Community Referral Program Information Packet Through this telephonic health education and case management assistance service, we provide continuity of care that enhances patient outcomes and health literacy. Following a referral submitted by your office, our highly trained staff calls the patient and works to meet their needs by providing education, navigation, counseling, and social support. This program can also collect and report non-identifying data back to the provider, helping reach best practices and meet outcome goals. Patient assistance, education, counseling for: Lung Cancer COPD Asthma Community Resources Medications And more! Our Staff: Registered nurses, respiratory therapists, certified tobacco treatment specialists, and counselors Spanish speakers on staff Interpreting service for over 200 languages 2 Community Referral Program Information Packet

What We Do Provide a means for individuals to become empowered by providing health education, promoting engagement in local health programs, and assisting with case management needs. Connection to Resources Connect your patients to local resources to ensure they have continuity of care. Available resources include: Better Breathers Club No one has to live with COPD alone. These Clubs are welcoming support groups for individuals with COPD. Patients can enter into a network of social support and mutual empowerment while participating in educational presentations ranging from breathing techniques to lung transplants. Arizona Smoke-Free Living This program creates smoke-free living options in Arizona by promoting smoke-free policies in residential communities. Your patients can find smoke-free housing with this resource. Freedom From Smoking ranked as one of the most effective smoking cessation programs in the country, Freedom From Smoking is available as an online program or as a group clinic. Since quit plans are not one size fits all, both formats offer a variety of evidence-based techniques to incorporate into their own individualized quit plan. Any telephonic cessation support service requests will be redirected to Arizona Smoker s Helpline. RESOURCES Outcome and Data Reporting A wide variety of data measures can be collected for you as our health professionals interact with your patients. Reporting can be customized to each provider s needs. Below are a few examples of customizable plans.
