Fees Guide - page 5 All our crowns, bridges and dentures are made using our dental laboratories' premier service ensuring the highest level of materials and technical expertise is used in their construction. We require at least 48 hours notice prior to rescheduling or cancelling any appointment. Late arrival for any appointment will be regarded as 'Failed to Arrive' for the appointment. Any patient who arrives more than 10 minutes late for an appointment will be regarded as a 'Late Arrival.' All patients are required to arrive at least 15 minutes prior to their appointment time to fill in a medical history and patient details forms. In an event of a 'Late Arrival', 'Late Cancellation' or 'Failure to Arrive' or any similar description, the practice will retain the deposit to defray expenses. In an event that a deposit has not been collected, the patient will be asked to pay the equivalent fees. All patients using a Voucher or similar must attend at least 15 minutes earlier than their appointment time to avoid loss of voucher entitlement. A full non discounted scale of fees will be charged for patients who have a dental plan or insurance or any similar reimbursement scheme. The practice will not be held responsible for any short payments or non-reimbursement to patients from their dental insurance or plans.
