Premier On-line Training in Integrative and Functional Medical Nutrition Therapy (IFMNT) How the Microbiome Shapes the Systemic Immune System in Health and Disease Q and A Session Susan Allen-Evenson RDN, CCN Hosts Kiran Krishnan!

Regarding the inner and outer mucus layers inside the gut: what happens after an enema, or preparation for a colonoscopy, or even the colonoscopy procedure? Do any of these 'damage' these mucus layers? What can be done to repair it? How long does it take to repair? How do you rebuild the mucosa layer once it has broken down?

The neonate microbiome development and exposures would be best under what diets - breast milk or formula?

What are your thoughts on Mast Cell Activation Disorder (MCAD) and what could be done to reverse it or at least mitigate the histamine response?

So for example, a child with eczema could in theory have a greater risk of developing another autoimmune disease later in life?

Please comment on how a disrupted microbiome and inflammation might contribute to adolescent acute lymphoblastic leukemia

What salt increases gut inflammation??what about natural salt from earth vs processed salt What about those with adrenal insufficiency who need more salt? Is there evidence that salt increases IL-17 and autoimmunity in those patients?

Are you saying the gut microbiota regulate glucocorticoids, like cortisol, production in the gut? Is cortisol produced anywhere besides the adrenals? How do we address adrenals?

Please describe/specify the kinds and quantities of colostrum and lactoferrin (the latter is in whey protein concentrate, right?) Where can you find lactoferrin? amount??

Seems like if you can only get one cytokine draw, a fed state after a typical meal would be better than fasted, as opposed to most other labs we request. You suggested eating Mc Donald s meal. Would it not be better to do a typical meal a person normally eats before testing? Does the CRP-hs test reflect gut inflammation?

Do we know which probiotic Dr Mc Farlin uses at UNT in his studies?

Is my understanding that probiotics have to be specific to the bacteria being treated and most sold in the market aren't as such... how do we know the exact strain before we suggest supplemental probiotic?

How do you know the quality of the probiotic? Some are refrigerated, others are not. Is there a dose formula? Would like to know sources for high quality specific microorganisms, such as Bacterioides fragilis, faecalibacterium prausnitzii?

What is your opinion of Florastor (Saccharomyces boulardii)? I have patients that claim it helps a lot with stomach discomfort. Any drawbacks of using regularly?

The elderly are more prone to c.diff, especially when in nursing homes - how could we reduce these occurrences? Elderly are more prone to SIBO as well - can we slow this from occurring? Is there any reason if you tolerated milk prior to having C-dif that you should avoid it when/ if you get C-dif?

I was in a recent discussion talking about foods such as Kombuchi Tea, foods with probiotics. It was brought up that we are not really delivering probiotics because of stomach acid neutralizing. Also same theory with FOS/Inulin containing foods are not getting to large intestine. What are you seeing Kiran? I've read that there is more esophageal and colon cancer in Asian countries that eat a lot of fermented vegetables and fish. Why is this?

Regionality without supplementation: are local soil bacteria important even if the food is "clean"? For instance, if I eat organic quinoa from the other side of the globe, could those soil bacteria pose an issue vs say locally grown cabbage? How could this affect other environmental allergies? Any newer data on this that you have seen?

Several times you mentioned glysophotase, ie., Roundup. How does this impact the microbiome and are there residual effects in commercially grown food?

Does the less toxic version of LPS vary from best response from highest fatty cold water fish ->true triglyceride O3 supplements -> O3 supplements that are returned to triglyceride from ethyl esters -> O3 ethyl ester supplements (lowest good response)? Or is there a negligible difference along that variable of absorption & utilization by our body in relation to the LPS? How can we test for LPS? How do you define high fat? What do biofilm disruptors do to LPS? Would it likely increase endotoxins & translocation of LPS? Does LPS affect the migrating motor complex?

So if I am working with a client with dysbiosis and they are taking herbal antimicrobials, it is making their condition worse (increases LPS)? What role does 30 day herbal "parasite cleanses" have in affecting gut microbiota? Your explanation of not using antimicrobials is counter to the protocols we learned in training. Is there a place for both?

Is NAFLD just from endotoxemia or also from fructose (Robert Lustig - fructose turned into glucose in liver, glycerol is intermediate = increased triglycerides)?

Would you please discuss a protocol to follow when someone has to take antibiotics.

Fungal meningitis in 45yo male patient who recognizes he over-consumed carbs (especially simple carbs). Then after a month, got shingles. Any benefits to taking MegaSporeBiotic?

Would MCT Oil have the same affect on gram negative bacteria as Coconut Oil? Do you recommend avoiding routine use of coconut oil? OMG! Coconut oil can be driving people's autoimmunity and endotoxemia because it causes die off of bacteria. So saturated fat leads to more toxemia. Do you only recommend omega 3 sources of fat??? I recommend monolaurin often with candida etc.. Is that a mistake?

How does palm oil fit in to those oils slides and did you mean refined coconut oil? Would avocado oil & other high MUFA oils be considered neutral like olive oil?

What about the ketogenic diet? It is high in fat, does it thin the mucus layer? How is it protective against cancer?

To what extent is intermittent fasting negatively affected if someone is taking medication which requires a small amount of food probably up to 50 calories, mostly carbohydrate probably one and no more than 2 times during the 16 hour fast? Thank you You promoted intermittent fasting, eat in 8 and not for 16. I heard in another presentation where it was suggested it was not good for post menopausal women to do intermittent fasting as it negatively effected bone health. 12 hrs over night was ok. It had to do with calcium metabolism and the VDR gene. Your thoughts?

Re: slide on what can we do? Do you recommend these strategies for those already with autoimmune diseases or for all of us attempting to prevent inflammation and autoimmune diseases? If you could pick 5 or 6 main strategies (since clients would resist doing all of it) what would you recommend? What are some of your favorite fibers to add? What recommendations do you have for those who have trouble with fiber (gas, pain, those using FODMAP diets) Can SCFA be taken orally? Do you recommend taking butyrate as a supplement too?

Is there a liquid version of antioxidants you suggested L- carnitine, lipoic acid, CoQ10 and taurine for those who have difficulty in swallowing pills, tablets, capsules? I see L-carnitine can be an effective therapy, but what about it's effect on GI microbes and TMAO production? What are recommendations for amounts of L-carnitine, lipoic acid, taurine, Zinc (and does it need to be balenaced with copper) Dosages for the key supplements for mucin production?

10-15 gm glutamine per day seems like a lot, any concerns of it becoming excitatory? How long should that regimen be continued? Glutamine can make some people very excitable and anxious, right?

I didn't see "exercise" listed - but I think it's also a beneficial factor. Any comments?

Who are the companies you have seen good success with EMF blocking devices? There are so many out there it is hard to know who is reputable

Can you explain why the glucose is worsened in the test group vs the placebo group on the slide you showed towards the end. The one showing the difference between 30 days of probiotic treatment vs placebo in the college students.

I have a client with dysbiosis who tried the MegaSporeBiotic and said it didn't agree with her. Any ideas as to how I can convince her to try it again?
