Health Maintenance tab is located on the left side menu. Health Maintenance will assist with Meaningful Use, Stage 2 objectives. Meaningful Use, objective #6 states the following: Use clinical support to improve performance on highpriority conditions. (Simply put, we document when certain health screenings were performed in the past or when they are due in the future). Meaningful Use, objective #12 states the following: Use relevant information to identify patients who should receive reminders for preventive/follow up care and send these reminders to the patients. Health Maintenance Overview Once the tab is accessed, it will show what HM expectations are being generated for your patient based on: Age, Sex, Diagnosis and Problems. Healthy Adult expectations include the following screenings or vaccines: Abdominal Aortic Aneurysm Screening Bone Density Screening Breast Cancer Screening Cervical Cancer Screening Colorectal Cancer Screening HPV Vaccine Influenza Vaccine Pneumococcal Vaccine Tetanus Vaccine Zoster Vaccine Due date will show today s date if an expectation has not been satisfied yet. You can manually change the due date by clicking the field and entering a new due date. Default frequency will show. You can manually change the frequency by clicking the field and entering a different frequency. Each Expectation will have varied options to click, depending on what you need to do during the visit. How to Place an Order from Health Maintenance Order hyperlinks will be at the front of the expectation and description will state *ORDER* You are not required to place screening or vaccine orders directly from Health Maintenance. You can still place these orders from your Powernote as you do now. The expectations will still be satisfied. 1. Click the Sign Orders button at bottom right which will take you to the normal Orders for Signature preview screen. 2. Fill in the required details for each order then sign the orders. o All vaccine orders will be sent to the nurse task list so they can chart the vaccine administration as usual. Satisfier Options Satisfier Options will be in the following 6 categories: Normal, Abnormal, Postponed, Cancel Permanently, Historical Vaccination and Received Elsewhere. Each category has its own set of satisfier reasons. Page 2 of 8
Manually Adding Expectations If you want to manually add an additional expectation to the list click on the +Add button. Click on the + next to Healthy Adult which will expand the selections. Checked selections are already on the list. Unchecked selections can be manually added. You can also add your own expectations by selecting the Free Text tab In this example we manually added a yearly chest x-ray due to history of + TB Tine Test. Once you free text the information you desire, click add then click OK and the expectation will be added to Health Maintenance List for this patient. Page 3 of 8
Sorting Pending Expectations Rearrange the Pending Expectations by clicking Sort by option Documenting Historical Vaccinations To show patient has already had a particular vaccination at an earlier time or it was performed elsewhere click Historical Vaccination 1. Choose Reason. 2. Select date vaccine given. 3. Type Comments if needed. 4. Select OK. Please Note Documenting historical vaccinations here will NOT add the vaccination to the Immunization tab. You will have to document in the Immunization tab as a separate workflow. However, if you document in the Immunization tab FIRST as a Historical vaccine, it will complete the vaccination in Health Maintenance. Page 4 of 8
Health Maintenance and Powernotes Health Maintenance will pull forward into your Powernote IF you click the box within the Auto-Populate window. Page 5 of 8
Invitations/Reminders Invitation- Patient will receive an invitation letter to call your office to schedule an appointment for either a screening or a vaccination. Reminder- If patient does not have the screening or vaccination completed by a certain timeframe, they will then receive a reminder letter that they missed getting this completed and will be encouraged once again to call the office to schedule an appointment. To generate an Invitation List follow these steps: 1. Click on the Invitations link from top row of icons. 2. Click Customize View 3. Click the Add window and search for your clinic. Click the box next to the clinic name and then click OK. 4. Your clinic will now be added into the location box. Click OK again to close the box. 5. Choose the Program (the Invitation you want to send) from the dropdown list. Page 6 of 8
6. Choose your date frame. Once date frame is chosen, the report will run. 7. Click Generate Communications from top row of Invitations icons. If patient is already signed up for My Team Care, the invitation/reminder will automatically be sent via the patient portal. If patient is not signed up for My Team Care, the invitation/reminder will automatically print. Interesting Facts We suggest that 1 assigned person (per clinic) be responsible for generating Invitations/Reminders each month on a routine basis. o Best scenario would be to generate the communication every 3 rd Monday of each month and set your before date to be the last day of the next month. (Example: On July 21 you generate communication for everything before August 31, 2014) Page 7 of 8
You should do this workflow for both the Patients tab and the Patients Without History Tab. Tracking Status will change from Qualified to Invitation Sent. If patient comes to the clinic and has the screening or vaccination completed, it will not show up on your list again until the system says it is due again. If patient does NOT come to the clinic within a specific timeframe then they will eventually roll around to the list again and the next time you generate the communication they will receive a reminder letter.
