County Health Department conducted 0 immediate Human Health Hazard investigations in CHILDHOOD LEAD MONITORING The Washburn County Health Department provides Childhood Lead Screening Surveillance and Lead Abatement under Wisconsin State Statute 254. There is no safe level of lead in the human body. Even very low levels of lead can cause permanent brain damage and negatively affect health throughout life. The Centers for Disease Control defines lead poisoning as a blood lead level of 5 mcg or greater. There was 1 child with elevated blood lead levels in Washburn County in

6 COMMUNICABLE DISEASE CONTROL AND PREVENTION Wisconsin Statute 252 requires that local hospitals and clinics report communicable diseases to Local Health Departments for investigation and surveillance. Preventing the spread of disease remains a public health priority. In 2018 the Washburn County Health Department conducted 301 communicable disease investigations, confirming 234 cases. Tick-borne diseases, including Lyme, Ehrlichiosis, and Babesiosis, accounted for 50% of the reported diseases. However, tick diseases did decrease in Enteric (food & water borne) diseases were up from 2017 (25 vs. 15) of reported diseases are tick-borne Sexually Transmitted Infections (STIs) were up from 2017 (36 vs 22), with Chlamydia still accounting for the majority of the cases. 7 REPRODUCTIVE HEALTH SERVICES The Washburn County Reproductive Health Program provides services through the Maternal Child Health block grant, per Wisconsin Statutes 252 & 253. In 2018, 72 clients received on-going Reproductive Health Services. The program provides: Reproductive health education and counseling, birth control education and supplies, sexually transmitted infection risk assessment, education, counseling and limited treatment, pregnancy prevention and early detection and referrals as needed. MATERNAL CHILD HEALTH PROGRAM Programs available for families with children include: Safe Sleep Initiative - 50 infants die in Wisconsin every year from unsafe sleep practices. In 2018, the Health Department offered community Safe Sleep education and training. This training focused on the ABCs of safe sleep: Alone, on their Back, in a Crib, in a Smoke-free environment. 4 community groups and 2 daycares received training. Safe Sleep Baby Boxes - Free baby boxes are offered to new and expecting mothers who do not have a safe place for baby to sleep. Each baby box contains a covered mattress pad and a spare mattress cover. Child Death Review (CDR) Team - Team of community stake holders who review all child deaths in order to identify community strategies to prevent additional deaths. In 2018, there were 0 child deaths in Washburn County. Newborn Mailing Program - Education packets containing materials on Immunization, Infant Safety, Healthy Birth Spacing and other Public Health Services are mailed to the parents of all Washburn County newborns. 108 packets were mailed in Well Water Testing - Kits are offered at no charge to households where a pregnant or breastfeeding woman and/or an infant resides. packets mailed as part of Newborn Mailing Program 07

8 WIC (WOMEN, INFANTS, AND CHILDREN) The WIC Program is a Supplemental Nutrition Program for Women, Infants and Children. WIC helps income eligible pregnant, breastfeeding and postpartum women, infants, and children up to age 5 years who have a health risk due to inadequate nutrition. The program improves pregnancy outcomes, reduces infant mortality and provides infants and children with a healthy start in life by improving poor or inadequate diets. In 2018, 441 participants received nutrition education and food benefits. Women Infants Children (1 up to 5) Total 08

9 COMMUNITY ACCESS POINT 120 Clients assisted Washburn County Health Department assists clients in enrolling in all Forward Health Programs. In 2018, the Washburn County Health Department assisted 120 participants in one or more Forward Health programs. Forward Health Programs are: - BadgerCare+ - Food Share - Child Care - Family Planning Only Services RABIES CONTROL State Statute 254 requires that the Health Department investigate all animal bites/exposures involving humans. Rabies is almost always fatal, unless post-exposure prophylaxis (treatment) is given. Public Health Nurses provide education regarding the need for human medical evaluation. They also order and enforce animal quarantine or rabies testing. All domestic animals, including dogs, cats, and ferrets, must be quarantined for 10 days following a bite or other exposure to a human. If an animal has a current rabies vaccine, they may be allowed to quarantine at home. Unvaccinated animals must be quarantined at a veterinary clinic at the owner s expense. Wild animals involved in a bite to either humans or pets are humanely euthanized and tested for rabies. In 2018, 54 animal bites/exposures were reported and investigated. 0 animal or human cases of rabies were reported. DOGS TOTAL BATS CATS 09

10 IMMUNIZATIONS Under Wisconsin Statute Chapter 252 and in conjunction with the Vaccine for Children Program and the Wisconsin Immunization Program, Washburn County Health Department offers recommended childhood vaccines to children 0-18 years, who meet program guidelines. In addition, a limited selection of vaccines are available to adults with no vaccine insurance coverage. As part of our chronic and communicable disease surveillance duties, we monitor the immunization records and send vaccine due letters to the parents of all Washburn County children under the age of three. In 2018, 65% of all Washburn County 2 years olds were up-to-date with 69% considered late up-to-date (completed by age 3). The state average is 72.93% up-to-date. Washburn County s rate is down over 10% from 2017, which is of concern as it means Washburn County is at risk for a vaccine-preventable disease outbreak, such as measles. 11 CARBON MONOXIDE POISONING PREVENTION The Health Department was awarded a grant through the Wisconsin Department of Health Services Environmental Tracking Program to conduct outreach activities and provide free detectors to low-income families to prevent carbon monoxide poisoning. Washburn County s rate of ER visits related to carbon monoxide is 28.1 per 100,000 people far in excess of the statewide rate of 7.9 per 100,000 people. 50% of detectors were distributed with the other 50% reserved for 2019 grant activities. OPIOID EPIDEMIC Over the summer, the Health Department was awarded an Opioid Education mini-grant through the Wisconsin Department of Health Services Opioid Harm Prevention Program (OHPP), mandating the use of CDC Prescription Awareness campaign materials during a three-month period. Two billboards were placed at the north and south ends of the county, as well as a 30-second video advertisement spot in the Spooner movie theater. Due to the success of the initial campaign, OHPP awarded the Health Department additional funds to expand the CDC Prescription Awareness campaign, from October 2018 through August Advertisements (seen to the left) are in 60 bathroom stalls in establishments throughout Washburn County. A Social Media Advertising campaign utilizing targeted Facebook advertisements is also underway.
