Subscriber Connection Manage Your Benefits Online Easily print ID cards, view benefits and eligibility, access member discounts and more through Delta Dental s Subscriber Connection member portal. The Subscriber Connection portal can be accessed at DeltaDentalKS.com/Sprint. Click on the Log in to the Subscriber Connection button. Enter your username and password and click Login. View Your Benefits & Eligibility Online Access your benefit information including a list of eligible dependents, you and your families annual maximum, your dental services covered, your portion of payment (coinsurance) and deductible and more through the Subscriber Connection member portal accessed at DeltaDentalKS.com/Sprint. View Your Claims and Explanation of Benefits (EOBs) Find out the status of a claim, including payment date and how the claim paid through the Subscriber Connection member portal accessed at DeltaDentalKS.com/Sprint. You also have the ability to look at past EOBs - or go green by signing up to receive EOBs by email instead of through the mail. View Your Claims and Explanation of Benefits (EOBs) - Continued STEP 4 As a Sprint employee, you most likely receive your Explanation of Benefits* (EOBs) electronically instead of by US mail. To update your email address or change the way you receive your EOB, from the Explanation of Benefits button, click on Go Green -- Explanation of Benefits Online. STEP 4 *Explanation of Benefits includes information on a summary of your claims, out of pocket expenses and what your co-pay will be. These are sent via mail or email whenever a claim is submitted. EOBs are NOT bills. STEP 5 STEP 5 Edit your profile information by changing the dropdown for How do you prefer to receive your explanation of benefits (EOB) to Online or By US Mail. Enter your email address and a secondary email address. Click Save. STEP 6 To view yearly EOB summaries, from the Claims & EOBs tab, click Yearly EOB. Locate a Dentist Find a Dentist Near You Easily find an in-network dentist on our website or mobile app. SEARCH FOR A DENTIST ONLINE From DeltaDentalKS.com/Sprint, click on Find a Dentist. You will be brought to a page where you can search for a dentist by: your location, proximity, network/ plan*, dentist name, dentist specialty and more! Once you have selected your search criteria, click Submit. *Your savings are the greatest when you visit a dentist in the Delta Dental PPO SM network. You MUST utilize a Delta Dental PPO dentist if you are enrolled in the Sprint Basic dental plan or no services will be covered. You will see a list of dentists that fit your search criteria. Your dentist list will also provide maps and travel directions to the selected dental offices. You can print, save as a PDF or email your list of selected dentists to yourself. Access your ID card Two Ways to Access Your ID Card Easily print your ID card through the Subscriber Connection member portal accessed through DeltaDentalKS.com/Sprint, or download the Delta Dental mobile app and use your mobile ID card. ACCESS YOUR ID CARD ONLINE: STEP 1 Log in to the Subscriber Connection through DeltaDentalKS.com/Sprint. SUBSCRIBER NAME Subscriber Number GROUP NAME Group Number Once logged in, click on the ID Cards button. STEP 1 STEP 2 Click the Print icon in the upper right hand corner. A PDF will appear that you can print, or download and save the wallet sized ID card. Please note that the card shown online will show only the employee s (primary subscriber s) name. This card will also serve as identification for all eligible dependents. STEP 3 Simply cut out the card along the dashed line and present the card to your dentist at your visit, or save a PDF file of your card for future use. STEP 2
Access Your ID Card - Continued ACCESS YOUR ID CARD ON THE MOBILE APP: STEP 1 Download the mobile app to your device. Delta Dental s mobile app is available for smartphones and tablets using ios (Apple) or Android. To download the app on your device, visit the Apple App Store or Google Play and search for Delta Dental. STEP 2 Log in to the mobile app using the same username and password used to log in to the Subscriber Connection member portal. If you haven t registered yet, there is a link on the home screen of the app to register for an account. You will have the option to use/set up Touch ID depending on your phone s capabilities (requires logging in with username/password to set up). STEP 3 Once you re logged in, your mobile ID card will be available immediately after the login. You can then tap on the ID card image to bring up the options to: save your ID card to your home screen of the app (to access before the login), email the card to your dentist or dependents, or add your ID card to your Google Wallet or Apple Wallet. ANSWERS TO DENTIST OFFICE QUESTIONS Q: May I visit any dentist I wish for treatment? What if my dentist doesn t participate with Delta Dental? A: For the Sprint Premium Plan, you may visit the dentist of your choice, however your savings are the greatest when you visit a dentist in the Delta Dental PPO network. You MUST utilize a Delta Dental PPO dentist if you are enrolled in the Sprint Basic Plan or no services will be covered. Q: How do you handle claims submitted by a nonparticipating dentist? A: Please note that if you visit a non-participating dentist, you may be required to submit a claim form for the services rendered and possibly pay the entire amount up front. In all instances, when a nonparticipating dentist renders services, any payment made by Delta Dental will be made directly to you, the subscriber. A claim form can be found at the bottom of page at DeltaDentalKS.com/Sprint. Q: How can I access my ID card? A: Easily use your mobile ID card through Delta Dental s mobile app - simply log in to view your ID card, show it at the dental office, or email it to a dependent or dentist. A: You most likely received an Explanation of Benefits (EOB) statement. This statement is not a bill, it explains what services your dentist provided and how Delta Dental processed and paid for the services. Q: What are predeterminations, and are they mandatory? A: A predetermination of benefits allows you to know in advance what procedures are covered, the amount your plan will pay toward treatment and your financial responsibility. Some groups, and some conditions, require a predetermination of benefits before treatment is performed. Treatment plans that involve prosthodontic services, orthodontic services, individual crowns (except stainless steel), gold restorations, surgical periodontics, endodontics and oral surgery, except for simple extraction of a single tooth, should be submitted to Delta Dental for predetermination of benefits. Even if it s not required, Delta Dental encourages predeterminations for extensive treatments or if you visit a non-participating dentist. If submitted by a participating dentist, there is no charge for a predetermination and it is valid for ninety (90) days.

