Objectives Describe how a comprehensive self-management diabetes education program can affect patient care. Define and clarify community based programming and resources available for individuals with chronic diseases and individuals caring for an individual with a chronic disease. How a Comprehensive Self-Management Diabetes Education Program Can Affect Patient Care
Our Unique Self-management Program Classes meet once per week for 2.5 hours for 3 consecutive weeks Patients are motivated to make lifestyle changes through Personal Quality Improvement Patients are offered: personal consultation referral to nutritionist problem solving regular follow-up phone calls to patients a community diabetes support group for ongoing support
The format of the diabetes education was changed to include group/classroom teaching Individual patient needs were assessed one-onone prior to being referred to the class Classes met once per week for 2.5 hrs, for 3 wks Patients were introduced to CQI methods during the first class, and encouraged to engage in personal quality improvements (PQI) of diabetes Managing your Diabetes You Can Do It! workbook used as classroom text Workbook was given to each patient
Workbook Description 8 chapters Ch 1 provides general introduction to diabetes, and involves patients in PQI and data collection immediately Ch 2 explains the theory behind CQI Ch 3-8 comprehensively covers diabetes education in line with requirements from the American Diabetes Association Structured to encourage and rely on patient participation In-Class Exercises Homework assignments
More on PQI Patients identify one personal challenge related to diabetes management e.g. the need to exercise more frequently, or eat less carbohydrates, and so on. Patients are asked to begin data collection immediately! To gain information about their challenge, and to identify a course of action Patients are taught data collection techniques to monitor changes Use of data table Graph on a control chart
