We are cmmitted t: wrking in cllabratin with bth internal and external partners being a lcal, prvincial and natinal resurce fr knwledge translatin and knwledge exchange ensuring excellence in prviding and supprting educatin in aging and health building knwledge, awareness and system capacity t prmte successful aging, and prevent r delay the expressin f frailty and its harmful cnsequences achieving excellence by integrating evidence infrmed practice, facilitatin and quality imprvement prmting an exceptinal learning experience fr students and trainees, staff, patients, care partner, families and cmmunity. CSAH Activities 2017-18 Recent funded prjects Reginal Educatin and Knwledge Exchange Prgram fr Health Care Cnsent and Advanced Care Planning ($263,000 fr 2015-17 frm SE Ontari LHIN) The Center fr Studies in Aging & Health (CSAH) staff cmpleted a cllabrative prject with the Suth East Health Care Cnsent (SE HCC) and Advance Care Planning (ACP) wrking grup t strengthen health care prvider educatin in all health care sectrs. This included the creatin f an nline prtal f infrmatinal and educatinal resurces t supprt HCC and ACP in SE Ontari. Educatinal resurces were develped and pilted t increase capacity and knwledge exchange. A reginal awareness raising, and disseminatin strategy was successfully implemented t key stakehlders inclusive f primary care, cmmunity care and LTC settings Outreach and Supprt t Ontari Cmmunities in Develping Age-Friendly Cmmunities and Utilizing Finding the Right Fit Age-Friendly Cmmunity Planning Guide ($200,000 fr 2015-17 frm Ontari Senirs Secretariat) The CSAH in partnership with Senirs Health Knwledge Netwrk (SHKN), Universities f Waterl, Huntingtn and Queen s University and the Ontari Interdisciplinary Cuncil fr Aging and Health (OICAH) were the recipients f a $200,000 Senirs Secretariat Grant (lcal funding $120,000 ver 2 years). This was used t supprt the implementatin Age-Friendly Cmmunity initiatives in Ontari. The CSAH team prvides leadership t prject prmtin and knwledge exchange effrts by identifying, adapting and disseminating infrmatin thrugh use f a custmized web prtal, Knwledge Brker, mnthly e-newsletter, webinar series and scial media. An evaluatin f 56 funded AFC cmmunities in Ontari was cmpleted. Expanded Outreach and Supprt t Ontari Age-Friendly Cmmunities ($382,000 fr 2016-19 frm Ontari Ministry f Senirs Affairs) Fllwing the successful develpment f the Ontari Age-Friendly Cmmunities Outreach Prgram, we were successful c-applicants in an applicatin fr expansin f the prgram t extend the scpe and fcus t include meeting the needs f small urban-rural cmmunities, and special ppulatins (indigenus and immigrant grups). We cntinue t supprt 84 agefriendly cmmunities in planning, implementatin and evaluatin activities via a dynamic website, mnthly e-newsletter, webinar series and knwledge brker supprt t 4 reginal age-friendly netwrks.

CSAH Activities 2017-18 Onging Knwledge Exchange & Capacity Building Activities Reginal Integrated Falls Preventin and Management Strategy fr Older Adults in Suth East (SE) Ontari. CSAH is ne f the leads in Suth East Ontari Integrated Falls Strategy t supprt increased capacity fr falls preventin and management in Suth East Ontari. The grup prvides reginal leadership and strategic directin fr the develpment f an integrated evidence-infrmed apprach t senirs fall preventin acrss the cntinuum f care in Suth East Ontari. The wrk was supprted thrugh multiple funding surces (Ontari Research Calitin (ORC) Early Researcher Funding and in-kind cntributins by Ontari Neurtrauma Fundatin). CSAH and Divisin f Geriatrics internally supprt this wrk. BSS-2018 Online Training & Educatin: Team-based Persn-Centred Care The Behaviural Supprts Services (BSS) initiative aims t prvide a blend f self-directed and mentred nline learning pprtunities fr Mbile Respnse Team (MRT) members and their Lng-term Care Hme (LTC) liaisns. This wrk has been undertaken by CSAH in cllabratin with Prvidence Care s Cmmunity Senirs Mental Health & Behaviural Supprt Services. The bjective is t ptimize team-building within the MRT grup as well as prviding them with the skills t engage with and maintain cllabrative engagement with their LTC clients. Thrugh the sharing f infrmatin, educatin and mentrship, the MRT Team and LTC strive t wrk tgether t deliver persn-centred care fr the elderly in LTC hmes. Practicum and Mentring Opprtunities fr Graduate and Pst-Dctral Students Interested in Pursuing Research Opprtunities in Aging and Health CSAH regularly have 3-4 such students each year frm Queen s University Health Sciences Departments attached t CSAH frm a variety f disciplines and backgrunds. Annual Cnference The CSAH annual cnference is a successful way f engaging cmmunity members and health care prfessinals n tpics f particular interest t lder peple and thse wh care fr them. It blends research with emerging best-practices frm the field. It perates n a cstrecvery basis. The 2018 Cnference Empwering Older Adults: Opprtunities and Pssibilities, will be held n May 25th.

