GPs, Pharmacists, LPC and CCGs Background information: MoreLife deliver integrated healthy lifestyle services as part of OneLife in Suffolk. This includes Smoking Cessation and Tobacco Harm Reduction. OneLife Suffolk deliver smoking cessation group and 1:1 programmes for smokers of tobacco throughout Suffolk. Our specialist service takes a psychological approach to smoking cessation, including behavioural change, treatment advice and provision and support. The programme mainly consists of delivering regular group interventions to clients. The post holder will also need to be aware of and committed to the vision, policies and practices of the organisation. 1
Purpose of job: The job has five distinct purposes these are: 1. Develop, nurture and support robust working relationships with key partner agencies delivering Smoking Cessation via the delivery of service promotion, mentoring and training to health professionals who deliver smoking cessation services or have regular contact with tobacco users. 2. To generate referrals in to the Stop Smoking service this includes ensuring delivery against individual performance targets. 3. Promote the Stop Smoking service at local health & wellbeing and community events along with local & national stop smoking campaigns 4. Provide ongoing client support via the facilitation of follow-up smoking cessation calls on behalf of partner agencies when required. 5. Provide holiday cover for Stop Smoking Practitioners when required. Key responsibilities: Partner Agencies: 1. Proactively approach Primary Care providers (GP practices and Pharmacies) to promote the Stop Smoking service and encourage health professionals to deliver paid-for smoking cessation services via the sharing of bespoke and localised data (e.g. the Cost Burden of Disease information). 2. Secure attendance once per quarter at Primary Care team meetings within the locality to promote the Stop Smoking service. 3. Secure attendance at other appropriate agencies (e.g. charities and community groups) within locality to promote the Stop Smoking service. 4. Work closely with the Stop Smoking Training Lead to deliver training to health professionals. 5. Ensure Primary Care partner compliance with data reporting undertaking regular audits and providing support when required. 6. Provide health professionals offering smoking cessation services with ongoing training opportunities, mentoring and ad-hoc support. 2
7. Provide both on-site and remote support regarding smoking cessation equipment (e.g. CO monitor calibration) and stock ordering. Referrals: 1. Proactively approach health professionals (e.g. GPs, Pharmacists, Community Nurses, Health Visitors, Dentists, Hospital staff), other social care providers (e.g. Charities, refugee agencies, Social Care Workers) and community groups (e.g. BAME groups, LGBT groups) to promote the Stop Smoking service and develop robust referral pathways. 2. Work with the Stop Smoking Service Management Team to develop and document robust referral pathways. 3. Secure a minimum number of referrals in to the Stop Smoking team each month. 4. Monitor referrals on a monthly basis to identify which providers / services require further support to improve referrals to the Stop Smoking service. 5. Work alongside the Marketing Team to harness Social Media activity for selfreferral generation and service promotion with a view to increasing both service and brand awareness. Community Promotion: 1. Represent the Stop Smoking service at community events attended by OneLife Suffolk (e.g. Summer Fayres, Stoptober events, Wellbeing events, Workplace health events, carnivals). 2. Attend community group meetings where appropriate to promote the Stop Smoking service. 3. Nurture and maintain relationships with community leaders to promote the Stop Smoking service where appropriate (e.g. within Faith communities) and in collaboration with the Engagement Team. Ongoing Client Support: 1. Support partner agencies and the OneLife Suffolk Stop Smoking service by facilitating follow-up calls with clients (e.g. to check on current smoking status, secure 4 week quits, assist in relapse prevention). 3
2. Review past clients who have exited the service and proactively contact them to check on current smoking status (where the appropriate consent has been provided). 3. Assist in the delivery of 12-month follow-up calls. 4. Assist with the delivery on OneLife Suffolk Club maintenance meetings when required. Other duties to include: 1. Proactively follow up leads generated from marketing campaigns. 2. Use of initiative to identify and follow up opportunities with GP Practices/Pharmacies and Workplaces who are not already on the Provider database. 3. Manage the Provider database to a high degree of accuracy to ensure targeted marketing activity can take place to generate new business. 4. Work closely with the marketing team to achieve target numbers for smoking cessation. 5. Liaise effectively with all services within OneLife Suffolk to ensure customer requirements are met. 6. Contribute to the team performance by sharing and implementing Best Practise Ideas. General 1. To positively promote access to all of the OneLife Suffolk offer and the aims and objectives of the OneLife Suffolk service. 2. Work with local area Engagement Team Practitioners to support, organise and engage with local community groups as necessary to enhance engagement and to develop and facilitate networks and opportunities. 4
3. To ensure that a professional service and image is maintained at all times, thereby acting as a role model to all staff. 4. To support a leadership style which is underpinned by strongly held values around equality, diversity and openness; effectively builds and maintains relationships with team members. 5. To record, monitor and, with the Stop Smoking Manager, actively improve on KPIs. 6. Monitor own activity against targets, engagement work, campaigns and events, as well as referral numbers to ensure programmes and activities meet necessary needs of stakeholders, commissioners and demand. 7. Collate feedback from our stakeholders and service users. Support production of analytical reports with the focus on furthering the development of the programmes and team. 8. To keep accurate records of work. 9. To identify, reach and engage effectively with target groups/areas/communities in line with KPI requirements. 10. To attend team meetings and contribute to the performance improvement process and other meetings as requested and appropriate. This includes attending monthly 1:1 meetings with the Stop Smoking Training Lead. 11. Understand own contribution to performance targets / KPIs which will be set by the Stop Smoking Training & Liaison Lead and remain accountable for delivery. 12. To manage and organise own time and activities responding effectively and in a timely manner to all work. 13. Employee specification 1. A relevant qualification (undergraduate degree preferred) OR relevant and substantial work experience in any of the following: a. Health Promotion b. Education c. Psychology d. A related field or equivalent experience in a Stop Smoking related field 2. Level 2 trained Smoking Cessation Advisor (desirable but not essential). 3. Excellent interpersonal and communication skills. 4. Excellent Information Technology skills with the ability to rapidly acquire new skills. 5. Ability to use own initiative and work flexibly as part of a team to tight deadlines. In addition, an adaptable and flexible approach to work, including the ability to work evenings and weekends when required. 6.
