Easy Read Kent Joint Commissioning Action Plan For Adults with Autism and or ADHD (2017 / 2021) Action Plan The plan was developed to address the needs identified from the Kent Autism Strategy and Joint Needs Assessment for Adults with Autism and or ADHD (Attention Deficit Hyperactivity Disorder). The Plan includes joint working with Health Commissioners across Kent Commissioning means arranging and paying for someone to run a service. This plan tells you about the 10 areas of work we will try to make improvements in for people with Autism and or ADHD in Kent and: What we need to do When it needs to be done by. There is a list of difficult words at the end. The plan says how we will: Improve services for people with Autism and or ADHD through commissioning the right services. 1

What needs to be done? This plan has 10 subjects. We list each subject and what needs to be done and when. We say how well we are doing by giving it a rating. We rate these with 3 colours. These are: Green - means it is okay. Amber - means it needs to be checked. Red - means we need to take action. We will check what has been done at the end of March 2018. 2

The 10 subject areas we are looking at are: 1. Improving Diagnosis, Assessment and Support: There are different healthcare organisations providing this service across Kent. We aim to join health and social care together so people receive better treatment. 2. Transforming Leadership, Planning and Commissioning: We want to improve the way we commission services to deliver the best outcomes for the people who receive them. We aim to work closely with health commissioners to do this. 3. Including Co-Production: Co-Production means people who use services have a say in how those services are designed and delivered. We aim to ensure people are involved in any service developments in the future. 4. Life facing Transitions: life transitions from teenager to adult can be a difficult time for people. We aim to ensure services support people through these changes. 3

5. Training and Further Education: Everyone should have access to training and further education. We aim to improve access and support by working with other organisations and educational services. 6. Employment: Working and being in some form of employment can be very rewarding; we aim to provide a range of support to increase the numbers of people in employment paid or voluntary. 7. Improving Housing, Care and Support We aim to undertake a needs assessment for housing. This means we will identify what people need from housing, care and support in order to live independently. 8. Development of the Workforce: The workforce means the people who deliver services. We aim to make sure there is enough knowledgeable and experienced staff to support people. 9. Criminal Justice System: We know that sometimes people can get into trouble with the police and or spend time in prison. We aim to understand how this impacts on people and how their experience can be improved through working with other organisations within the criminal justice system. 4

10. Carers: Carers are people who care for another person. We want to understand carers and what their needs are in order to support them. The next section goes through these 10 subjects and what will be looked at. 5

1.3 To develop multidisciplinary diagnosis and assessment including those with complex needs 1.4 To develop post diagnostic support services 1.5 To develop provision of specialist interventions 1.6 To develop positive behaviour management service Business Case for consideration - an integrated Kent Multidisciplinary Team (aligns with All-age Neurodevelopmental Pathway from 16+). Appropriate Post Diagnostic Support Services are accessible, are non-dependent, inclusive / with throughput and include a system wide approach Services integrate specialist treatment options for coexisting conditions e.g. Mental Health within the Multidisciplinary Team, such as Psychosocial & Biomedical (pharmacological, physical and dietary). Autism Enablement Services / adapted Cognitive Behavioural provision will reduce crisis situations, improve outcomes for people and reduce the costs of expensive packages of care (NICE CG142). Responsive positive behaviour management service is available to provide support for complex and challenging behaviour (Mansell, Dec 2017 April June 2018 April - June 2018 April - June 2018 7

(NICE CG142). 1.7 To provide specialist information, advice and training service for mainstream professionals 1.8 Ensure Autism / ADHD Planning / Services align with Kent & Medway Transforming Care Programme All health and social care professionals providing care and support for adults with autism are provided with access to training in order to have a broad understanding of the nature, development and course of autism, the impact on personal, social, educational and occupational functioning (NICE CG 142). Autism/ADHD community-based planning / services align with and support the Kent & Medway Transforming Care Programme target trajectories for reduction in inpatient out of area placements April - June 2018 Dec 2017 2. Transforming Leadership, Planning and Commissioning: We want to improve the way we commission services to deliver the best outcomes for the people who receive them. We aim to work closely with health commissioners to do this. 8