Q: I have a dependent who is fully disabled, how long is he/she covered under my policy? A: As long as the individual remains disabled, fully dependent on you for support and unmarried, he or she can remain a dependent on your insurance policy. Written proof is required from the attending physician to verify the dependent s disability. Q: Can I add family members to my dental coverage at any time? A: No, dependent family members can only be added during your annual enrollment period or when a qualifying event occurs. Q: What does Delta Dental consider a qualifying event? A: The following are considered qualifying events: birth/legal custody/adoption, death, loss of other dental insurance coverage or marriage/divorce. Q: When am I eligible for dental coverage? Does Delta Dental have waiting periods for services? A: To determine if you have a waiting period, you can check the Benefits & Eligibility section of the Subscriber Connection or contact our Customer Service department at 866.913.3375. Q: Can I elect coverage during a contract year? A: Coverage can only be elected at your employer s annual enrollment date or if a qualifying event occurs. Q: I m covered under two dental plans, how is my coverage handled? A: If you and your family are covered by both Delta Dental and another dental carrier or a medical plan that offers dental coverage - Delta Dental coordinates with the other carrier. Generally, if you are covered as an employee and also as a dependent of an employee at another company, the coverage through your employer is primary. Children covered by parents who work for different employers are usually primary under the plan of the parent whose birthday occurs first in a calendar year (not necessarily the oldest parent). In determining coverage, total payments from both carriers cannot exceed 100 percent of the approved fee for the service. Please note that some groups have specified a carve-out clause in their dental programs that might limit a secondary carrier s payment. A: If you re a member of Delta Dental, you do not need a referral to receive care from a specialist. However, we strongly encourage you to use the services of a Delta Dental participating specialist to maximize your benefit coverage. Q: How do I check to see if I am eligible for coverage? Q: When does coverage begin after a qualifying event occurs? A: Coverage begins at the first of the month following the date of the qualifying event. Frequently Asked Questions ANSWERS TO CLAIM QUESTIONS Q: I had a tooth surgically extracted (pulled) and the insurance claim was filed with my medical insurance. Why doesn t my dental insurance cover the claim? A: Depending on your group coverage, claims for surgical extraction of wisdom teeth may need to first be submitted to your medical insurance carrier, and then to your dental insurance. Some medical plans pay up to 100 percent of this procedure, so filing this type of extraction with your medical insurance could be of benefit to you. If there are any remaining charges after the medical insurance carrier has paid, your dental plan may cover the remainder or a portion of the remaining balance. Contact our Customer Service department at 866.913.3375 for specific details. Q: Why does Delta Dental reimburse orthodontic payments monthly instead of paying the entire amount up front? A: Orthodontic payments are distributed throughout the period of time services are rendered. This eliminates problems and confusion if the treatment is completed early, if the member s coverage is terminated, if the group terminates or if the dependent reaches the age limit for orthodontic benefits or for dependent coverage. Delta Dental s policy is to pay only for completed services, so once the monthly visit is completed, a payment will follow. Q: Why was my exam not covered when my dentist referred me to a specialist? A: According to your group contract, examinations are covered benefits that may be subject to frequency limitations. This is true whether the examination is performed by a general dentist or a specialist. Q: I had individual X-rays taken and Delta Dental paid for a full mouth X-ray, why? A: Delta Dental s policy states the fee for individual (periapical) or bitewing X-rays cannot be higher than the fee for a full-mouth X-ray series. If this occurs, Delta Dental will pay the amount for a fullmouth series. ANSWERS TO DEPENDENT QUESTIONS Q: My child has braces and we just got switched to Delta Dental - are our orthodontic payments covered under our Delta Dental insurance? A: Depending on your particular circumstances, Delta Dental may or may not assume coverage of orthodontic benefits.
