For section (3) Create your Security Question and Answer a. Please create a security question that will be asked of you should you forget your password. 6. For section (4) Enter your name a. Enter your first name and then your last name 7. For section (5) Enter your email address a. Enter your email address 8. For section (6) Gender, Ethnicity and Race a. Answer A through C 9. For section (7) CME/CMU credits a. Select No 10. For section (8) Course Survey a. Select if you would like to participate in a Survey regarding the course you take. Otherwise, select No 11. Once you have completed sections 1-8 on this page, click Submit (located at the bottom of the page) a. Select your Language Preference b. Type your Institutional email address (the email address assigned to you by CSI) c. Select your Gender d. Select your Highest Degree Completed e. Skip Employee Number f.
