Annual Review 2016 Easy-read version David in his place of work, Sheffield Town Hall This report tells you about how we support people and the work we did in the last year

Affinity Trust is 25 years old Helping people to live how they want to I am Leo Sowerby, the Chief Executive of Affinity Trust. This year Affi nity Trust has been supporting people with learning disabilities for 25 years. We always look at different ways to give people the best support. CQC says our services are good quality. We would like to support more people with learning disabilities to live how they want to. We fi nd different ways to do this. We have opened two new services that give the right support for people who have been living in hospital and want to move into their own home. We think it is very important for everyone with a learning disability to choose what they want to do. Please read on to hear about some of the people we support. Leo with some of the people we support Affinity Trust believes that anyone with a learning disability can be supported to live in their own home in the community. This is Ruth. Ruth used to live in a hospital where there were lots of rules. Ruth was not able to choose what she wanted to do and this made her angry and upset sometimes. Ruth in her new home Affi nity Trust helped Ruth to move into her own home. Her Support Workers have had special training about Ruth s needs and how to support her to get used to where she lives now. Ruth is very happy in her new home. She does not get upset or angry very much. Ruth really likes being able to choose what she wants to do. She has been on holiday, likes to cook her own food, and goes cycling with her Support Worker. 2 3

Meet Anita Going to work Affinity Trust supports people who have other needs as well as a learning disability, like Anita who uses a wheelchair. Anita has a lot of needs and does not use words to say what she wants. The people who support her have been trained to understand what Anita wants. Affi nity Trust works hard to make sure that Anita can still do what she would like to. We make sure people with higher needs get good support, like everyone else. Anita likes to joke and laugh. She is going to college. She has learned fl ower arranging and food preparation. She will do a music course soon. Anita is supported to spend time with friends. Anita Many people we support want a job. We think that they should be able to work if they would like to, so we support them to fi nd a job. Reece wanted a job. Affi nity Trust supported him to fi nd out about lots of different jobs. He found a job that he wanted to do in a hospital. At fi rst, it did not seem that Reece would be able to get a job he liked. His support team worked hard to make sure that he did not give up. Reece got the job he wanted in a hospital. He says: I really like my job Reece and being on the staff. Reece liked going to the Christmas Party with his friends at work. 4 5

Affinity Trust has its 25th Birthday The people who work for Affinity Trust This year Affinity Trust is 25 years old. When we started we supported 6 people with learning disabilities and had 5 staff. Now we support 900 people with learning disabilities and have 2000 staff. Andrew cuts the cake There were some parties to celebrate that we have grown bigger and support more people with learning disabilities. People said how much they like Affi nity Trust: My staff listen to what I want. Support is going good. Paul I enjoy working at the shop and I enjoy going to college too I am doing things that I like doing. Emily I am so proud to lead Affi nity Trust as we celebrate 25 years of supporting people with learning disabilities. Leo, Chief Executive We train our staff to give the best support to people with learning disabilities. Our staff support people to try new things and to be more independent if they want to be. We make sure that they know how to support each person in the way that they want to be supported. They really care about the people they support. Ajitha works for Affi nity Trust. She really enjoys supporting people to do what they choose and seeing them learn new things. 6 7 Ajitha

Trying new things Supporting more people Affinity Trust thinks that the people we support should really enjoy life and have a chance to try things that interest them. Score Community Opportunities helped organise the community games this year. Over 100 people with learning disabilities did sports like football, Zumba, and Tai Chi. They had fun. Ben goes to the Leeds Opportunities services. He enjoys making benches and other things using wood. People like the things Ben makes and some wanted to buy them. Ben uses the money from selling the things he makes to buy more tools and wood so he can make more furniture. Ben More people are choosing us to support them because they like the way we work with them. We now support 900 people with learning disabilities. This year, we started to support people in Oxfordshire. We now support 41 people there. We will be supporting 20 people in Portsmouth to move from residential care homes to supported living. We started supporting people in Northamptonshire and Wolverhampton. David enjoys gardening 8 9

Family and friends are important Affinity Trust uses all the money it gets to make sure we have good services We know that family and friends are important to the people we support. Affi nity Trust listens to the people we support, their family, and friends. Paul s parents are pleased with Paul s support from Affi nity Trust. They like how we work with them and Paul to fi nd the right support for him. We support Paul to go to college and go horse riding, which he loves. Paul loves horses Paul s dad says: Thanks to Affi nity Trust Paul is happy. That means so much to both his mother and me. In 2016 Affinity Trust was paid 45 million pounds to provide support. This is over 2.5 million pounds more than what we were paid the year before. We look after our money well. Affi nity Trust has grown because lots of people like the way we support them. We hope to support more people in the future. Malcolm Maggie and Jo Malcolm Thomas is our Chairman. He says thank you to all the staff for their hard work. Playing the parachute game 10 11 Vincent
