Introduction Solid organ transplantation is one of the most relevant advances of modern medicine of the last 60 years. The field has evolved from the pioneering era to the current situation where organ transplantation may be considered a routinary therapeutic option, lifesaving, reliable and cost-effective when compared with alternative treatments of acute and chronic end organ diseases. However, transplantation is a complex, multidisciplinary specialty. Modern immunosuppressive therapy has improved patient and graft outcomes, but, this places transplanted patients at risk for a variety of infectious complications, which remain in fact a significant cause of morbidity and mortality. Clinical organ transplantation poses significant recipient risks, particularly donor transmitted infections and malignancies, which must be weighed against the benefits of receiving a transplant in the context of organ shortages. Regional and national governments have developed regulations to ensure safety from the moment of donor identification to postoperative transplant recipient follow-up. Management of infectious complications in solid organ transplant recipients is complex and rapidly evolving. Transplant surgeons and physicians working in transplant centers, together with those working in non transplant centers, require a continuous update in order to guarantee an adequate management of this growing highly complex population. I am pleased to present the program of the Fourth International Congress on Infections in Organ Transplantation which will be held in Stresa, Italy, on May 8-10, 2013. The program comprises multiple sessions dealing with various aspects of infectious complications following solid organ transplantation, including donortransmitted diseases. Outstanding national and international experts from CNT, WHO and CDC will present the most updated data on the hottest topics. This congress will be a unique opportunity for physicians and surgeons interested in the field. No other scientific event cover a so extensive number of topics. This year format will try to be more interactive with clinical case presentations and discussion. All previous editions have been highly successful and we are convinced that this year format will be even better, taking advantage of the wonderful location of Stresa on lake Maggiore.
