Easy Read Kent Joint Commissioning Action Plan For Adults with Autism and or ADHD (2017 / 2021) Action Plan The plan was developed to address the needs identified from the Kent Autism Strategy and Joint Needs Assessment for Adults with Autism and or ADHD (Attention Deficit Hyperactivity Disorder). The Plan includes joint working with Health Commissioners across Kent Commissioning means arranging and paying for someone to run a service. This plan tells you about the 10 areas of work we will try to make improvements in for people with Autism and or ADHD in Kent and: What we need to do When it needs to be done by. There is a list of difficult words at the end. The plan says how we will: Improve services for people with Autism and or ADHD through commissioning the right services. 1
What needs to be done? This plan has 10 subjects. We list each subject and what needs to be done and when. We say how well we are doing by giving it a rating. We rate these with 3 colours. These are: Green - means it is okay. Amber - means it needs to be checked. Red - means we need to take action. We will check what has been done at the end of March 2018. 2
The 10 subject areas we are looking at are: 1. Improving Diagnosis, Assessment and Support: There are different healthcare organisations providing this service across Kent. We aim to join health and social care together so people receive better treatment. 2. Transforming Leadership, Planning and Commissioning: We want to improve the way we commission services to deliver the best outcomes for the people who receive them. We aim to work closely with health commissioners to do this. 3. Including Co-Production: Co-Production means people who use services have a say in how those services are designed and delivered. We aim to ensure people are involved in any service developments in the future. 4. Life facing Transitions: life transitions from teenager to adult can be a difficult time for people. We aim to ensure services support people through these changes. 3
5. Training and Further Education: Everyone should have access to training and further education. We aim to improve access and support by working with other organisations and educational services. 6. Employment: Working and being in some form of employment can be very rewarding; we aim to provide a range of support to increase the numbers of people in employment paid or voluntary. 7. Improving Housing, Care and Support We aim to undertake a needs assessment for housing. This means we will identify what people need from housing, care and support in order to live independently. 8. Development of the Workforce: The workforce means the people who deliver services. We aim to make sure there is enough knowledgeable and experienced staff to support people. 9. Criminal Justice System: We know that sometimes people can get into trouble with the police and or spend time in prison. We aim to understand how this impacts on people and how their experience can be improved through working with other organisations within the criminal justice system. 4
10. Carers: Carers are people who care for another person. We want to understand carers and what their needs are in order to support them. The next section goes through these 10 subjects and what will be looked at. 5

Appropriate Post Diagnostic Support Services are accessible, are non-dependent, inclusive / with throughput and include a system wide approach Services integrate specialist treatment options for coexisting conditions e.g. Mental Health within the Multidisciplinary Team, such as Psychosocial & Biomedical (pharmacological, physical and dietary). Autism Enablement Services / adapted Cognitive Behavioural provision will reduce crisis situations, improve outcomes for people and reduce the costs of expensive packages of care (NICE CG142). (NICE CG142). 1.7 To provide specialist information, advice and training service for mainstream professionals 1.8 Ensure Autism / ADHD Planning / Services align with Kent & Medway Transforming Care Programme All health and social care professionals providing care and support for adults with autism are provided with access to training in order to have a broad understanding of the nature, development and course of autism, the impact on personal, social, educational and occupational functioning (NICE CG 142). 7. Improving Housing, Care and Support We aim to undertake a needs assessment for housing. This means we will identify what people need from housing, care and support in order to live independently.