2.3 To extend the membership of the Autism Collaborative, and to develop the group into an Autism Action Alliance and implement the Action Plan The Autism Action Alliance has core and associate membership and acts as an influential group to transform system wide June April 2018 2.4 To develop joint integrated commissioning plans between Local Authorities and Clinical Commissioning Groups and review these annually. Strategic integrated commissioning plans are in place for Autism /ADHD services within Kent June April 2018 3. Including Co-Production: Co-Production means people who use services have a say in how those services are designed and delivered. We aim to ensure people are involved in any service developments in the future. 10

What needs to be done and why What will be done Date done by Status colour Evidence 3.1 To ensure people with autism and their families and carers are involved and have their opinions heard A service user and carer group is formed and has membership on the Autism Action Alliance / is consulted on service planning April June 2018 3.2 To improve the understanding of Commissioners of the experience of people with autism (including the effectiveness of services and the outcomes achieved for individuals). This will include feedback from people with autism and their families and carers The service user and carer group has influence in strategic planning of services and service reviews / audits. Commissioners of services will include the group on codesign / coproduction of procurement of services for people with Autism / ADHD April June 2018 4. 5. Training and Further Education: Everyone should have access to training and further education. We aim to improve access and support by working with other organisations and educational services. 5.2 To establish tracking of people subject to Educational Health Care Plans above statutory school age to remain alert of those Not in Education, Employment or Training and whose destinations are not known An educational register will be developed to track additional needs progress for people with Autism / ADHD Sept 2018 5.3 To commission outcomes for 0-25 Speech and Language Therapy Scope and consider commissioning options for Speech and Language Therapy Sept 2018 5.4 To ensure educational establishments provide autism friendly support and environments and promote positive outcomes Guidelines will encourage best practice within educational settings Sept 2018 5.5 To ensure fewer young people drop out of college without qualifications Root cause analysis of individual cases will lead onto development of the guide for best practice within educational provisions for people with Autism / ADHD Sept 2018 14

6. Employment: Working and being in some form of employment can be very rewarding; we aim to provide a range of support to increase the numbers of people in employment paid or voluntary. 7. Improving Housing, Care and Support We aim to undertake a needs assessment for housing. This means we will identify what people need from housing, care and support in order to live independently. What needs to be done and why What will be done Date done by Status colour Evidence 7.1 To undertake a housing needs assessment of people with autism Housing needs assessment identifies and informs future commissioning intentions for accommodation needs in line with Kent housing strategy April 2018 16

7.2 To develop a diverse housing market that provides various levels of skilled support Varied skilled support models within housing ensure service needs are met April 2018 7.3 To work with the market to develop a range of housing related support services The models of related housing support will outline related care / support needs to include independent living April 2018 7.4 To ensure social care, health and educational support services provide skilled, effective, autism friendly support, using an enabling and empowering approach 7.5 To develop specialist services that can provide assessment and treatment, ideally within individuals current home or, where necessary, in small local specialist units implementing positive behaviour support and the SPELL framework System wide autism awareness training will promote inclusion and effective support, focused on independence Specialist interventions will be flexible to suit the individuals needs wherever possible. 8. Development of the Workforce: The workforce means the people who deliver services. We aim to make sure there is enough knowledgeable and experienced staff to support people. What needs to be done and why What will be done Date done by Status colour Evidence 8.1 To ensure a range in the level of training for autism is identified as a priority within local workforce development plans across relevant agencies and providers Training packages will cater for diverse market and equip staff to understand better the needs of people with autism- training aligns with Kent s workforce training strategy April 2018-2021 18

8.2 To work with other organisations and agencies, including those in educational settings, the criminal justice system, employment support services, and the voluntary sector, to promote the provision and take-up of autism awareness training among their staff training among their staff Training programme for autism awareness will target a diverse workforce across health, social care, educational, welfare and law enforcement sectors. Raising awareness and promotion of reasonable adjustments for people with autism April 2018-2021 9.