It features a prtal fr Senirs Health Knwledge Netwrk and prvincial Age Friendly Cmmunity prtal in French and in English. Pages are specifically designed fr caregivers and health care prviders frm multiple sectrs such as cmmunity, hspitals, lng-term care and primary care. Sageducatin Sageducatin is the centres learning cntent management website. Curses are maintained and updates fr specific audiences. Gerntlgical Online Learning and Develpment (GOLD) Resurces T address the cntinuing educatin needs f Regulated Health Care Prfessinals and Unregulated Health Care Prvides, CSAH has created and develped a series f nline curses geared at prviding engaging and interactive learning resurces n a variety f tpics that are f cncern t thse prviding care t lder ppulatins. The wrk has been supprted thrugh multiple external funding surces and internally supprted by CSAH and Divisin f Geriatrics. The resurces are available t be used by staff at Prvidence Manr, Prvidence Care Hspital, CCAC, and within cntinuing educatin activities.
Partnerships Partnerships We are actively prmting the creatin f an Ontari netwrk f researchers interested in age-friendly cmmunities, in cllabratin with Evergreen. The intent is t facilitate dialgue, sharing and cllabratin between researchers and practitiners. This prcess will culminate in at a tw- day meeting in May 2018. Representatives f CSAH are actively netwrking and participating in a number f lcal and prvincial initiatives including: OASIS Bard Reginal Elder Abuse Preventin (REAP) Suth East Health Care Cllabrative Cuncil f Clinical and Academic Prgrams in Aging and Health at Prvidence Care Ontari Interdisciplinary Cuncil f Aging and Health Reginal Geriatric Prgrams f Ontari (RGPs) Ministry f Senir s Affairs Liaisn Cmmittee, Ontari Geriatric Learning Center Steering Cmmittee We prvide nging supprt and leadership t the Senirs Health Knwledge Netwrk (SHKN), including hsting web prtal, crdinating e-newsletter and mnthly meetings. Cntinue t expand prvincial and natinal netwrking and active cllabratins, including: Establishment f Prvincial Naturally Occurring Retirement Cmmunity (NORC) Resurce Centre is being explred by three rganizatins that are already cllabrating t bring an innvative Naturally Occurring Retirement Cmmunity (NORC) mdel t Ontari. The partners are: UHN OpenLab (Trnt), Oasis Senir Supprtive Living (Kingstn), and The Centre fr Studies in Aging and Health (CSAH) at Prvidence Centre/Queen s University (Kingstn). We have been invited thrugh the SHKN t submit a prpsal fr develpment f a Knwledge Translatin and Transfer Hub related t Develpmental Disabilities by Ontari Ministry f Cmmunity and Scial Services. The Research & Curriculum Librarians at Fanshawe Cllege have apprached us fr assistance n a research prject entitled "Digital Inclusin f Marginalized Senirs: Access t Health Care Infrmatin", t supprt senirs in the Lndn area in accessing healthrelated resurces. This prject is funded via a grant frm ecampus Ontari.
