Take-A-Break: Consultation on the outcome of our review of short breaks for adults with a learning disability and their carers Consultation Questionnaire 1

We wanted to know what people with learning disabilities and their families think about short break services in the NHSCT. We did this by talking to people with learning disabilities and their families. We also met with public representatives and with people who provide services to people with learning disabilities. We wrote a report with recommendations about how we could make our short breaks better. We want to know what people think about our plans to make things better. We would like you to complete this questionnaire. This will help us to know whether our plans for our short break services are good ideas or not good ideas. learning disabilities. Draft 1 August 2016

About you Please tell us whether you are answering this questionnaire By yourself On behalf of an organisation or group What is the name the organisation or group? Please tell us your : Name: Address: Phone number: Email Address: Are you happy for us to contact you in case we want to talk to you about your answers? August 2016

Our Recommendations We will make sure that the people who need the most support get the help that they need first. We will do this by making sure that we are person centered and look at the needs of the person with a learning disability and their families or carers. Do you think this is a good idea? We will make sure that carers needs are fully assessed. This means that we will look closely at the help and support that is needed so that we can provide the right type of short break. Do you think this is a good idea? We will make sure that we give carers better information about what types of short breaks are available. Do you think this is a good idea?

We will encourage people to choose self directed support. We will develop our short break services for people with challenging behaviours. We will also make sure that staff are properly trained and that short break buildings are the right place for people with challenging behaviours. Some people have complex needs. We will pay for people with complex needs to have short breaks in places like Private Nursing Homes or more specialist units. This will be available for as long as is needed.

We will develop our Share the Care service. We will provide short breaks which include things to do during the day. We will keep thinking about how we can improve our short break services. We will do this by working with a group of people with learning disabilities and families

We will make sure that people who plan services in rthern Ireland know that there is a need for more short break services. Please write any other comments about our short breaks service in this box. August 2016
