Factors Influencing Health and Education! Healthy communities and a healthy environment are critical to individual health and wellness, but there is a disparity in what we fund.
Minnesota Department of Health (MDH) and DHS Accountable Communities for Health Grants $4.0 Million for 12 new Accountable Communities for Health Encourage clinical and community partnerships that provide patient-centered coordinated care for the whole person. Address health problems within communities by coordinating support systems to keep people healthy Accountable Communities for Health goal is to bring together a broad range of community partners, including local public health, behavioral health, social services, long term care, primary care, schools and other organizations that contribute to a person s health.

Is there a need to focus on student health? 2012-2013 Anoka-Hennepin High School Health Screening activities that included 2,200 students had the following results: 29% of students had a borderline blood pressure 17% of students had an Abnormal Blood Pressure 19% of students had a Total Cholesterol >170mg / dl 6% of students had a Non-HDL s >145mg / dl 28% of female students had a have BMI s >24 (considered overweight/obese) 35% of male students had a have BMI s >23 (considered overweight/obese)
Build on Existing 19 Year Partnership Northwest Metro Healthy Student Partnership Anoka-Hennepin Schools and Allina Health have partnered for 19 years to provide basic biometric screening to students in health class Over 2,200 high school students screened every year at all Anoka-Hennepin High Schools
